Background: Excessive gingival display (EGD) has a negative impact on a pleasant smile. Minimally invasive therapeutic modalities have become the standard treatment in many dentistry fields. Therefore, the aim of this study is to compare the clinical outcomes of open-flap (OF) and minimally invasive flapless (FL) esthetic crown lengthening (ECL) for the treatment of EGD.Methods: A split-mouth randomized controlled trial was conducted in 28 patients presenting with EGD. Contralateral quadrants received ECL using OF or FL techniques. Clinical parameters were evaluated at baseline and 3, 6, and 12 months post-surgery. The local levels of receptor activator of nuclear factor-kappa B ligand (RANKL) and osteoprotegerin (OPG) were assessed by enzyme-linked immunosorbent assay at baseline and 3 months. Patients' perceptions regarding morbidity and esthetic appearance were also evaluated. Periodontal tissue dimensions were obtained by computed tomography at baseline and correlated with the changes in the gingival margin (GM).Results: Patients reported low morbidity and high satisfaction with esthetic appearance for both procedures (P > 0.05). RANKL and OPG concentrations were increased in the OF group at 3 months (P < 0.05). Probing depths were reduced for both groups at all time points, compared with baseline (P < 0.05). There were no differences between groups for GM reduction at any time point (P > 0.05).Conclusions: FL and OF surgeries produced stable and similar clinical results up to 12 months. FL ECL may be a predictable alternative approach for the treatment of EGD.

Background: Little information on the factors influencing intraoperative cardiac arrest and its outcomes in trauma patients is available. This survey evaluated the associated factors and outcomes of intraoperative cardiac arrest in trauma patients in a Brazilian teaching hospital between 1996 and 2009.Methods: Cardiac arrest during anesthesia in trauma patients was identified from an anesthesia database. The data collected included patient demographics, ASA physical status classification, anesthesia provider information, type of surgery, surgical areas and outcome. All intraoperative cardiac arrests and deaths in trauma patients were reviewed and grouped by associated factors and also analyzed as totally anesthesia-related, partially anesthesia-related, totally surgery-related or totally trauma patient condition-related.Findings: Fifty-one cardiac arrests and 42 deaths occurred during anesthesia in trauma patients. They were associated with male patients (P<0.001) and young adults (18-35 years) (P = 0.04) with ASA physical status IV or V (P<0.001) undergoing gastroenterological or multiclinical surgeries (P<0.001). Motor vehicle crashes and violence were the main causes of trauma (P<0.001). Uncontrolled hemorrhage or head injury were the most significant associated factors of intraoperative cardiac arrest and mortality (P<0.001). All cardiac arrests and deaths reported were totally related to trauma patient condition.Conclusions: Intraoperative cardiac arrest and mortality incidence was highest in male trauma patients at a younger age with poor clinical condition, mainly related to uncontrolled hemorrhage and head injury, resulted from motor vehicle accidents and violence.

Objectives: The purpose of this study was to determine if intra-abdominal pressure (IAP) could predict acute renal injury (AKI) in the postoperative period of abdominal surgeries, and which would be its cutoff value. Patients and methods: A prospective observational study was conducted in the period from January 2010 to March 2011 in the Intensive Care Units (ICUs) of the University Hospital of Botucatu Medical School, UNESP. Consecutive patients undergoing abdominal surgery were included in the study. Initial evaluation, at admission in ICU, was performed in order to obtain demographic, clinical surgical and therapeutic data. Evaluation of IAP was obtained by the intravesical method, four times per day, and renal function was evaluated during the patient's stay in the ICU until discharge, death or occurrence of AKI. Results: A total of 60 patients were evaluated, 16 patients developed intra-abdominal hypertension (IAH), 45 developed an abnormal IAP (>7 mmHg) and 26 developed AKI. The first IAP at the time of admission to the ICU was able to predict the occurrence of AKI (area under the receiver-operating characteristic curve was 0.669; p=0.029) with the best cutoff point (by Youden index method) >= 7.68 mmHg, sensitivity of 87%, specificity of 46% at this point. The serial assessment of this parameter did not added prognostic value to initial evaluation. Conclusion: IAH was frequent in patients undergoing abdominal surgeries during ICU stay, and it predicted the occurrence of AKI. Serial assessments of IAP did not provided better discriminatory power than initial evaluation.

