Background. Hepatic epithelioid hemangioendothelioma is a rare malignant tumor of vascular origin with frequent multifocal appearance. Liver resection may cause tumor spread. Liver transplantation has been indicated for unresectable nodules. We hypothesized that adjuvant interferon treatment is effective to prevent metastasis after liver resection. We report a case of multifocal hepatic epithelioid hemangioendothelioma successfully treated with interferon pulse therapy and bilobar hepatic resection.Methodology. CT scan and magnetic resonance imaging diagnosed three nodules in the liver (segments IV, VI and VII). Histopathology and specific immunostaining of a percutaneous nodule biopsy confirmed the diagnosis of hepatic epithelioid hemangioendothelioma. The treatment protocol included daily interferon alpha 2b 9 weeks before and 1 week after resection of liver segments IV, VI and VII.Results. The postoperative outcome was complicated by a self-limited biliary fistula. The patient remains tumor free at 3 years after liver resection and currently enjoys excellent health.Conclusion. Interferon pulse therapy and hepatic resection was a good option to treat multifocal bilobar hepatic epithelioid hemangioendothelioma; it may prevent metastasis dissemination.

Background: Melanoacanthoma (MA) has been described in the oral mucosa as a solitary lesion or, occasionally, as multiple lesions. MA mainly affects dark skinned patients and grows rapidly, showing a plane or slightly raised appearance and a brown to black color. The differential diagnosis includes oral nevi, amalgam tattoos, and melanomas. We report here the case of a 58-year-old black woman who presented multiple pigmented lesions on the hard palate. Case presentation. Based on the differential diagnosis of melanoma, a punch biopsy (4 mm in diameter) was performed. The material was fixed in 10% formalin, embedded in paraffin, and stained with hematoxylin-eosin or submitted to immunohistochemical analysis. Immunohistochemistry using antibodies against protein S-100, melan-A, HMB-45, MCM-2, MCM-5, Ki-67 and geminin was performed. Immunohistochemical analysis revealed strong cytoplasmic immunoreactivity of dendritic melanocytes for proteinS-100, HMB-45 and melan-A.Positive staining for proliferative markers (MCM-2, MCM-5, Ki-67) was only observed in basal and suprabasal epithelial cells, confirming the reactive etiology of the lesion. The diagnosis was oral Melanoacanthoma (MA). Conclusion: The patient has been followed up for 30 months and shows no clinical alterations. MA should be included in the differential diagnosis of pigmented lesions of the oral cavity. © 2013 das Chagas e Silva de Carvalho et al.; licensee BioMed Central Ltd.

