This study analyzes an accident in which two maintenance workers suffered severe burns while replacing a circuit breaker panel in a steel mill, following model of analysis and prevention of accidents (MAPA) developed with the objective of enlarging the perimeter of interventions and contributing to deconstruction of blame attribution practices. The study was based on materials produced by a health service team in an in-depth analysis of the accident. The analysis shows that decisions related to system modernization were taken without considering their implications in maintenance scheduling and creating conflicts of priorities and of interests between production and safety; and also reveals that the lack of a systemic perspective in safety management was its principal failure. To explain the accident as merely non-fulfillment of idealized formal safety rules feeds practices of blame attribution supported by alibi norms and inhibits possible prevention. In contrast, accident analyses undertaken in worker health surveillance services show potential to reveal origins of these events incubated in the history of the system ignored in practices guided by the traditional paradigm.

Workplace accidents involving machines are relevant for their magnitude and their impacts on worker health. Despite consolidated critical statements, explanation centered on errors of operators remains predominant with industry professionals, hampering preventive measures and the improvement of production-system reliability. Several initiatives were adopted by enforcement agencies in partnership with universities to stimulate production and diffusion of analysis methodologies with a systemic approach. Starting from one accident case that occurred with a worker who operated a brake-clutch type mechanical press, the article explores cognitive aspects and the existence of traps in the operation of this machine. It deals with a large-sized press that, despite being endowed with a light curtain in areas of access to the pressing zone, did not meet legal requirements. The safety devices gave rise to an illusion of safety, permitting activation of the machine when a worker was still found within the operational zone. Preventive interventions must stimulate the tailoring of systems to the characteristics of workers, minimizing the creation of traps and encouraging safety policies and practices that replace judgments of behaviors that participate in accidents by analyses of reasons that lead workers to act in that manner.

Air accidents represent a small proportion of the flights registered worldwide. Airplane collisions in the air are rare. In September of 2006, a Boeing 737-800 collided in midair with a Legacy Jet. It was the largest accident registered in the history of Brazilian aviation until that time. The present study explores aspects of press coverage of the accident. Data and information reported in the media about the accident from September 2006 to August 2007 were collected and discussed. Media coverage called attention to two unusual aspects: politicisation of the discussion, culminating in the opening of congressional inquiries, and equally the concomitance of police investigations interfering in the work of agencies responsible for the official accident investigation. Emphasis on assigning guilt and establishing penalties may close the windows of opportunity an accident had opened for discussions on the improvement of air safety. In Brazil, political imperatives and organizational pressures have interfered and the possibilities of organizational learning from the accident have been drastically curtailed.

Partindo de apresentação da proposta de análise de acidentes baseada no modelo sistêmico e não linear de acidentes, dito de ressonância funcional, desenvolvido por Erik Hollnagel, este texto discute críticas às concepções deterministas de acidente e aos modelos lineares de representação desses eventos. Aspectos do modelo de ressonância funcional são usados como eixo definidor de proposta de agenda de discussões a ser desenvolvida pelos interessados no tema da análise de acidentes, destacando-se: a) a defesa do abandono da idéia de causas de acidentes e sua substituição pela de explicação desses eventos; b) apresentação dos conceitos de variabilidade de desempenhos e adaptações locais e a proposta de sua utilização como bases para a identificação de perfis ou aspectos típicos de acidentes. Por fim, discute-se a necessidade de incorporação dos conceitos apresentados no debate sobre novos caminhos para as práticas de análises e de prevenção de acidentes em nosso país.

Neste artigo analisam-se os laudos e dados obtidos das investigações de acidentes graves e fatais do trabalho efetuadas pelo Instituto de Criminalística (IC), Regional de Piracicaba. Foram analisados 71 laudos de acidentes ocorridos em 1998, 1999 e 2000. Os acidentes envolvendo máquinas representam 38,0%, seguido pelas quedas de altura (15,5%) e em terceiro lugar os causados por corrente elétrica (11,3%). Os laudos concluem que 80,0% dos acidentes são causados por atos inseguros cometidos pelos trabalhadores, enquanto que a falta de segurança ou condição insegura responde por 15,5% dos casos. A responsabilização das vítimas ocorre mesmo em situações de elevado risco em que não são adotadas as mínimas condições de segurança, com repercussão favorável ao interesse dos empregadores. Observa-se que estas conclusões refletem os modelos explicativos tradicionais, reducionistas, em que os acidentes são fenômenos simples, de causa única, centrada via de regra nos erros e falhas das próprias vítimas. A despeito das críticas que tem recebido nas duas últimas décadas no meio técnico e acadêmico, esta concepção mantém-se hegemônica prejudicando o desenvolvimento de políticas preventivas e a melhoria das condições de trabalho.