This paper presents a Computer Aided Diagnosis (CAD) system that automatically classifies microcalcifications detected on digital mammograms into one of the five types proposed by Michele Le Gal, a classification scheme that allows radiologists to determine whether a breast tumor is malignant or not without the need for surgeries. The developed system uses a combination of wavelets and Artificial Neural Networks (ANN) and is executed on an Altera DE2-115 Development Kit, a kit containing a Field-Programmable Gate Array (FPGA) that allows the system to be smaller, cheaper and more energy efficient. Results have shown that the system was able to correctly classify 96.67% of test samples, which can be used as a second opinion by radiologists in breast cancer early diagnosis. (C) 2013 The Authors. Hypothermia is a common phenomenon in the perioperative period, and it affects 60 to 90% of patients submitted to anesthetic-surgical procedures. In order to minimize its incidence, warming methods are used. Such methods can be passive, such as orthopedic cotton, sheets and blankets, or active, such as warm-air blankets and thermal mattresses. In this scenario, the present study aimed at comparing two warming methods used in the intraoperative period. Patients submitted to abdominal surgery in the specialties of gynecology and gastric surgery from August to September 2010 were included in the study. After randomization, they were divided into two groups: one using a thermal blanket (group I) and one using orthopedic cotton (group II). At last, 9 patients were included. The variables for each question were considered according to occurrence frequency. Comparison between groups was performed by Student’s t test. With the purpose to analyze whether there was an association, the chi-square test or Fisher’s Exact test was used. Whenever it was applicable for multiple comparisons, Tukey’s test was utilized; p values < 0.05 were considered to be statistically significant for analysis. The sample comprised 6 males and 3 females submitted to gynecological and gastric surgeries. Their mean age was 48 years for group I and 46.2 years for group II. A predominance of general anesthesia was observed. The time of permanence in the operating room ranged from 80 to 360 minutes. With regard to warming parenteral solutions, the procedure was performed on 5 patients, and infusion of warm solution into the abdominal cavity was performed on 50% of the sample. Concerning the warming method used, 5 patients used a thermal blanket. In view of the results presented, it was not possible to conclude which warming method should be used due to sample size

WHO has launched the “Safe Surgery Saves Lives” program with the purpose to reduce damage to surgical patients and define safety standards specifically in order to reduce such patients’ morbi-mortality by providing surgery teams and hospital managers with orientation about the standards of safe surgery and a uniform service evaluation instrument for national and international surveillance. Hence, this study aimed at applying the WHO safe-surgery checklist to the surgical specialties of a university hospital and evaluating the team’s opinion about the influence of its application on the safety of surgical process and on the team’s interpersonal communication. It was a descriptive, analytical, qualitative field study conducted in the surgery facilities of a university hospital in a public establishment in São Paulo state. The checklist was applied to eight surgical specialties, resulting in a total number of 30 surgeries. Its application was conducted by the researcher in three phases: Sign in, Time out, Sign Out. Next, one member of the surgery team was invited to voluntarily participate in the study by signing an informed consent form and answering guiding questions. Thirty members of the surgery team participated in the study. Bardin’s Content Analysis Method was used to organize and analyze the data. As regards the safety provided by the checklist, the following thematic categories emerged: “It reduces risk and possible complications”; “It standardizes conducts and reviews safety steps”; “It allows for better understanding of the process”; and “It provides safety to the team as a whole”. The category “It is not included in the institution’s routine” emerged from the subjects’ statements when they understood that, in this form, the checklist does not provide safety to surgical procedures. The genetic selection and the nutritional management to improve milk production make the dairy cattle more susceptible to the development of diseases, such as the abomasal displacement. It is the most frequently detected abomasal problem and it is the main cause of abdominal surgeries in dairy cattle. It is a multifactorial disease that occurs mainly in dairy cattle of high production during the puerperium. The abomasal displacement can occur to the right (DAD) or to the left (DAE), being the former more frequent than the latter. It is related to feeding management and occurs in animals that also have other diseases such as hypocalcaemia, ketosis and retained placenta. The disease causes economical losses in dairy cattle because of the costs with treatment, reduction of production, increase of the interval between the parturition, loss of body weight, early discard of the matrix and mortality. The most usual clinical signs are apathy, dehydration, low to serious ruminal timpanismo (gas accumulation in the abomasum) with reduction or lack of motility, liquid splash sound during the ballottement of the right flank, metallic sound to percussion, presence of a structure similar to distended viscera in the thorax or in the paralombar cavity on the side corresponding to the displacement, and liquefied, dark, scarce and fetid feces. The treatment is surgical, and the most used technique is the omentopexy on the left flank. The hidroelectrolytic correction must be performed and the concomitant diseases must be treated. The prophylaxis consists of adequate nutrition and pre-parturition management, besides reduction of stress and other diseases of the puerperium

This paper consists in the evaluation of the exposure rate to ionizing radiation to which professionals working in surgical procedures which require radiological examinations are subjected. Were initially performed real-time readings of exposure rate within four distinct operating rooms during the execution of four surgical procedures that made use of fluoroscopy equipment (including three orthopedic surgeries, one in the shoulder, one in the arm, another for deployment of metal pin in the leg region, and a fourth for vascular procedure); in these surgeries were used ionization chamber detector and an electrometer. In order to check the values achieved, was made a re-evaluation of the distribution of the rate of exposure to radiation, from the surgical procedures, now with thermoluminescent dosimeters (TLDs). For this, thirty TLDs were distributed in the operating rooms, arranged in points of interest as occupation by professionals. The TLDs were prepared for thirty consecutive days, after which they were removed and replaced with new dosimeters not exposed yet. The dosimeters were subjected to reading of the rate of exposure; this procedure was repeated for four months without interruption. The quantification of the results sought primarily to convert the rate of exposure for equivalent dose rate, both in measurements with ionization chamber as in measurements with TLDs, in order to highlight the presence of the biological effect of ionizing radiation for comparisons within scientific context. Then, the results were plotted to establish the relationship between the values of equivalent dose and the distance to the central axis of the x -ray source, confirming the inverse square law for distance. Finally, the values were associated with the maximum limit recommended by the legislation for occupationally exposed individuals.
