Objective: To verify, in extremely preterm infants, if disagreement between obstetricians and neonatologists regarding proactive management is associated with early death.Study Design: Prospective cohort of 484 infants with 23 0/7 to 266/7 weeks, without malformations, born from January 2006 to December 2009 in eight Brazilian hospitals. Pro-active management was defined as indication of ≥1 dose of antenatal steroid or cesarean section (obstetrician) and resuscitation at birth according to the international guidelines (neonatologist). Main outcome was neonatal death in the first 24 h of life.Result: Obstetricians and neonatologists disagreed in 115 (24%) patients: only neonatologists were proactive in 107 of them. Disagreement between professionals increased 2.39 times the chance of death in the first day (95% confidence interval 1.40 to 4.09), adjusted for center and maternal/neonatal clinical conditions.Conclusion: In infants with 23 to 26 weeks of gestation, disagreement between obstetricians and neonatologists, translated as lack of antenatal steroids and/or vaginal delivery, despite resuscitation procedures, increases the odds of death in the first day. © 2012 Nature America, Inc.

Doppler ultrasound enables the assessment of blood flow in real time and has been widely used to aid in pregnancy diagnosis and monitoring in dogs and cats. This technique has a diferencial when compared with the B-mode ultrasound, as it allows the evaluation of vascular hemodynamic characteristics, while the two-dimensional ultrasound allows the diagnosis of pregnancy and fetal viability. Thus, the objective of this review is to describe how circulation is characterized in the uterine and fetal arteries in bitches during pregnancy using Doppler ultrasonography.

The modernization of image equipaments associate to the veterinary professionals qualification allowed an advance in diagnosis veterinary medicine, and, consequently, in premature pregnancy diagnosis in bitches. Currently, owners and creators search for information related to pregnancy detection, fetal development and viability and litter size determination. Through diagnosis methods as radiology and ultrasonography, associated to clinics aspects, the diagnosis of early pregnancy become more acurated and precise, allowing more quality in the accompaniment of prenatal of these bitches. Ultrasonographic exams help the accompaniment of these pregnancies through of visuals recourses, which can supply information related to pregnancy detection, embrionary and fetal development, fetal viability e litter size determination through fetals mensurations. The radiographic study has as principal indication the fetal counting, and has been the most acurated for this analyses. It may be done after the da s after the luteini ing hormone surge, when occurs the fetal mineralization. Due to its importance in the present time and in small animals clinic, the objective of this study is to discuss the main radiographics and ultrassonographics aspects of the diagnosis of pregnancy in bitches

Identificaram-se, por ultrassonografia, os ovários fetais e o sexo dos fetos em uma loba-guará. Ao exame ultrassonográfico, foi possível identificar estruturas fetais e realizar medidas (relação entre diâmetro cranial e abdominal dos fetos) que indicavam idade fetal de 59 dias. Observaram-se: estruturas torácicas e abdominais formadas, diafragma, membros, estruturas do crânio definidas, coluna vertebral, medula, costelas, batimentos cardíacos normais, movimentação fetal, câmaras e valvas cardíacas, grandes vasos, rins, peristaltismo intestinal e ovários fetais. O exame ultrassonográfico foi eficaz nessa espécie, pois foi possível observar estruturas fetais para a avaliação da viabilidade fetal e, também, identificou-se a imagem ovariana em um dos fetos, mostrando-se importante para estudos de sexagem fetal.

Objective To assess the viability of the early diagnosis of fetal gender in material plasma before 7 weeks of pregnancy by real-time polymerase chain reaction (real-time PCR), starting at 5 weeks of pregnancy.Method peripheral blood was collected from pregnant women, starting at 5 weeks of gestation. After centrifugation, plasma was separated for fetal DNA extraction. DNA was analyzed by quantitative real-time PCR for two genomic regions, one on the Y chromosome (DYS-14) and the other shared by both sexes (beta-globin), by the TaqMan Minor Groove Binder (MGB) probe assay. The results of the examinations were compared to fetal gender determined after delivery.Results A total of 79 examinations of fetal DNA in maternal plasma were performed for 52 pregnant women. Accuracy according to gestational age was 92.6% (25 of 27 cases) at 5 weeks, and 95.6% (22 of 23 cases) at 6 weeks. These results also demonstrate that fetal DNA is present at low concentrations in maternal plasma at 5 weeks (8.5 genome equivalents (GE)/mL) and 6 weeks (34.1 GE/mL) of pregnancy.Conclusion Quantitative real-time PCR and TaqMan MGB probes specific for the detection of fetal gender in maternal plasma starting at 5 weeks of gestation have good sensitivity and excellent specificity. Purpose: to verify the viability of early diagnosis of fetal gender in maternal plasma by the real-time polymerase chain reaction (real-time PCR) starting at the 5th week of pregnancy. Methods: peripheral blood was collected from pregnant women with single fetus starting at the 5th week of gestation. After centrifugation, 0.4 mL plasma was separated for fetal DNA extraction. The DNA was analyzed in duplicate by real-time PCR for two genomic regions, one of the Y chromosome and the other common to both sexes, through the TaqMan® method, which uses a pair of primers and a fluorescent probe. Patients who aborted were excluded. Results: a total of 79 determinations of fetal DNA in maternal plasma were performed in 52 pregnant women. The results of the determinations were compared to fetal gender after delivery. Accuracy according to gestational age was 92.6% (25 of 27 cases) at 5 weeks with 87% sensitivity, and 95.6% (22 of 23 cases) at 6 weeks with 92% sensitivity. Starting at the 7th week of pregnancy, accuracy was 100% (29 of 29 cases). Specificity was 100% regardless of gestational age. Conclusion: real-time PCR for the detection of fetal gender in maternal plasma starting at the 5th week of gestation has good sensitivity and excellent specificity. There was agreement of the results in 100% of the cases in which male gender was diagnosed, regardless of gestational age, and from the 7th week of gestation for female gender diagnosis.
