Police officers are exposed to impact noise coming from firearms, which may cause irreversible injuries to the hearing system.Aim: To evaluate the noise exposure in shooting stands during gunfire exercises, to analyze the acoustic impact of the noise produced by the firearms and to associate it with tonal audiometry results.Study design: Cross-sectional.Materials and methods: To measure noise intensity we used a digital sound level meter, and the acoustic analysis was carried out by means of the oscillations and cochlear response curves provided by the Praat software. 30 police officers were selected (27 males and 3 females).Results: The peak level measured was 113.1 dB(C) from a .40 pistol and 116.8 dB(C) for a .38 revolver. The values obtained for oscillation and Praat was 17.9 +/- 0.3 Barks, corresponding to the rate of 4,120 and 4,580 Hz. Audiometry indicated greater hearing loss at 4,000Hz in 86.7% of the cases.Conclusion: With the acoustic analysis it was possible to show cause and effect between the main areas of energy excitation of the cochlea (Praat cochlear response curve) and the frequencies of low hearing acuity.

O trato olivococlear medial realiza o controle eferente das células ciliadas externas, regulando as contrações lentas e atenuando as rápidas. Com a pesquisa da amplitude das emissões otoacústicas sem e com estimulação acústica contra, ipsi ou bilateralmente, é possível estimar as condições desse trato, uma vez que o efeito resultante de redução/supressão das emissões indica seu funcionamento. O envelhecimento implica em diminuição da atividade do sistema auditivo central, em função da degeneração das estruturas envolvidas nas habilidades auditivas. OBJETIVO: O objetivo foi investigar o efeito da idade na atividade do trato sobre a cóclea, com a análise da amplitude das emissões com estimulação acústica contralateral. MATERIAL E MÉTODO: A casuística foi composta por 75 indivíduos agrupados conforme a idade. A metodologia foi o modo convencional, com clique linear e o ruído branco. ESTUDO DE CASO: A análise considerou a resposta das orelhas e a comparação entre os grupos. RESULTADOS: Os resultados revelam diferenças estatisticamente significantes entre o response das emissões sem e com estimulação acústica contralateral, nos indivíduos (20 a 39 anos). O efeito redução/supressão das emissões diminui com a idade (quarta década). CONCLUSÃO: O envelhecimento prejudica a efetividade da atividade do trato.

O audiograma ou o relato do paciente não é suficiente para determinar a presença ou não de zonas mortas na cóclea (ZMC), nem identificar sua extensão. OBJETIVO: Investigar, utilizando o teste TEN, ZMC de indivíduos com perda auditiva neurossensorial (PANS). CEDALVI/ HRAC-USP-Bauru, de agosto de 2003 a fevereiro de 2004. TIPO DE ESTUDO: Estudo de coorte contemporânea com corte transversal. Casuística e Métodos: O TEN foi aplicado nos grupos G1 (5 mulheres com limiares tonais aéreos dentro do padrão de normalidade); G2 (4 mulheres e 5 homens com PANS moderada plana); G3 (19 mulheres e 24 homens com PANS com o grau variando entre leve a profundo). RESULTADOS: Para G1, o valor de TEN para eliminar o tom de teste foi, em média, próximo ao limiar absoluto para todas as freqüências. Não foi observada ZMC em nenhuma das orelhas testadas do G2. Para as 76 orelhas do G3, 6 não apresentaram indício de ZM. CONCLUSÕES: O TEN é efetivo para indicar ZMC em indivíduos com PANS descendente. Há evidência de diferença na detecção do tom puro na presença de ruído entre indivíduos com PANS em altas freqüências e com PANS plana, pois se observou diferença significativa entre o limiar mascarado e absoluto apenas para PANS descendentes e não para as planas.

Two experiments evaluated an operant procedure for establishing stimulus control using auditory and electrical stimuli as a baseline for measuring the electrical current threshold of electrodes implanted in the cochlea. Twenty-one prelingually deaf children, users of cochlear implants, learned a Go/No Go auditory discrimination task (i.e., pressing a button in the presence of the stimulus but not in its absence). When the simple discrimination baseline became stable, the electrical current was manipulated in descending and ascending series according to an adapted staircase method. Thresholds were determined for three electrodes, one in each location in the cochlea (basal, medial, and apical). Stimulus control was maintained within a certain range of decreasing electrical current but was eventually disrupted. Increasing the current recovered stimulus control, thus allowing the determination of a range of electrical currents that could be defined as the threshold. The present study demonstrated the feasibility of the operant procedure combined with a psychophysical method for threshold assessment, thus contributing to the routine fitting and maintenance of cochlear implants within the limitations of a hospital setting.

The brain stem auditory-evoked potential (BAEP) is an electrophysiologic test that detects and records the electrical activity in the auditory system from cochlea to midbrain, generated after an acoustic stimulus applied to the external ear. The aim of this study is to obtain normative data for BAEP in Dalmatian dogs in order to apply this to the evaluation of deafness and other neurologic disorders. BAEP were recorded from 30 Dalmatian dogs for a normative Brazilian study. Mean latencies for waves I, III, and V were 1.14 (±0.09), 2.62 (±0.10), and 3.46 (±0.14) ms, respectively. Mean inter-peak latencies for I-III, III-V, and I-V intervals were 1.48 (±0.17), 0.84 (±0.12), and 2.31 (±0.18) ms, respectively. Unilateral abnormalities were found in 16.7% of animals and bilateral deafness was seen in one dog. The normative data obtained in this paper is compatible with other published data. As far as we know this is the first report of deafness occurrence in Dalmatian dogs in Brazil.

