The usefulness of fluoride-releasing restorations in secondary caries prevention may be questioned because of the presence of other common sources of fluoride and because of ageing of the restorations. This study tested the hypothesis that glass-ionomer cement restorations, either aged or unaged, do not prevent secondary root caries, when fluoride dentifrice is frequently used. Sixteen volunteers wore palatal appliances in two phases of 14 days, according to a 2 x 2 crossover design. In each phase the appliance was loaded with bovine root dentine slabs restored with either glass-ionomer or resin composite, either aged or unaged. Specimens were exposed to cariogenic challenge 4 times/day and to fluoridated dentifrice 3 times/day. The fluoride content in the biofilm (FB) formed on slabs and the mineral loss (Delta Z) around the restorations were analysed. No differences were found between restorative materials regarding the FB and the Delta Z, for either aged (p = 0.792 and p = 0.645, respectively) or unaged (p = 1.00 and p = 0.278, respectively) groups. Under the cariogenic and fluoride dentifrice exposure conditions of this study, the glass-ionomer restoration, either aged or unaged, did not provide additional protection against secondary root caries. Introduction: the patients with specials necessities ne ed a singular dentistry treatment because they have limitations due to theirs deficiency. The quality of oral health is related to the clinical situation of patient. The individuals with motor and understanding problems have poor oral health. Aim: this st udy aimed to review the literature about the importance of the early dentistry treatment of pacients with specials necessities. Methodology: the review of literature was made through t he Pubmed and Bireme. Studies about dental caries and periodontal dise a se of patient s with specials necessities had shown high index of caries and periodontal disease. Therefore it is important that there are capacity p rofessionals and multidisciplinary treatment. It is necessary to supply the dificulties of treatment like an xiety of parents, systemic problems and discrimination. Conclusion: the early treatment results in good coperation during the treatment, and the aquisition of cares for all life. The best way is a program of oral health with instructions about diet, oral hygiene, control of bacterials, motivation and interaction between patients and professionals.

The purpose was to evaluate the effect of acid etching time on the bond strength of a simplified etch-and-rinse adhesive system to noncarious and caries-affected dentin of primary and permanent teeth. Methods: Twenty-four extracted primary and permanent teeth were divided into three groups, according to the acid etching time. Four teeth from each group were exposed to a microbiological caries-inducing protocol. After caries removal, noncarious and caries-affected dentin surfaces were etched with 37 percent phosphoric acid for five, 10, or 15 seconds prior to the application of Prime & Bond NT adhesive. Crowns were restored with resin composite and prepared for microtensile testing. Data were submitted to Kruskal-Wallis and Mann-Whitney tests (á=0.05). Results: Higher bond strengths were obtained for noncarious dentin vs. cariesaffected dentin for both primary and permanent teeth. Reducing the acid etching time from 15 to five seconds did not affect the bond strength to caries-affected or noncarious dentin in primary teeth. For permanent teeth, lower bond strength values were observed when the noncarious dentin was etched for five seconds, while no difference was seen between 10 and 15 seconds. Conclusions: For Prime & Bond NT, the etching of dentin for five seconds could be recommended for primary teeth, while 10 seconds would be the minimum time for permanent teeth.

The Health Sciences, prior to the planning of prevention programs, adopt the tools to analyze the population profile, entitled epidemiological surveys, to obtain data on the need for heathcare and the possibility of treating health events. The aim of this study was to evaluate dental conditions in schoolchildren, aged five to fourteen, enrolled in public schools in Américo Brasiliense, SP Brazil. The dmft and DMFT index, caries experience, and prevalence of fluorosis were analyzed. For this epidemiological study, 1,137 children were selected. Four calibrated dentists performed oral exams according to WHO criteria (1997). The data were collected using descriptive analyses. The results showed that 94% of the five-year-old children were caries free, and the value of dmft was 1.44. DMFT was 1.19 in 12-year-olds. The lowest and the highest fluorosis prevalence could be observed in eight and fourteen-year-olds, respectively. It could be concluded that the city of Américo Brasiliense has developed respectful educational and preventive oral health programs which presented satiosfactory results in the present epidemiological study.

