Handgrip strength as a predictor of bone mineral density in cirrhotic outpatients

Osteoporosis is well recognized as a cirrhosis complication; however, most studies assessing this condition included only patients on liver transplantation lists with an elevated rate of bone diseases. While general population studies show that handgrip strength is clearly associated with bone mineral density, until now this tool has not been applied to cirrhotic patients in relation to their bone condition. This study aimed to evaluate whether handgrip strength, bone and liver tests may be useful as predictors of bone disease in cirrhotic outpatients. 129 subjects were included (77 men and 52 women). Dual energy X-ray absorptiometry was applied to evaluate lumbar-spine and femoral-neck T scores. Osteoporosis/osteopenia rates were 26.3%/35.6% in the lumbar spine and 6.9%/41.8% in the femoral neck, respectively. Model selections were based on backward procedures to find the best predictors of low T scores. For lumbar spine, only low handgrip strength and high parathyroid hormone levels were clearly related to low T scores. For femoral neck, only age was associated with low T scores. Handgrip strength may serve as an effective predictor of low lumbar spine T score among cirrhotic outpatients. As cirrhosis affects the lumbar spine more than the femoral neck, these results suggest that handgrip strength should be tested in all cirrhotic patients as a first indicator of bone health. New laws for author's rights: adequate for digital preservation?

Introduction. Besides technical and economic-organisational problems, digital preservation also faces legal issues, especially regarding copyright legislation, since all digital preservation strategies involve copying materials and/or using software which is typically copyrighted. The purpose of this paper is to ascertain the extent to which current copyright laws meet the preservation requirements of library materials..Methodology. A cross-sectional analysis of recently updated national copyright laws as well as the impact of the other two protection methods: contractual and technological.Results. Even after the latest updates current copyright legislation is almost useless for digital preservation activities since the opportunities provided by WIPO Copyright Treaty to adapt and extend copyright exceptions and limitations have been used to the full.Conclusion. We need a legislation reform that will make it possible to carry out all required copying and communication activities and software use, even if circumventing technological protection is needed. But that is not enough for licensed works. The best solution for this kind of work is to include specific clauses in the licences that facilitate preservation activities. Thus, cooperation between both parties, libraries and rights' holders, is essential.

A Educação Moral e a Educação para a Cidadania implicam na reflexão sobre qual é a sociedade que queremos. É somente a partir dessa definição que se torna possível discutir a contribuição da instituição escolar no processo de transformação social e, por conseguinte a organização de um ambiente pedagógico visando a formação do cidadão enquanto EU inserido num contexto universal propiciando as condições necessárias ao desenvolvimento de pessoas solidárias, fraternas, com capacidade de discutir, questionar, cooperar e transformar o meio em que vive. Ao estabelecer uma relação entre Educação Moral e Educação para a Cidadania estou priorizando, na ação pedagógica, o trabalho com a construção do conceito de cooperação, igualdade e justiça, conceitos estes que permitirão a compreensão e a construção, pelo sujeito, dos princípios éticos subjacentes aos direitos humanos e ao conceito de democracia enquanto pilares da organização de uma sociedade livre, igualitária e justa.
