The Brazilian Consensus on Gastroesophageal Reflux Disease considers gastroesophageal reflux disease to be a chronic disorder related to the retrograde flow of gastroduodenal contents into the esophagus and/or adjacent organs, resulting in a variable spectrum of symptoms, with or without tissue damage. Considering the limitations of classifications currently in use, a new classification is proposed that combines three criteria - clinical, endoscopic, and pH-metric - providing a comprehensive and more complete characterization of the disease. The diagnosis begins with the presence of heartburn, acid regurgitation, and alarm manifestations (dysphagia, odynophagia, weight loss, GI bleeding, nausea and/or vomiting, and family history of cancer). Also, atypical esophageal, pulmonary, otorhinolaryngological, and oral symptoms may occur. Endoscopy is the first approach, particularly in patients over 40 yr of age and in those with alarm symptoms. Other exams are considered in particular cases, such as contrast radiological examination, scyntigraphy, manometry, and prolonged pH measurement. The clinical treatment encompasses behavioral modifications in lifestyle and pharmacological measures. Proton pump inhibitors in manufacturers' recommended doses are indicated, with doubling of the dose in more severe cases of esophagitis. The minimum time of administration is 6 wk. Patients who do not respond to medical treatment, including those with atypical manifestations, should be considered for surgical treatment. Of the complications of gastroesophageal reflux disease, Barrett's esophagus presents a potential development of adenocarcinoma; biopsies should be performed, independent of Barrett's esophagus extent or location. In this regard the designation short Barrett's is not important in terms of management and prognosis. © 2002 by Am. Coll. of Gastroenterology.

Amitraz (AM) and romifidine (RMF), two alpha-2 adrenoceptor agonists, produce sedative effect and decrease spontaneous locomotor activity (SLA) of horses. The behavioral effects and sedation after intravenous injection of RMF (0.06mg/kg) or AM 0.1mg/kg (AM 0.1) or AM 0.4mg/kg (AM 0.4) were compared in horses. RMF caused head ptosis (HP) until 45 min. The lower AM dose induced HP from 45 to 60 minutes and from 120 to 150 minutes, and the higher dose induced HP until 180 minutes. Data concerning changes in SLA were not conclusive. RMF or AM 0.4 caused a greater sedation than AM 0.1 until 20 min. After 20 minutes, the sedation caused by AM 0.4 was greater than that of RMF or AM 0.1. Romifidine caused consistent sedation until 45 minutes. The amitraz emulsion produced a dosedependent sedation until 180 minutes. Comparing to romifidine, the emulsion of amitraz induced a more substantial sedation. At dosages and dilution applied, amitraz is an effective sedative for horses.

This paper characterizes humic substances (HS) extracted from soil samples collected in the Rio Negro basin in the state of Amazonas, Brazil, particularly investigating their reduction capabilities towards Hg(II) in order to elucidate potential mercury cycling/volatilization in this environment. For this reason, a multimethod approach was used, consisting of both instrumental methods (elemental analysis, EPR, solid-state NMR, FIA combined with cold-vapor AAS of Hg(0)) and statistical methods such as principal component analysis (PCA) and a central composite factorial planning method. The HS under study were divided into groups, complexing and reducing ones, owing to different distribution of their functionalities. The main functionalities (cor)related with reduction of Hg(II) were phenolic, carboxylic and amide groups, while the groups related with complexation of Hg(II) were ethers, hydroxyls, aldehydes and ketones. The HS extracted from floodable regions of the Rio Negro basin presented a greater capacity to retain (to complex, to adsorb physically and/or chemically) Hg(II), while nonfloodable regions showed a greater capacity to reduce Hg(II), indicating that HS extracted from different types of regions contribute in different ways to the biogeochemical mercury cycle in the basin of the mid-Rio Negro, AM, Brazil. The third generation (G-3) polypropylenimine hexadecaamine dendrimer (DAB-Am-16) was grafted onto the silica gel surface. The modified silica interacts easily with Cu(2+) and then with potassium ferrocyanide to form copper hexacyanoferrate. The modified silica was characterized by the following techniques: Nuclear Magnetic Resonance (NMR), FTIR spectroscopy, Energy Dispersive X-ray (EDX) and Cyclic Voltammetry. The modified silica containing copper hexacyanoferrate (CuHCFSD) was tested for a voltammetric determination of nitrite using a modified graphite paste electrode.

Objective To compare the effects of decompressive surgery (DSX), electroacupuncture (EAP), and DSX followed by EAP (DSX + EAP) for the treatment of thoracolumbar intervertebral disk disease (IVDD) in dogs with severe neurologic deficits of > 48 hours' duration.Design Retrospective case series and prospective clinical trial.Animals-40 dogs between 3 and 6 yEAPs old and weighing between 10 and 20 kg (22 and 44 lb) with long-standing (>48 hours) clinical signs of severe neurologic disease attributable to thoracolumbar IVDD.Procedures Thoracolumbar medullar injury was classified on the basis of neurologic signs by use of a scale ranging from 1 (least severe) to 5 (most severe). The DSX dogs (n = 10) were retrospectively selected from those that underwent DSX for the treatment of thoracolumbar IVDD. In addition, 19 dogs received EAP alone and 11 dogs underwent DSX followed by EAP (DSX + EAP). Outcome was considered a clinical success when a dog initially classified as grade 4 or 5 was classified as grade 1 or 2 within 6 months after the end of treatment.Results The proportion of dogs with clinical success was significantly higher for dogs that underwent EAP (15/19) than for dogs that underwent DSX (4/10); the proportion of dogs with clinical success for dogs that underwent DSX + EAP was intermediate (8/11).Conclusions and Clinical Relevance EAP was more effective than DSX for recovery of ambulation and improvement in neurologic deficits in dogs with long-standing severe deficits attributable to thoracolumbar IVDD. The predation on vertebrates by birds, including bats, is very common in some families (Accipitridae, Falconidae, Tytonidae, Strigidae), constituting their main diet. For other families (except those that feed strictly on fish), it is occasional and sometimes a matter of opportunity. Here we recorded the predation on a bat (Platyrrhinus lineatus) by the neotropical bird plush-crested jay (Cyanocorax chrysops). on July 26, 2009, around 11:00 am, we recorded an individual of C. chrysops taking part of a P. lineatus on its beak. This record occurred on Jacarezinho Farm (Valparaiso [long dash] SP). The prey species identification was done by visual observation, considering the bat size and its external morphology, especially by the presence of the white dorsal bright stripe. Probably, Platyrrhinus lineatus behavior, which involves living together or in couples on tree canopies, made the capture by the plush-crested jay easier. This is a new record for the diet of C. chrysops and highlights the necessity of additional studies related to birds diet in the Neotropics, even the more conspicuous ones.
