The traditional operational and hazard control paradigm of an electrical installation has several issues pertaining to the diversity of equipment. A large maintenance inventory is a reason for concern for any manager, but the arc flash hazard is a particularly new phenomenon and the effects of equipment diversity on this phenomenon are even newer. The class of arc hazard can be increased, simply by changing the fuse link or circuit breaker manufacturer. Management pressure to operate as well as non-standard practices and installations are also partly responsible. The aim of this study is to take a typical situation in an industrial plant and to statistically simulate, by means of a dedicated software program, the fuse arc flash interruption performance of various manufacturers. The purpose of this study is to obtain a more objective indication of the influence of different fuse link manufacturers on arc flash incident energy. © 2011 IEEE.

The distribution of natural gas is carried out by means of long ducts and intermediate compression stations to compensate the pressure drops due to friction. The natural gas compressors are usually driven by an electric motor or a gas turbine system, offering possibilities for energy management, one of these consisting in generating energy for use in-plant or to commercialize as independent power producer. It can be done by matching the natural gas demand, at the minimum pressure allowed in the reception point, and the storage capacity of the feed duct with the maximum compressor capacity, for storing the natural gas at the maximum permitted pressure. This allows the gas turbine to drive an electric generator during the time in which the decreasing pressure in duct is above the minimum acceptable by the sink unit. In this paper, a line-pack management analysis is done for an existing compression station considering its actual demand curve for determining the economic feasibility of maintaining the gas turbine system driver generating electricity in a peak and off-peak tariff structure. The potential of cost reduction from the point of view of energy resources (natural gas and electric costs) is also analyzed. (C) 2010 Elsevier Ltd. All rights reserved.

The Brazilian Consensus on Gastroesophageal Reflux Disease considers gastroesophageal reflux disease to be a chronic disorder related to the retrograde flow of gastroduodenal contents into the esophagus and/or adjacent organs, resulting in a variable spectrum of symptoms, with or without tissue damage. Considering the limitations of classifications currently in use, a new classification is proposed that combines three criteria - clinical, endoscopic, and pH-metric - providing a comprehensive and more complete characterization of the disease. The diagnosis begins with the presence of heartburn, acid regurgitation, and alarm manifestations (dysphagia, odynophagia, weight loss, GI bleeding, nausea and/or vomiting, and family history of cancer). Also, atypical esophageal, pulmonary, otorhinolaryngological, and oral symptoms may occur. Endoscopy is the first approach, particularly in patients over 40 yr of age and in those with alarm symptoms. Other exams are considered in particular cases, such as contrast radiological examination, scyntigraphy, manometry, and prolonged pH measurement. The clinical treatment encompasses behavioral modifications in lifestyle and pharmacological measures. Proton pump inhibitors in manufacturers' recommended doses are indicated, with doubling of the dose in more severe cases of esophagitis. The minimum time of administration is 6 wk. Patients who do not respond to medical treatment, including those with atypical manifestations, should be considered for surgical treatment. Of the complications of gastroesophageal reflux disease, Barrett's esophagus presents a potential development of adenocarcinoma; biopsies should be performed, independent of Barrett's esophagus extent or location. In this regard the designation short Barrett's is not important in terms of management and prognosis. © 2002 by Am. Coll. of Gastroenterology.

The aim of this study is to identify and analyse the factors that affect the adoption of Green Supply Chain Management practices based on empirical evidence from the Brazilian electronics sector. Data are collected in a survey of 100 electronics companies and analysed using statistical analysis of variance and regression methods. The study finds that the size of the company, previous experience with Environmental Management Systems, and the use of hazardous inputs are positively correlated with GSCM practices adoption. Surprisingly, formal pressure from the stronger tier/player in the supply chain is not correlated with the adoption of GSCM practices. Finally, we present some explanations for these findings and suggestions for future research. © 2013 Copyright Taylor and Francis Group, LLC.

