The study on several components of intervertebral joints is essential to understand the spine's degenerative mechanisms and to assess the best method for their treatment. For such study it is necessary to know the mechanical properties of the isolated intervertebral disc (ID) mechanical properties and, it is necessary to evaluate its stresses and strains. In order to assess the ID displacements, a fine, U-shaped blade was developed, over which two extensometers connected in a Wheatstone bridge were placed. The device was then tested on porcine spine ID, where compression loads were applied and the extremities displacements of the blade coupled to the intervertebral disc were measured. Stress/strain diagram, both on the compression and on the decompression phases, evidencing the non-linear nature of such relationship. With the experiment, it was possible to obtain approximate values of the longitudinal elasticity module (E) of the disc material and of the Poisson coefficient (n ). After several tests, E results are compatible with those obtained by others studies, with very simple and low-cost device. This experiments can be used for obtained others mechanical properties of isolated ID with precision and accuracy.

Purpose: This study evaluated the affect of disc displacement and articular disc repositioning on stability after surgical counterclockwise rotation and advancement of the maxillomandibular complex.Patients and Methods: A total of 72 patients (59 females, 13 males), with an average age of 30 years (range, 15 to 60 years) were evaluated. The patients were divided into 3 groups. Group 1 (G1; n = 21), with healthy temporomandibular joints (TMJs), underwent double jaw surgery only. Group 2 (G2; n = 35), with articular disc dislocation, underwent articular disc repositioning using the Mitek anchor (Mitek Surgical Products, Westwood, MA) technique concomitantly with orthognathic surgery. Group 3 (G3; n = 16), with articular disc dislocation, underwent orthognathic surgery only. Average postsurgical follow-up was 31 months. Each patient's lateral cephalograms were traced, digitized twice, and averaged to estimate surgical changes and postsurgical stability.Results: After surgery, the occlusal plane angle was decreased significantly in all 3 groups: by -6.3 +/- -15.0 degrees in G1, by -9.6 +/- 4.8 degrees in G2, and by -7.1 +/- 4.8 degrees in G3. The maxillomandibular complex was advanced and rotated counterclockwise similarly in all 3 groups, with advancement at the menton of 12.4 +/- 5.5 mm in G1, 13.5 +/- 4.3 mm in G2, and 13.6 +/- 5.0 mm in G3; advancement at the B point of 9.5 +/- 4.9 mm in G1, 10.2 +/- 3.7 mm in G2, and 10.8 +/- 3.7 mm in G3; and advancement at the lower incisor edge of 7.1 +/- 4.6 mm in G1, 6.6 +/- 3.2 mm in G2, and 7.9 +/- 3.0 mm in G3. Postsurgery, the occlusal plane angle increased in G3 (2.6 +/- 3.8 degrees; 37% relapse rate) but remained stable in G1 and G2. Postsurgical mandibular changes in the horizontal direction demonstrated a significant relapse in G3 at the menton (-3.8 +/- 4.1 mm; 28%), the B point (-3.0 +/- 3.4 mm; 28%), and the lower incisor edge (-2.3 +/- 2.1 mm; 34%) but remained stable in G1 and G2.Conclusions: Maxillomandibular advancement with counterclockwise rotation of the occlusal plane is a stable procedure for patients with healthy TMJs and for patients undergoing simultaneous TMJ disc repositioning using the Mitek anchor technique. Those patients with preoperative TMJ articular disc displacement who underwent double-jaw surgery and no TMJ intervention experienced significant relapse. Mielografia é uma técnica radiográfica na qual se administra meio de contraste no espaço subaracnóideo para avaliar a medula espinhal. Múltiplas projeções radiográficas fornecem uma exploração circunferencial da medula, auxiliando na localização mais precisa de compressões nesta região. Objetivou-se demonstrar a contribuição das projeções oblíquas, pouco exploradas em exames de mielografia, para a localização de lesões medulares extradurais em cães e gatos causadas por processo degenerativo do disco intervertebral. Foram avaliadas 116 mielografias e observou-se que em 36,2% dos casos as projeções oblíquas foram imprescindíveis para a localização exata das lesões. A associação entre as projeções ventrodorsal e oblíquas se mostraram mais úteis para a localização da lesão do que quando avaliadas isoladamente e as projeções oblíquas esquerda e direita foram igualmente importantes.

As doenças que afetam o sistema músculo-esquelético acometem centenas de milhões de pessoas no mundo e estão entre as causas mais comuns de invalidez e sofrimento crônico. A doença vertebral degenerativa constitui uma exacerbação do processo de envelhecimento, podendo estar envolvidos os fatores genéticos, ambientais ou lesões traumáticas, deformidades e doenças preexistentes. Muito se tem discutido sobre os múltiplos fatores envolvidos na degeneração discal, mas sua etiologia permanece indeterminada. Contudo, atualmente a participação da genética parece muito mais forte do que se suspeitava anteriormente. Neste artigo, é abordada a participação de alguns genes no processo discogênico, bem como o que isso representa para o melhor entendimento da etiopatogênese da doença e na melhora de seu tratamento.