The human poliomavirus is the etiologic agente of Progressive multifocal leukoencephalopathy (PML), a disease characterized by focal lesions not expansives of the central nervous system that develops in imunocompromissed patients, specially people with aids. The main aim of the study was to evalute the prevalence of the JCV excretion in urine samples of patients with aids, without PML, to compare two JCV DNA detection techniques through of two diferents genomic regions and to evaluate the genotypic characterization of the positive samples. A total of 75 samples were colected in the Instituto de Infectologia Emílio Ribas, in Sao Paulo, Brazil, between may and november, 2009. To detect the JC virus it was made the DNA extraction and then the polimerase chain reaction (PCR). Firstly a fragment of 215 bp was amplified, which corresponds to the codifying gene of the strutural protein of de JC vírus capsid VP1. All the samples were later submitted to another PCR that uses a pair of primers complementaries to the early region of the JCV (T antigen) amplifying a fragment of 173 bp. Followed by the digestion of the amplified product with the restriction enzime BamH1, resulting in two smaller fragments (120 bp and 53 bp). The JC vírus was detected in 53 samples, for both techniques (70,7% for VP1 PCR, and the restriction enzime BamH1), 34/46 were men (73,9%) and 19/29 were women (65,5%). The JCV excretion was higher in individuals that were over 46 years old. Regarding the seven genotypes described in the literature, the ones that were more prevalent among the JC positive patients were 3B and 3A with 10 samples each (21,0%), the 2B with 9 samples (19,0%) and genotype 6, with six samples (13,0%). As in the brown patients as the white ones, the most prevalent genotype was 3B. In the present study it was observed a high prevalence of JCV DNA (70,7%) and the genotype 3 (43,0%)... Purpose: To assess visual outcomes and patient satisfaction following implantation of the Sulcoflex® multifocal intraocular lens (IOL; Rayner Intraocular Lenses Ltd., Hove, UK) in a procedure combining capsular bag lens implantation with sulcus placement of the Sulcoflex® IOL. Setting: Instituto de Oftalmologia de Assis, Assis, SP, Brazil. Methods: Cataract patients > 45 years, with hyperopia ≥ 1.50 D and potential acuity measurement ≥ 20/30 undergoing Sulcoflex® multifocal IOL implantation were included. Monocular and binocular uncorrected near and distance visual acuity (VA) were evaluated at five days, one month, and three months postoperatively. Contrast sensitivity and refraction were measured in a subset of patients three months postoperatively. Patient satisfaction was assessed one month postoperative. Results: This non-consecutive case series comprised 25 eyes of 13 patients. Eleven eyes (52%) had pre-existing retinal pathologies. Monocular distance VA improved significantly at all follow-up visits. At final follow-up, 88% of eyes had monocular uncorrected distance VA (UDVA) of at least 20/25 and 24% had monocular UDVA of 20/20. All eyes had binocular UDVA of at least 20/25, and 58% had binocular UDVA of 20/20. Monocular uncorrected near vision (UNVA) was J1 in 68% of eyes and all patients had binocular UNVA of J1. Of all eyes studied, 92% and 58% achieved a spherical equivalent within 1 D and −0.5 D, respectively. The majority of patients reported satisfaction with visual outcomes. Complications included a postoperative intraocular pressure spike in four eyes. Conclusion: The Sulcoflex® multifocal IOL improves near and distance VA in cataract patients with retinal abnormalities and good VA potential.

Avian mycobacteriosis was diagnosed in a captive scarlet macaw (Ara macao) that presented multifocal granulomas on subcutaneous tissue, sciatic nerves, infraorbital sinus, trachea, air sacs, muscles, spleen and liver. Microscopically, central areas of caseous necrosis surrounded by epithelioid macrophage, multinucleated giant cells, and lymphocytes were observed. Acid-fast bacilli were demonstrated by Ziehl-Neelsen stain. Inoculation into Lowenstein-Jensen, Stonebrink and Petragnani media, yielded Mycobacterium spp, which was identified as Mycobacterium avium by polymerase chain reaction technique (PCR).
Differential gene expression analysis of Paracoccidioides brasiliensis during keratinocyte infection

O presente trabalho estudou a intoxicação acidental por arsênico em um lote de 24 vacas Girolando, as quais tiveram acesso a pasto pulverizado com herbicida à base de metano arsonato ácido monossódico (MSMA). Os bovinos apresentaram apatia, anorexia e diarreia profusa. Foram necropsiados na fazenda dois animais de 14 que morreram. Os principais achados macroscópicos foram úlceras abomasais e congestão renal. No exame microscópico, as principais lesões observadas foram abomasite e omasite necro-hemorrágica multifocal acentuada e, nos rins, necrose tubular difusa. As concentrações médias de arsênico em vacas com sinais clínicos foram 1,19±0,40, 10,52±2,16 e 76,06±48,37ppm no sangue, leite e fezes, respectivamente. Os níveis de arsênico encontrados em dois animais necropsiados foram 25,58 e 23,85ppm em fígado, e 28,71 e 35,94ppm em rins, respectivamente. No feto de uma vaca necropsiada, os níveis de arsênico mensurados no fígado e rim foram 9,0 e 8,92ppm, respectivamente. A concentração de arsênico no capim do piquete pulverizado foi 111,58ppm. No Brasil, o uso MSMA na composição de pesticidas e herbicidas é permitido somente para uso agrícola, mas não pecuário. A utilização desse ou de outros produtos à base de arsênico na pecuária pode causar altos índices de mortalidade no rebanho, além de diminuição da produção e contaminação de produtos de origem animal.

