Obstructive Sleep Apnea Syndrome (OSAS) is characterized by snoring and / or repetitive pauses in breathing during sleep associated with hypoxemia and hypercapnia. Affects approximately 1-3% of children, especially preschoolers. There is an association of OSAS with negative impact on neurocognitive development. However, there are few studies using protocols to assess cognition and behavioral disorders in children with OSAS. The objective of this study was to analyze the major studies related to the topic within the last two decades. The main studies published between 1995 to 2011 were identified through MEDLINE and LILACS databases. We selected the most relevant for OSAS in children, neurocognitive effects and association between obstructive disorders / OSAS in the target population (children). It was concluded that the studies analyzed showed that there is strong evidence regarding the overlapping of OSAS in children with neurocognitive disorders and that treatment of OSAS has positive impact on neurocognitive development.

Introduction: Obstructive sleep apnea syndrome is related to cardiopulmonary complications in children. It is important to know its patophysiology and possible complications to help reduce risks in this group. Aims: To report three cases of severe cardiorespiratory complications of obstructive sleep apnea managed in the intensive care unit (ICU). Case report: Two children with no previous diagnosis of obstructive sleep apnea syndrome suffered acute congestive heart failure and acute lung oedema with need of ICU and improved after adenotonsillectomy. In a third case, the patient had acute lung oedema as a complication after adenotonsillectomy. Conclusions: Paediatricians and otolaryngologists must be aware of the clinical manifestations of severe sleep apnea. Early referring to treatment and special attention at pre and post surgical periods are essentials to avoid serious complications.

Introdução: A fibromialgia é uma doença caracterizada principalmente por dor músculo-esquelética difusa e crônica. Os distúrbios do sono em fibromiálgicos podem ter papel não somente na etiologia da dor crônica, mas também na perpetuação dos sintomas, portanto o estudo das condições do sono nestes pacientes é relevante na medida em que possibilita um entendimento das condições fisiopatológicas e permite uma abordagem ampla para o tratamento da fibromialgia. Objetivo: Apresentar a relação entre os distúrbios do sono e os sintomas da fibromialgia e sua importância na avaliação dos fibromiálgicos. Metodologia: Foi realizado levantamento bibliográfico do período de 1990-2009, nas bases de dados Medline, Pubmed, Lilacs e Scielo, através das palavras chaves: sleep, fibromyalgia, sleep disorders, insomnia, pain. Resultados: A literatura demonstra uma alta prevalência de distúrbios do sono em pacientes portadores de fibromialgia e uma relevante relação entre as anormalidades no padrão do sono e o sintoma da dor crônica.

The Alzheimer's dementia (AD) is the most prevalent form of dementia in the elderly. Among the harmful changes arising from the DA, there may be sleep disturbance and impaired quality of life. Physical activity is an important non-pharmacological feature that would provide positive effects for the treatment of disease. However, there are still few studies that clarify the effects of resistance training in this population. Thus, the present study has as objective analyzes the effects of the resistance training in sleep disturbance and quality of life of patients with AD. Participated in this study 22 patients with clinical diagnosis of AD divided into two groups: Training Group (TG) and Social Interaction Group (SIG). The subjects of TG underwent a resistance training protocol, appropriate to the individual conditions, during three days no consecutive in the week, with duration of 60 minutes each session, for 16 weeks. The SIG won't participate in any systematized physical activity, but they realized other activities, such as reading, poetry, hiking and painting, among others. This protocol also was realized three times in week for 16 weeks, with duration of 60 minutes each session. Both groups were assessed at the beginning of the program and after 16 weeks. For the assessment of sleep disturbance was used the Mini Sleep Questionnaire and to assess of quality of life was used the Scale for Assessment of Quality of Life in Alzheimer's disease (QL), versions: patient, caregiver, family and the final score. Due to the scalar nature of the data and small sample, all variables were analyzed by nonparametric statistics. Difficulties with sleep are frequent in the pre-schooler children. The aim of this study was to present a case report of behavioural intervention by parental guidance to sleeping problems in a pre-schooler child. Participated in this study a four years old boy that had difficulties to sleep without the presence of his parents and his mother who was oriented through a parental program. The program of intervention was composed by five sessions in which the mother acquired education about the child’s sleep, received orientation about the establishment of time and routine to sleep and about the use of techniques (extinction and positive reinforcement) for the improvement of the sleep difficulties. The sleep and behaviour were evaluated in four periods (pre-treatment, post-treatment, one and six months follow up) according to the following instruments: 1) UNESP Scale of Habits and Hygiene of the Sleep – Children Version, 2) Scale of Sleep Disorders for Children and Teenagers, 3) Child Behavior Checklist (CBCL 1,5-5) and 4) sleep diary. The results showed that after intervention the child developed independent sleep, reduced the bed resistance and showed improvement in daily behaviors. To conclude: a behavioral intervention by parental guidance was effective to sleep problems of the child.

Purpose: To determine the frequency of glaucoma and evaluate the behavior of 24-hour intraocular pressure in patients with the obstructive sleep apnea syndrome (OSAS). Methods: Eleven consecutive patients with OSAS, diagnosed by polysonography, were avaliated in a cross-sectional study. Demographic data were analyzed: age, sex, race/color, weight, height and associated diseases. The patients were submitted to complete ophthalmologic examination, including the visual field, as well as to 24-hour intra-ocular pressure (IOP) evaluation by an applanation tonometer at 9h, 12h, 15h, 18h, 24h and 6h in the lying and sitting positions. The diagnostic criterion for glaucoma was alteration of the visual field (VF) compatible with glaucoma and one or more of the following alterations: cup-disc ratio >= 0.7, hemorrhage, wedge-shaped defect, bayonet-shaped vessels, Hoyt's sign, asymmetry > than 0.2 between cup/disc ratio of the eyes. The angle should be opened without alterations. Results: 9 (82%) of 11 patients showed glaucoma or were suspected to have glaucoma, 9% of which exhibited normal tension glaucoma and 73% were suspected to have glaucoma for presenting alterations in the optic nerve or ocular hypertension. The mean for the IOP values of the 11 patients was observed to be the highest at 6 o'clock, when they were lying down. Variations of IOP >= 5 mmHg occurred in 7 (64%) of the patients, and variations of up to 14 mmHg and IOP peaks of up to 32 mmHg were observed. Conclusion: OSAS may be an important risk factor for the development of glaucoma, particularly that of normal tension glaucoma. Patients with OSAS must be referred to an ophthalmologist and those professionals must be attentive to the association of sleep disorders in patients with open-angle glaucoma.