The Family Health Strategy is a primary care public policy that is becoming a decisive step towards the transformation of the Brazilian healthcare model. This study evaluated the dental knowledge among individuals who attended a Family-Health Unit (FHU) in the city of São Carlos, SP, Brazil. Methods: The evaluation was based on a questionnaire with 20 questions about oral health, prevention and development of dental caries and periodontal disease. The questionnaire was given to 168 individuals, over 18 years of age and both sexes, to fill out under the supervision of a trained dentist. After data collection, the answers contained in the original questionnaires were entered into a database built using the program Excel® (Microsoft Corporation, USA) and a descriptive statistical analysis was done. Results: 66.67% of the subjects reported having received information about dental caries and periodontal disease before attending the FHU, and dentists were cited as the main source of such information (60.71%). With regard to dental caries, 70.24% of the individuals stated that they knew about the subject, but the majority of answers revealed incorrect explanation of it. The multifactorial etiology (bacteria/sugar/poor hygiene) was not mentioned. A low percentage of subjects (24.40%) claimed to know what periodontal disease is, and of these, the highest percentage of responses was related to gingivitis (26.83%) and gingival bleeding (12.20%). Out of all subjects, 80.36% affirmed that oral hygiene is important, and among them, the main reasons cited were the maintenance of oral health (29.63%), to avoid diseases (16.30%) and to preserve and prevent disease (14.81%). Conclusion: Responses from the questionnaires revealed deficient dental knowledge among the individuals. To improve the understanding about this issue, the information obtained through the questionnaire can be used to develop educational programs that will focus specifically on the major deficiencies found.

The aim of this study was to assess the knowledge level on oral health promotion for babies of all mothers (n=60), aged 18-42 years, assisted in the Preventive Dentistry Clinic of the Faculty of Araraquara, at Universidade Estadual Paulista (UNESP), during the period 2005-2008. Methods - An interview was performed using a questionnaire with 18 open and five closed questions about knowledge and practices related to caries prevention as well as demographic information. Methods - The majority believed that breast milk does not cause caries (73,3%), that caries is not a transmissible disease (51,6%) and that anatibiotics cause caries (63,3%). Almost all mothers (93,3%) responded that brushing teeth could prevent the disease, action that should be initiated since first tooth erupts (75%). For 73,3% of the respondents, the use of dental floss should be initiated onle after all deciduous teeth erupt. Results - The majority (83,3%) knew what was fluoride, but a third of them (33,3%) did not know the best time to start using it. The dentist counseling related to oral health of babies was considered imoprtant by 96,7% of mothers. Conslusion - The mothers presented good knowledge about baby's oral hygiene, however, they did not know about other important variables for oral health promotin of babies.

This study focused on the risk factors in mother-child relationship that predispose babies to the development of dental caries. A prospective cohort study with 80 mother-child pairs was conducted. The mothers responded at 12, 18 and 30 months after their children's birth, to questions about variables related to diet, sucking habits, and oral care. Children were clinically examined to verify caries lesions (white spot lesions or cavitation). Data were analysed using Chi squared or Fisher's exact tests. The significance level was set at 5 %. Of the total, 3.75 % showed cavitated lesions after 18 months; 6.25 and 45 % had spot white lesions, respectively, at 18 and 30 months. The cariogenic diet was high at 12 (63.75 %) and 30 (88.75 %) months. Good oral hygiene was present in a minority of children at 12 months (46.25 %), but increased at 30 months (65 %), helping to prevent cavities and white spot lesions over this period (p = 0.0005). The variables of the blocks sucking habits and diet were not associated with caries. The lack of oral care in children was a risk factor for dental caries development.