Temporomandibular joint (TMJ) disorder is a term that encompasses a number of overlapping conditions, such as closed lock. Closed lock of the TMJ is considered a consequence of a nonreducing deformed disc acting as an obstacle to the sliding condylar head that usually causes a decrease in the maximum mouth opening and acute pain. The management of the TMJ is still controversial. Thus, arthrocentesis of the TMJ is a valuable modification of the traditional method of arthroscopic lavage, which consists of washing the joint in order to remove chemical inflammatory mediators and intra-articular adhesions, changing intra-articular pressure. TMJ disorder has always presented as a therapeutic challenge to maxillofacial surgeons. Therefore, this paper aimed to describe a clinical report of a closed lock of the left TMJ in a 19-year-old female subject who was successfully treated by arthrocentesis procedure. © 2013 by Mutaz B. The aim of this research is to verify the relationship between the maturity levels of environmental management and the adoption of green supply chain management (GSCM) practices by electro-electronic companies in Brazil. In this work a two-phase research was conducted, with one quantitative and the other qualitative. The quantitative phase aimed to test whether a relationship between the maturity levels of environmental management and GSCM exists, while the qualitative phase tried to detail the characteristics of this relationship. The quantitative phase was conducted through a survey with 100 Brazilian electro-electronic companies and the collected data were processed using Structural Equation Modeling. For the qualitative phase, a multiple case study was conducted with three companies located in Brazil. The results indicate that: (1) The main hypothesis was confirmed and considered statistically valid, indicating that, indeed, the maturity level of environmental management influences the adoption of GSCM practices; (2) a coevolution tends to occur between the environmental maturity and the GSCM practices; that is, the more developed is the company's environmental management, more complex GSCM practices are adopted; and (3) the GSCM internal practices tend to present a greater relative adoption than the external practices; these external practices of GSCM tend to be adopted when the company is inserted in a higher environmental stage and/or operates under a scenario of stronger normative environmental pressure. BACKGROUND: Hypertension is the most prevalent comorbidity after heart transplantation (HT). Exercise training (ET) is widely recommended as a key non-pharmacologic intervention for the prevention and management of hypertension, but its effects on ambulatory blood pressure (ABP) and some mechanisms involved in the pathophysiology of hypertension have not been studied in this population. The primary purpose of this study was to investigate the effects of ET on ABP and arterial stiffness of HT recipients.METHODS: 40 HT patients, randomized to ET (n = 31) or a control group (n = 9) underwent a maximal graded exercise test, 24-hour ABP monitoring, and carotid-femoral pulse wave velocity (PWV) assessment before the intervention and at a 12-week follow-up assessment. The ET program was performed thrice-weekly and consisted primarily of endurance exercise (40 minutes) at similar to 70% of maximum oxygen uptake (Vo(2MAX))RESULTS: The ET group had reduced 24-hour (4.0 +/- 1.4 mm Hg, p < 0.01) and daytime (4.8 +/- 1.6 mm Hg, p < 0.01) systolic ABP, and 24-hour (7.0 +/- 1.4 mm Hg, p < 0.001) daytime (7.5 +/- 1.6 mm Hg, p < 0.001) and nighttime (5.9 +/- 1.5 mm Hg, p < 0.001) diastolic ABP after the intervention. The ET group also had improved Vo(2MAX) (9.7% +/- 2.6%, p < 0.001) after the intervention. However, PWV did not change after ET. No variable was changed in the control group after the intervention.CONCLUSIONS: The 12-week ET program was effective for reducing ABP but not PWV in heart transplant recipients. This result suggesfs that endurance ET may be a tool to counteract hypertension in this high-risk population. (C) 2015 International Society for Heart and Lung Transplantation. All rights reserved.

The use of bamboo as construction and raw material for producing products can be considered a feasible alternative to the abusive use of steel, concrete and oil byproducts. Its use can also reduce the pressure on the use of wood from native and planted forests. Although there are thousands of bamboo species spread about the world and Brazil itself has hundreds of native species, the use and basic knowledge of its characteristics and applications are still little known and little disseminated. This paper's main objective is to introduce the species, the management phases, the physical and mechanical characteristics and the experiences in using bamboo in design and civil construction as per the Bamboo Project implemented at UNESP, Bauru campus since 1994. The results are divided into: a) Field activities - description of the technological species of interest, production chain flows, types of preservative treatments and clump management practices for the development, adaptation and production of different species of culms; b) Lab experiments - physical and mechanical characterization of culms processed as laminated strips and as composite material (glue laminated bamboo – glubam); c) Uses in projects - experiences with natural bamboo and glubam in design, architecture and civil construction projects. In the final remarks, the study aims to demonstrate, through practical and laboratory results, the material's multi-functionality and the feasibility in using bamboo as a sustainable material.