Brainstem auditory evoked potential (BAEP) reflects the electrical activity along the auditory pathway, from the cochlea to the brainstem, and contributes for the diagnosis of deafness in dogs. BAEP recording may require chemical restraint in some cases, so this study was designed to analyze the impact of sedation with morphine and acepromazine on the BAEP recordings of 16 dogs with normal hearing. BAEPs were recorded before and during sedation with a combination of morphine (0.5mgkg(-1)) and acepromazine (0.05mgkg(-1)) given intramuscularly. The protocol employed allowed safe and effective animal restraint. Sedation increased the latency of waves II and III and intervals I-III and I-V but did not interfere with wave identification. and showed to be safe in the dogs tested. Based on the current literature this is the first study which assessed the impact of sedation on BAEPs in dogs in Brazil.

Frontonasal dysplasia (FND) is a rare malformative complex affecting the frontal portion of the face, the eyes and the nose; it may occur singly or associated with other clinical signs. No systematic studies describing hearing in this condition were found. AIM: To evaluate hearing sensitivity and sound stimulus conduction from cochlea to brainstem in patients with clinical signs of FND. METHODS: 21 patients with isolated or syndromic FND were submitted to a clinical (otological/vestibular antecedents and otoscopy) and instrumental (pure tone and speech audiometry, tympanometry and brainstem auditory evoked response) hearing evaluation. DESIGN: A clinical, cross-sectional observational prospective study. RESULTS: Hearing thresholds were normal in 15 (70%) patients, abnormal in 5 (25%), mostly with conductive hearing loss; one patient did not cooperate with testing. The tympanometric curve was type A in 30 (72%) ears, type C in 5 (12%), type As in 4 (9%) and type B in 3 (7%). The auditory brainstem response (ABR) showed no abnormalities. CONCLUSION: Patients with FND showed no abnormalities in the auditory system from cochlea to brainstem in this study. Mild conductive hearing loss found in some is probably related to cleft palate. Further evaluation of hearing pathways at higher levels is recommended.

Las neuronas de la raíz coclear son las primeras neuronas del sistema nervioso central a recibir la información auditiva proveniente de la cóclea y se conectan con centros de integración sensoriomotora del tronco encefálico, especialmente con el núcleo reticular caudal del puente. Funcionalmente las neuronas de la raíz coclear están envueltos en el circuito elemental del reflejo auditivo de sobresalto juntamente con las células ganglionares del órgano de Corti, el núcleo reticular caudal del puente y las motoneuronas de la médula espinal. El reflejo auditivo de sobresalto presenta una serie de modulaciones como la habituación, la sensibilización, la inhibición por estímulo previo y la potenciación por un estímulo adverso. Las alteraciones en la reacción refleja de sobresalto y sus diferentes modulaciones poseen valor diagnóstico en la clínica médica de enfermedades neurodegenerativas y psiquiátricas como Parkinson y Esquizofrenia. Las modulaciones del reflejo auditivo de sobresalto ocurren mediante la influencia de diversos núcleos sobre los componentes del circuito elemental de este reflejo. El núcleo menos estudiado del circuito neural del reflejo auditivo de sobresalto es el núcleo de la raíz coclear. El patrón de conectividad eferente de las neuronas de la raíz coclear es bien conocido, sin embargo poco se conoce sobre los orígenes de sus aferencias y la identidad neuroquímica de las mismas. Estudios previos demostraron que el soma y en las dendritas de las neuronas de la raíz coclear están cubiertos por botones sinápticos de cuatro tipos, sugiriendo que existan diversos orígenes para estos terminales axonales, con características neuroquímicas propias. El conocimiento de las aferencias a las neuronas de la raíz coclear es relevante para comprender cual el papel del núcleo de la raíz coclear en las modulaciones del reflejo auditivo de sobresalto. PURPOSE: to analyze the levels of noise emitted by nationally-sold toys for use by children from 1 to 5 year old; to compare the values among the toys with and without the seal of Inmetro (National Institute of Metrology, Standardization and Industrial Quality) and to analyze the sound spectrum of toys, in order to identify the area of the cochlea that may be more affected by these noises. METHOD: measurements were performed on 20 sound toys (10 with the seal of the Inmetro and 10 without the seal) with the use of digital sound level meter in an acoustically treated room, and the sound analysis was performed using the Praat program. RESULTS: toys placed at 2.5 cm from the equipment with the seal of the Inmetro had an intensity ranging from 61.50 to 91.55 dB (A) and from 69.75 to 95.05 dB (C), positioned at 25 cm ranged from 58.3 to 79.85 dB (A) and from 62.50 to 83.65 dB (C). The results of the toys without warranty stamps placed at 2.5 cm ranged from 67.45 to 94.30 dB (A) and 65.4 to 99.50 dB (C) and the distance of 25 cm recorded from 61. 30 to 87.45 dB (A) and 63.75 to 97.60 dB (C), so that the findings demonstrated that there are noisy toys that go beyond the values recommended by the current legislation in both groups, with and without warranty stamps . CONCLUSION: the toys without the seal of Inmetro showed intensities values significantly higher than the other group, offering more risk to the children s hearing health.
