Objective. To develop and validate a new short and simple measure of health-related quality of life (HRQL) in children with juvenile idiopathic arthritis (JIA).Methods. The Paediatric Rheumatology Quality of Life Scale (PRQL) is a 10-item questionnaire that explores HRQL in two domains: physical health (PhH) and psychosocial health (PsH). Validation of the parent proxy report and child self-report versions of the instrument was accomplished by evaluating 472 JIA patients and similar to 800 healthy children. Validation analyses included assessment of feasibility, face and content validity; construct and discriminative ability; internal structure and consistency; test-retest reliability; responsiveness to clinical change; and minimal clinically important difference.Results. The PRQL was found to be feasible and to possess both face and content validity. The PRQL score correlated in the predicted range with most of the other JIA outcome measures, thereby demonstrating good construct validity, and discriminated well between different levels of disease severity. Assessment of internal structure (factor analysis) revealed that the PhH and PsH subscales identify two unambiguously separated domains. The internal consistency (Cronbach's alpha) was 0.86. The intraclass correlation coefficient for test-retest reliability was 0.91. The PRQL revealed fair responsiveness, with a standardized response mean of 0.67 in improved patients. Overall, the PRQL appeared to be more able to capture physical HRQL than psychosocial HRQL.Conclusion. The PRQL was found to possess good measurement properties and is, therefore, a valid instrument for the assessment of HRQL in children with JIA. This tool is primarily proposed for use in standard clinical care.

A larvicultura da maioria das espécies de peixes enfrenta o desafio da dependência do alimento vivo (AL) e da falta de dietas formuladas (DF) que atendam plenamente às necessidades das larvas. A baixa digestibilidade e a qualidade nutricional das DFs são alguns dos fatores que explicam o insucesso quando as larvas recebem apenas FD. Para avaliar o efeito da combinação da DF com o AL no crescimento e na sobrevivência de larvas de jundiá (Rhamdia quelen), comparando com o uso separado da DF ou do AL, larvas recém eclodidas (5,57 mm; 1,41 mg) foram estocadas inicialmente em 12 aquários de 10 L (100 larvas por aquário). Quatro réplicas foram alimentadas ad libitum com uma das três dietas por 20 (para DF) ou 48 dias (para AL ou a combinação DF + AL). As larvas alimentadas com apenas DF apresentaram crescimento e sobrevivência reduzidos quando comparadas àquelas alimentadas com AL ou a combinação DF + AL. Adicionalmente, as larvas do tratamento DF + AL apresentaram maior crescimento em peso (170 mg) que aquelas alimentadas apenas com AL (110 mg). O melhor desempenho das larvas alimentadas com DF + AL mostra que a maioria dos nutrientes exigidos pelas larvas é fornecida mais adequadamente quando ambas as dietas são fornecidas juntamente. Contudo, trabalhos sobre nutrição larval poderão contribuir ainda mais sobre a elucidação deste tema quando feitas comparações com o uso combinado de DF + AL, do que apenas testando isoladamente novos ingredientes e fontes protéicas normalmente utilizadas na elaboração de dietas para juvenis e adultos.

Tick paralysis (TP) is a rare disease with rapid progression and potential fatal evolution. Immediately after the diagnosis, removal of all ticks from the body of the patient is mandatory. The present study reports for the first time a human case of the disease in Brazil. The patient had loss of muscle strength, decreased reflexes and marked palpebral ptosis. Six hours after removal of the last tick, the ptosis improved and on the following day, the patient had near total regression of the symptoms. This report emphasizes the possible presence of similar cases that should be promptly diagnosed and quickly treated. A new induction pattern for TP in humans associated with immature stages of ticks is also presented.