Purpose To evaluate condylar changes 1 year after bimaxillary surgical advancement with or without articular disc repositioning using longitudinal quantitative measurements in 3-dimensional (3D) temporomandibular joint (TMJ) models. Methods Twenty-seven patients treated with maxillomandibular advancement (MMA) underwent cone-beam computed tomography before surgery, immediately after surgery, and at 1-year follow-up. All patients underwent magnetic resonance imaging before surgery to assess disc displacements. Ten patients without disc displacement received MMA only. Seventeen patients with articular disc displacement received MMA with simultaneous TMJ disc repositioning (MMA-Drep). Pre- and postsurgical 3D models were superimposed using a voxel-based registration on the cranial base. Results The location, direction, and magnitude of condylar changes were displayed and quantified by graphic semitransparent overlays and 3D color-coded surface distance maps. Rotational condylar displacements were similar in the 2 groups. Immediately after surgery, condylar translational displacements of at least 1.5 mm occurred in a posterior, superior, or mediolateral direction in patients treated with MMA, whereas patients treated with MMA-Drep presented more marked anterior, inferior, and mediolateral condylar displacements. One year after surgery, more than half the patients in the 2 groups presented condylar resorptive changes of at least 1.5 mm. Patients treated with MMA-Drep presented condylar bone apposition of at least 1.5 mm at the superior surface in 26.4%, the anterior surface in 23.4%, the posterior surface in 29.4%, the medial surface in 5.9%, or the lateral surface in 38.2%, whereas bone apposition was not observed in patients treated with MMA. Conclusions One year after surgery, condylar resorptive changes greater than 1.5 mm were observed in the 2 groups. Articular disc repositioning facilitated bone apposition in localized condylar regions in patients treated with MMA-Drep. © 2013 American Association of Oral and Maxillofacial Surgeons.

Protrusion of the intervertebral disc can cause chronic progressive compression of the spinal cord, and the neurological associated signs vary according to anatomical location, duration and dynamic force of compression. The type II disc disease is not fully understood, and there are many controversies about is clinical significance and best form of treatment. The most important aspect of conservative treatment is rest and physiotherapy associated with mild exercise, and use of corticosteroids to reduce vasogenic edema. The spinal decompression by surgery for removal of disc material from within the medullary canal is the surgical treatment of choice in type I disc disease, but has technical limitations in the type II disc disease. The purpose of this paper is to review the pathophysiology and treatment of chronic intervertebral disc disease and discuss the controversies in medical treatment and the use of some traditional and new surgical techniques.

The intervertebral disc diseases have a high casuistic in clinical routines veterinary among dogs. These conditions can cause pain, paresis or paralysis of limbs depending on the degree of injury. Clinical signs are not limited to the intervertebral disc, other anatomical parts are included. In conventional medicine, treatment may be surgical or clinical, by the use of corticosteroids with muscle relaxants. Acupuncture was an alternative to treating these diseases due to its anti-inflammatory and analgesic effects. Although there are few studies for the treatment of discopathies, acupuncture triggers the release of substances such as dynorphins, beta-endorphins and serotonin, among others; that act inhibiting somatic and visceral pain. Electroacupuncture is used to potentiate the effects of acupuncture. It can be performed with different frequencies according to the clinical evolution. However there are different opinions related to the greater efficacy in relation to acupuncture. The restraint and monitoring of the treated animal are important factors for its recovery. The association between acupuncture and conventional treatment (surgical or clinical) achieved better results than acupuncture alone. This proves that acupuncture can be considered an option in order to minimize the complications of surgery or assist in patient recovery

Temporomandibular joint disorders causing limitation of mouth opening and pain may be related to the restriction of slipping or forward displacement of the mandibular condyle. Temporomandibluar arthrocentesis is the lavage of the joint and is regarded as a simple method that produces good results in patients with these symptoms. This article reports a case of disc displacement and closed lock in a patient who reported limited mouth opening and pain as the main symptoms. The treatment proposed was arthrocentesis and lavage, combining a corticosteroid with a sodium hyaluronate solution. It is concluded that arthrocentesis is a simple, efficient and barely invasive method for particular types of temporomandibular joint disorders and should be considered before opting for more invasive surgical procedures.

Evaluation of the prevalence and characteristics of tinnitus in a Brazilian series of sleep bruxism patients. In this descriptive study, 100 patients (80 women and 20 men) were selected through the self-report of grinding teeth during sleep, confirmed by room mate or family member. They were evaluated according to a systematized approach: a questionnaire for orofacial pain and the Portuguese version of the Research Diagnostic Criteria for Temporomandibular Disorders. The patients were divided into two groups: group A, 54 patients with complaint of tinnitus and group B, 46 patients without tinnitus complaint. The mean age was 37.85 (13-66 years) and 34.02 years (20-59 years), respectively, for groups A and B (P = 0.1164). There was statistically significant difference between the two groups, with higher prevalence for the group A, in relation to: presence of chronic facial pain (P = 0.0007); number of areas painful to palpation in the masticatory and cervical muscles (P = 0.0032); myofascial pain in the masticatory muscles (P = 0.0003); absence of teeth without prosthetic replacement (P = 0.0145) and indices of depression (P = 0.0234). Structural alterations of the TMJ, like disc displacement and vertical dimension loss did not differ for the two groups. Tinnitus frequency was higher in patients with sleep bruxism and chronic facial pain. Myofascial pain, number of areas painful to palpation in the masticatory and cervical muscles, higher levels of depression and tooth absence without prosthetic replacement were more frequent in the group with tinnitus.

This study aims to evaluate safety and effectiveness of the use of orthesis in postoperative degenerative disorders of the cervical spine. Although widely used, there are not defined criteria for the use of cervical collars, and their importance on cervical arthrodesis. A systematic literature review was undertaken and a multicentre controlled clinical trial comprising 32 specialized services, with a total of 257 patients who met the inclusion criteria of the review. Clinical and radiographic outcomes were compared in post surgical patients that used or not rigid cervical collar during 6 to 12 weeks after surgery. The evidence available on literature, although weak, suggests that cervical collar decreases the clinical scores in first weeks after surgery. Furthermore, it does not alter the rate of fusion when used concomitantly with plaque and screws through anterior access in cervical degenerative diseases on a single anatomical level.