BACKGROUND: Paracoccidioidomycosis is a systemic mycosis of dermatological interest due to the frequency of cutaneous and mucosal lesions. The involvement of the external genitalia is extremely rare and few cases have been reported.OBJECTIVE: To study the prevalence of external genitalia lesions in paracoccidioidomycosis patients, identify clinical characteristics and compare with what is observed in the specific literature.METHODS: This is a cross-sectional, descriptive study, with focus on paracoccidioiodomycosis patients with external genitalia lesions. The demographic and clinical aspects of cases were compared with what has been reported so far on LILACS, SciELO e MEDLINE data bases.RESULTS: Data of 483 cases of paracoccidioidomycosis were studied in a 42-year period. Six (1.2%) patients showed specific lesions on external genitalia. Five patients were male with mean age of 47.2 years and all of them presented with the chronic multifocal clinical form. Only one, a 15-year-old female patient was observed who showed a subacute clinical form, juvenile type.CONCLUSIONS: Compromise of the genitourinary tract among paracoccidioidomycosis patients is rare and even rarer when only the external genitalia are considered. As observed in the classical picture of paracoccidioidomycosis patients, the male gender and the chronic multifocal clinical form prevailed in the present study.

The aim of this study was to evaluate the application of PCR technique for the detection of BoHV-5 in routinely formalin-fixed, paraffin-embedded brain tissues in 20 naturally infected calves affected by fatal meningoencephalitis. Brains were divided into two halves, one kept fresh for virus isolation and PCR assay, targeting the glycoprotein C gene from BoHV-5 genome. The other half brain, corresponding to posterior cortex region, was submitted to formalin fixation and embedded into paraffin blocks for microscopic evaluation and total DNA isolation. Most of the slides showed severe multifocal non-supurative encephalitis with neuronal degeneration, neurophagia, and no acidophilic intranuclear inclusions could be found in neurons and glial. The 20 fresh samples were confirmed, by virus isolation and PCR assay, as having the BoHV-5 virus and, respective glicoprotein C sequence, while 15 of 20 formalin-fixed, paraffin-embedded samples were considered positive for the same analysis. The results revealed the first description of PCR efficiency, applied to formalin-fixed, paraffin-embedded brain collected from naturally infected calves, improving the detection of BoHV-5 from archival samples in South America. The aim of this study was to investigate the histopathological changes in reproductive system (testicles, epididymis, seminal vesicles, and prostate) of small male ruminants after Toxoplasma gondii infection. Eight sheep were inoculated with T. gondii: group I, four sheep (2.0 x 10(5) P-strain oocysts); group II, four sheep (1.0 x 10(6) RH-strain tachyzoites); and group III, two uninfected sheep maintained as control. Infection with T. gondii was confirmed by seroconversion (indirect fluorescent antibody test-IgG) in all the infected animals beginning on post-inoculation day (PID) 7. on PID 70, all the animals were euthanized and tissue samples (testicles, epididymis, seminal vesicles, and prostate) were collected and processed for histological analysis. The main changes detected were a focal mononuclear interstitial inflammatory infiltrate in the prostate and seminal vesicles; diffuse testicular degeneration associated with calcification foci and a multifocal mononuclear interstitial inflammatory infiltrate; and a mononuclear interstitial infiltrate and focal necrotic areas of the muscle fibers surrounding the seminal vesicles. The histopathological findings of this work, along with the detection of T. gondii in the examined parenchyma tissues (immunohistochemistry) and the results obtained by other authors examining different tissues, suggest that histological changes diagnosed in the reproductive system of rams infected with T. gondii are strongly suggestive of toxoplasmatic infection.

