BackgroundLichen planus is a mucocutaneous disease with manifestation in the oral mucosa, the gingiva being one of the most affected regions. In some cases, the lesion may be painful and lead to fragility of the tissues, so that precise diagnosis and adequate treatment are indispensible factors for improving the clinical condition. The aim of this study was to evaluate the effectiveness of plaque control in the improvement of clinical features and painful symptoms of oral lichen planus with gingival involvement.MethodsTwenty patients diagnosed with gingival lichen planus confirmed by histopathological examination were selected. The patients were evaluated by a trained examiner, with regard to the clinical features of the lesions [Index of Escudier etal. (Br J Dermatol, 157, 2007, 765)]; painful symptoms (Visual Analog Scale); and periodontally, as regards the visible plaque and gingival bleeding indices. Periodontal treatment consisted of supragingival scaling and oral hygiene instruction, with professional plaque removal afterward for a period of 4weeks. The entire sample was evaluated at the baseline and at the conclusion of treatment, and the results were analyzed by the Wilcoxon nonparametric test.ResultsThe data demonstrated that the majority of patients were women (90%), with a mean age of 55.9years. Periodontal treatment resulted in statistically significant reduction (P<0.05) in the periodontal indices, with consequent improvement in the clinical features and painful symptoms of the lesions.ConclusionsIt was demonstrated that plaque control was effective in improving the clinical features and painful symptoms of oral lichen planus with gingival involvement.

The present study compared the expression of cytokeratins CK6, CK16 and CK19 and pan-cytokeratin (PAN) in oral mucosa cells between smokers and nonsmokers in order to determine the stage of cell differentiation and to consequently infer proliferative activity and expressions indicative of a potential for malignant differentiation. Thirty smokers and 30 non-smokers seen at the clinics of FOSJC-UNESP were screened. Smears were obtained from the left lateral border of the tongue with a cytobrush and slides were processed for immunohistochemistry using the antibodies reported Conventional microscopy was used for qualitative analysis. The results were analyzed statistically by the Z test, Fisher's exact test and comparison of two proportions (plus-4 confidence interval method). The expression of CK6 (p=0.002), CK16 (p=0.003), CK19 (p=0.0001) and PAN (p=0.008) was higher in oral mucosa smears from smokers compared to non- smokers. ln conclusion, increased epithelial proliferation is observed in the oral mucosa of smokers as demonstrated by the increased expression of CK6 and CKJ6, and these cells present alterations in epithelial maturation

A Síndrome da Imunodeficiência Adquirida (AIDS) é uma doença causada pelo vírus da imunodeficiência humana (HIV) e acomete milhões de indivíduos pelo mundo todo. Para seu tratamento, utilizam-se antirretrovirais em associação ou isoladamente, e um dos mais utilizados é a zidovudina, 3’-azido-3’-desoxitimidina (AZT). Porém, sua baixa biodisponibilidade causada pela elevada taxa de metabolização hepática e baixa capacidade de permeação através das membranas biológicas, resulta na necessidade de administração de elevadas doses do fármaco, várias vezes ao dia, o que acaba gerando níveis plasmáticos tóxicos e, consequentemente, efeitos colaterais graves e desconforto ao paciente. Sendo assim existe a necessidade de uma otimização das formulações orais contendo AZT. Os sistemas de liberação de fármacos com propriedades mucoadesivas podem ser uma alternativa para melhorar as propriedades biofarmacêuticas, pois permitem o contato íntimo do fármaco com a mucosa intestinal e por um tempo prolongado, o que pode melhorar a biodisponibilidade de determinados fármacos, como o AZT. As dispersões sólidas (DS) são um tipo de sistema de liberação de fármaco que visam melhorar a biodisponibilidade de fármacos com baixa biodisponibilidade. Para desenvolvimento destes sistemas, vários materiais poliméricos podem ser utilizados, dentre eles o ftalato de hidroxipropil metilcelulose (HPMCP), com propriedade gastrorresistentes e mucoadesiva e o glicolato sódico de amido (GSA), que é um polímero intumescível e mucoadesivo. No desenvolvimento do trabalho as dispersões foram preparadas a partir da técnica de evaporação do solvente, contendo AZT, GSA e HPMCP nas proporções 1:5:7,5, 1:5:10 e 1:5:15, respectivamente, sendo obtidas com êxito a partir da técnica escolhida. The Combination Syndrome represents an important pathologic condition of the masticatory system that requires a complex rehabilitation treatment. The presence of flaccid mucosa is one of the most common features due to bone resorption of the pre-maxilla caused by anterior hyperfunction of the mandibular teeth on the maxillary complete denture, where the impact of oclusal loading in this region results in movement of the maxillary prosthesis into the ridge, resulting in increased resorption of the residual ridge, internal misfit and loss of the retention of the maxillary complete denture. The presence of flaccid mucosa, in such cases, leads to problems of support and stability of the maxillary complete denture, which can be treated by surgical reduction of the thickness of the tissue or using specific techniques of fabrication of prostheses. This article describes the oral rehabilitation of a patient with the Combination Syndrome where the surgical reduction of flaccid mucosa was not performed, using a modified technique of functional impression in two steps, with the objective of generating forces and less distortion in flaccid mucosa during functional impression.

Oral verruciform xanthoma represents an uncommon entity, which affects mainly oral mucosa. This paper presents the major clinical and histological features of oral verruciform xanthoma and reports a case on the tongue. The differential diagnosis and a literature review are also provided in light of recent information.