Introduction: Propolis has plenty of biological and pharmacological properties and its mechanisms of action have been widely investigated in the last years, using different experimental models in vitro and in vivo. Researchers have been interested in the investigation of isolated compounds responsible for propolis action; however, there is lack of clinical research on the effects of propolis.Strategy and objectives: Since propolis-containing products have been marketed and humans have used propolis for different purposes, the goal of this review is to discuss the potential of propolis for the development of new drugs, by comparing data from the literature that suggest candidate areas for the establishment of drugs against tumors, infections, allergy, diabetes, ulcers and with immunomodulatory action.Conclusions: The efficacy of propolis in different protocols in vitro and in vivo suggests its therapeutic properties, but before establishing a strategy using this bee product, it is necessary to study: (a) the chemical nature of the propolis sample. (b) Propolis efficacy should be compared to well-established parameters, e.g. positive or negative controls in the experiments. Moreover, possible interactions between propolis and other medicines should be investigated in humans as well. (c) Clinical investigation is needed to evaluate propolis potential in patients or healthy individuals, to understand under which conditions propolis may promote health. Data point out the importance of this research field not only for the readers and researchers in the scientific community waiting for further clarification on the potential of propolis but also for the pharmaceutical industry that looks for new drugs. The modern approach to the development of new chemical entities against complex diseases, especially the neglected endemic diseases such as tuberculosis and malaria, is based on the use of defined molecular targets. Among the advantages, this approach allows (i) the search and identification of lead compounds with defined molecular mechanisms against a defined target (e.g. enzymes from defined pathways), (ii) the analysis of a great number of compounds with a favorable cost/benefit ratio, (iii) the development even in the initial stages of compounds with selective toxicity (the fundamental principle of chemotherapy), (iv) the evaluation of plant extracts as well as of pure substances. The current use of such technology, unfortunately, is concentrated in developed countries, especially in the big pharma. This fact contributes in a significant way to hamper the development of innovative new compounds to treat neglected diseases. The large biodiversity within the territory of Brazil puts the country in a strategic position to develop the rational and sustained exploration of new metabolites of therapeutic value. The extension of the country covers a wide range of climates, soil types, and altitudes, providing a unique set of selective pressures for the adaptation of plant life in these scenarios. Chemical diversity is also driven by these forces, in an attempt to best fit the plant communities to the particular abiotic stresses, fauna, and microbes that co-exist with them. Certain areas of vegetation (Amazonian Forest, Atlantic Forest, Araucaria Forest, Cerrado-Brazilian Savanna, and Caatinga) are rich in species and types of environments to be used to search for natural compounds active against tuberculosis, malaria, and chronic-degenerative diseases. The present review describes some strategies to search for natural compounds, whose choice can be based on ethnobotanical and chemotaxonomical studies, and screen for their ability to bind to immobilized drug targets and to inhibit their activities. Molecular cloning, gene knockout, protein expression and purification, N-terminal sequencing, and mass spectrometry are the methods of choice to provide homogeneous drug targets for immobilization by optimized chemical reactions. Plant extract preparations, fractionation of promising plant extracts, propagation protocols and definition of in planta studies to maximize product yield of plant species producing active compounds have to be performed to provide a continuing supply of bioactive materials. Chemical characterization of natural compounds, determination of mode of action by kinetics and other spectroscopic methods (MS, X-ray, NMR), as well as in vitro and in vivo biological assays, chemical derivatization, and structure-activity relationships have to be carried out to provide a thorough knowledge on which to base the search for natural compounds or their derivatives with biological activity.

The stable oxygen and carbon isotopic composition of caliche in fluvial and supratidal rocks of the Abo Formation (Permian), south-central New Mexico, is controlled by palecoclimate and depositional environment. Fluvial caliche consists of low-Mg calcite nodules and vertically oriented tubules that display stage II texture. Micrite matrix support, brecciation, ooids/pisoliths, aveolar-septal texture, and peloids are common in the fluvial caliche and, along with red color and slickensides in the host shale, indicate pedogenesis in a well-oxidized vadose zone. In contrast, periodic waterlogging of the supratidal paleosols, probably due to high water table, is indicated by drab colors, carbonaceous flecks, horizontal rhizoliths, and the paucity of vadose textures in the stage II caliche nodules.Stable oxygen isotopes are similar in the fluvial and supratidal caliches and range from 21.6 to 30.5 parts per thousand (SMOW). The data exhibit a crude bimodality and delta-O-18 enrichment with a decrease in age (higher in the section). Consideration of these data in the context of delta-temperature relations suggests that 1) surface waters responsible for caliche formation increased in delta-O-18 (from roughly -8 to + 1 parts per thousand) over the 18 m.y. time interval that separated the lowest stratigraphic nodule horizon from the highest, 2) the increasing delta-O-18 values also reflect a warming trend (approximately 15-degrees to nearly 30-degrees-C) in the mean monthly temperature over this same time period, with perhaps an associated increase in Permian ocean temperatures, and 3) the significant variation in delta-O-18 from oldest to youngest caliche was probably enhanced by the amount effect, such that as the temperature increased, the amount of precipitation decreased, resulting in high delta-O-18 values.Caliches in the Abo are enriched in heavy carbon (-7.2 to -1.5 part per thousand PDB) compared to that of soil carbonate derived exclusively from C3 plants (-12 part per thousand PDB), and the supratidal caliches contain somewhat heavier carbon compared to the fluvial caliche. The delta-C-13 values for both environments increase with a decrease in caliche age. These results indicate that as the temperature increased and rainfall decreased with time, the level of C3 plant productivity apparently declined, allowing a greater influx of atmospheric CO2 into the soil. This can only occur when soil respiration rates are quite low or at very shallow depths (less than 10 cm), or both. Atmospheric CO2 seems to have invaded the supratidal soils to a somewhat greater extent than the fluvial soils.