Purpose: The purpose of this study was to analyze the clinical and histological features of actinic cheilitis (AC).Patients and Methods: A total of 29 patients with AC were clinically evaluated, and incisional biopsies were performed to confirm the clinical diagnosis. Histological features were analyzed, and dysplasia was classified as mild, moderate, or severe. The chi(2) test was used for the following variables: gender, age, race, and smoking habits. The degree of dysplasia was related to these variables (Fisher's test) to test for independence between them (P <.05).Results: of the patient group, 72.41% were male, 75.86% were overage 40 years, 93.10% were white, and 72.41% were nonsmokers. Clinically, all patients presented with multifocal lesions. The following manifestations were seen: dryness, atrophy, scaly lesions, swelling of the lip, erythema, ulceration, blurred demarcation between the lip vermilion border and the skin, marked folds along the lip vermilion, white spots or plaques, crusts, blotchy areas, and areas of pallor. Keratosis, granulosis, hyperplasia, acanthosis, or atrophy and dysplasia were found in the epithelial tissue; elastosis, inflammatory infiltrate, and vasodilatation were found in the connective tissue. Dysplasia was mild in 10.34% of the patients, moderate in 27.59%, and severe in 62.07%. Absence of sample homogeneity was observed in regard to gender, age, race, and smoking habits. It was not possible to reject the hypothesis of independence between mild, moderate, or severe dysplasia and gender, age, race, and smoking habits.Conclusions: Dryness, atrophy, and scaly lesions were the most common clinical findings observed. Dysplasia, inflammatory infiltrate, and vasodilatation, as well as elastosis, were the most common histological findings observed. Gender, age, race, or smoking habits were not related to the degree of dysplasia in the sample. (c) 2008 American Association of Oral and Maxillofacial Surgeons.

OBJETIVO: O objetivo deste estudo foi determinar o perfil da deficiência imune em um grupo bem definido de epilepsia: crianças com síndrome de West (SW) e seus padrões EEG de evolução, idade-dependentes, como os complexos onda-aguda- onda lenta generalizadas da síndrome de Lenox-Gastaut (SLG) e as pontas multifocais independentes (PMI). MÉTODO: Um grupo de 50 crianças, 33 com SW, 10 com SLG, 7 com PMI e 20 crianças sadias (controle) foram avaliadas em relação aos seguintes parâmetros:determinação de subpopulações de linfócitos T (CD1, CD3, CD4 e CD8), relação CD4/CD8 e resposta proliferativa de linfócitos frente a fitohemaglutinina (PHA), na presença de plasma autólogo ou de plasma AB (homólogo). A prova cutânea de sensibilização ao Dinitroclorobenzeno (DNCB) foi realizada apenas nos pacientes. Os níveis séricos de IgG, IgA e IgM foram comparados aos valores normais em crianças Brasileiras, em diferentes faixas etárias. RESULTADOS: A resposta ao DNCB foi ausente ou fracamente reativa em 76% dos pacientes. Níveis séricos elevados de IgG (45,7%) e de IgM (61,4%) e baixos de IgA (23,9%) foram detectados nos pacientes. A determinação das subpopulações de linfócitos T em sangue periférico mostrou: deficiência nas proporções de células CD3+ (p<0,05) e de CD4+ (p<0,05), aumento de CD8+ (p<0,01) e diminuição da relação CD4 / CD8 (p<0,001). A proporção de células CD1+ no grupo controle manteve-se menor que 3%, enquanto que em 18% dos pacientes esses níveis variaram entre 3 e 11%. A resposta proliferativa de linfócitos frente a PHA revelou índices blastogênicos significativamente mais baixos apenas quando células dos pacientes foram cultivadas na presença do próprio plasma (plasma autólogo). Quando estas células foram cultivadas na presença de plasma AB, não se evidenciou diferença significativa em relação ao grupo controle. CONCLUSÃO: A imunodeficiência na SW caracterizou-se por: anergia, alteração de imunidade mediada por células e dos níveis de imunoglobulinas, presença de timócitos imaturos na circulação periférica e deficiência funcional de linfócitos T induzida por fatores plasmáticos inibidores. Discutem-se as principais evidências de disfunção imune como imunodeficiência e autoimunidade.