This report describes the consequences and some aspects of the origin and development of victim blaming in accident analysis, and some methods for investigating such events, with particular emphasis on the situation in Brazil. In Brazil, the spread of this practice seems to have been helped by several factors. (1) the idea that occupational accidents are simple phenomena with a limited number of causal factors linked to unsafe actions and/or conditions. In the past, the theory of accident proneness had less influence than in other countries. (2) Government regulations that stipulate the hiring of health and safety officers, production of educational material, and preventive campaigns that emphasize the role of the victim's faulty behavior in the origin of an accident. (3) Mandatory implementation of standardized models for accident investigation directed toward searching for a single cause. Usually one conclusion, expressed in terms of unsafe acts or conditions, is formulated so that whoever performs an unsafe act is responsible for the accident. (4) Lack of knowledge, as shown in Brazilian publications on occupational accidents and in the evolution of studies on the nature of accident phenomena and of strategies adopted for their prevention.

This paper presents a brief review about site investigation procedures for contaminated sites recommended by Brazilian and Canadian environmental agencies as well as discusses the theme of geo-environmental investigation as applied to Brazilian practice. The main definitions on the theme are reviewed and some guidelines are proposed for conducting a geo-environmental investigation of Brazilian contaminated sites using different site and laboratory investigation techniques based on the presented review and on the experience obtained from the investigation of a solid waste disposal site in the interior of the state of São Paulo, Brazil.

This investigation was carried out within the Parana sedimentary basin, Brazil, involved the sampling of effluents and groundwater from monitoring stations situated at different sites at São Paulo State, and was realized with the purpose of evaluating the presence of fats, oil and grease (FOG) in different matrices. Several tests were realized with very distinct materials (cooking oil, butter, margarine, pig fat, vacuum pump oil) in order to properly calibrate the spectrophotometric system. Each matrix was dissolved with 1,1,1-trichloroethane and from the stock solutions it was prepared working standards from different dilutions. The data obtained were plotted on absorbance vs. concentration graph that yielded a successful calibration curve when a mineral oil for vacuum pump was utilized in the experiments at a wavelength corresponding to 410 mn. The results obtained for the analyzed samples were compared with the limiting value established by the São Paulo State legislation on the prevention and pollution control of the environment that was established in 8(th) September 1976 by Rule No. 8468.

There has been a rapid increase in the complexity and integration of many safety-critical systems. In consequence, it is becoming increasingly difficult to identify the causes of incidents and accidents back through the complex interactions that lead to an adverse event. At the same time, there is a growing appreciation of the need to consider a broad range of contextual factors in the aftermath of any mishap. A number of regulators, operators and research teams have responded to these developments by proposing novel techniques to support the analysis of complex, safety-critical incidents. It is important to illustrate these different approaches by applying them to a number of common case studies. The following pages, therefore, show how STAMP and AcciMap might support the Serviço Público Federal investigation into the explosion and fire of the Brazilian launch vehicle VLS-1 VO3. © 2006 Elsevier Ltd. A suit able decision-making on managing a contaminated site characterization program is strongly dependent of the diagnosis process. A detailed diagnosis can be done based on a Conceptual Site Model (CSM) elaboration using high resolution site characterization tools. The piezocone (CPTu) test is a high resolution tool which allows attaching several specific sensors, like the resistivity probe. This hybrid device is called the resistivity piezocone (RCPTu). A simulated geo-environmental site characterization program was performed on an erosion site using different tools (direct push tools soil samplers, hollow stem auger (HSA) drilling and RCPTu tests) to develop the CSM for a site similar to the Brazilian conditions. It was observed a good agreement between the site profiles interpreted by the different methods. The resistivity sensor attached to the piezocone improved the interpretation and the decision-making process on site was significantly better for the CSM elaboration. The RCPTu test data also allowed identifying the hydrogeological heterogeneities. The present study shows that the RCPTu test is also a useful and powerful tool to development an accurate CSM in a Brazilian condition, especially in an approach that prioritizes high resolution geo-environmental investigation. © 2013 Taylor & Francis Group.