Avaliou-se a ação antiarrítmica do isofluorano em cães submetidos a arritmias ventriculares pelo uso de cloreto de bário, utilizando-se de seis cães, machos e fêmeas, que receberam uma dose de 3mg/kg de peso IV de cloreto de bário a 2,5% (G1). O mesmo protocolo foi repetido, nos mesmos animais, sob anestesia geral com isofluorano (G2). Usou-se a eletrocardiografia computadorizada para avaliar o ritmo cardíaco, a duração e/ou amplitude das ondas e os intervalos eletrocardiográficos. Não se verificou alteração no ritmo cardíaco em G2, diferente de G1, que apresentou freqüentes arritmias ventriculares na forma de bigeminismo e taquicardia ventricular multifocal. Houve diferença significativa entre os grupos em relação à freqüência cardíaca nos minutos iniciais de observação, quando ocorreu aumento na freqüência cardíaca em G1. A utilização do isofluorano conferiu ação antiarrítmica em cães com arritmias induzidas pelo cloreto de bário, reforçando suas indicações a pacientes com risco considerável de desenvolvimento de arritmias ventriculares.

Aproximadamente, 15% das discopatias em cães acometem a região cervical, sendo a dor o principal sinal clínico. Descreve-se a ocorrência de protrusão de disco cervical em 17 cães, agrupados segundo a raça, sexo, peso, idade, assim como a distribuição quanto à duração dos sintomas, acometimento dos discos intervertebrais (DIV), tempo de recuperação e porcentagem de sucesso, em relação à condição neurológica presente antes da cirurgia. A raça Dachshund representou 29,5% (n=5), cães sem raça definida, Poodle e Cocker Spaniel Inglês, 17,6% cada (n=9), Pinscher, 11,8% (n=2) e Dálmata, 5,9% (n=1). Destes, 58,8% eram machos (n=10) e 41,2%, fêmeas (n=7), com peso entre 2 e 29kg, e idade média igual a 5,8 anos. O quadro neurológico desses animais correspondia à dor e ataxia, com exceção de um cão Dálmata, 11 anos de idade, que apresentava tetraparesia. A duração dos sinais variou de 2 a 90 dias. Os DIV mais acometidos foram C2/3 (40%), C3/4 (25%), C4/5 (15%), C5/6 (10%) e C6/7 (10%), sendo que alguns animais apresentavam lesões múltiplas. O procedimento foi padrão para todos os animais, através da fenestração e curetagem de todos os DIV abordados pelo acesso ventral, ou seja, de C2/3 até C6/7, empregando-se para isso instrumental usado para remoção de tártaro dentário (curetas Gracey, curetas McCall, extratores de tártaro S.S.White e McCall). O tempo médio de recuperação foi de 9 a 38 dias, sendo que 100% deles recuperaram totalmente as funções neurológicas. Conclui-se que a fenestração ventral apresenta excelentes resultados no tratamento das discopatias cervicais, desde que bem selecionados os pacientes, inclusive, com respeito aos diagnósticos diferenciais.

São descritos a epidemiologia, os sinais clínicos, os achados de necropsia e a histopatologia de seis casos de febre catarral maligna (FCM) em bovinos de 5 fazendas localizadas nos estados de Mato Grosso do Sul e São Paulo. A doença ocorreu em bovinos de ambos os sexos e com idades variando de 4 meses a 11 anos. Os índices de morbidade variaram de 0,25% a 6.6% e a letalidade foi de 100%. A evolução clínica foi aguda (2-3 dias) em seis casos e crônica em um (3 meses). O diagnóstico presuntivo de FCM nos casos descritos neste relato foi baseado nos sinais clínicos, achados de necropsia e confirmados pela histopatologia. Os principais sinais clínicos nos casos agudos foram febre, corrimento mucopurulento pelas fossas nasais, opacidade da córnea, sialorréia, úlceras em várias superfícies mucosas e distúrbios nervosos. O bovino do caso crônico mostrou opacidade da córnea e distúrbios neurológicos. Os principais achados de necropsia incluíam hiperemia e lesões diftéricas em várias superfícies epiteliais e a histopatologia consistiu de vasculite, focos de infiltrado mononuclear multifocal em vários órgãos e necrose de superfícies epiteliais.
