Eventos adversos a medicamentos (EAM) ainda são uma das principais limitações nos serviços de cuidados com a saúde, causando risco aos pacientes, além de grande impacto econômico. Assim, fazem-se necessários métodos de identificação de eventos adversos mais eficazes que a tradicional notificação espontânea, a qual revela apenas 5% do total de eventos. A utilização de critérios rastreadores (ou trigger tools), elaborados pelo Institute for Healthcare Improvement, tem se mostrado como a alternativa mais válida e eficiente para a triagem dos EAM. Com o objetivo principal de identificar os principais eventos adversos e medicamentos envolvidos, uma seleção de três critérios rastreadores foi utilizada, a saber: INR (razão normalizada internacional) > 6, WBC (contagem de leucócitos) < 3000/mm³ ou registros de episódios de sedação excessiva, letargia ou queda. Informações sobre os rastreadores foram coletadas e analisadas a partir dos prontuários de pacientes internados no Hospital Estadual Américo Brasiliense internados, de novembro de 2011 a janeiro de 2012 e maio a julho de 2012, a fim de se confirmar a ocorrência de evento adverso. Foram analisados prontuários referentes a 3318 hospitalizações, nas quais foram detectados 69 eventos adversos, 49 dos quais foram causados por apenas quatro das 12 classes de medicamentos envolvidos em algum EAM: anticoagulantes orais, relacionados ao rastreador INR>6 (eficácia = 0,75), sedativos e antipsicóticos, relacionados ao rastreador de sedação excessiva, letargia ou queda (eficácia = 0,52), e antivirais, relacionados ao rastreador WBC<3000 (eficácia = 0,48). Quando os rastreadores eram acionados e não estavam relacionados a EAM, os fatores de confusão mais recorrentes foram: torpor secundário ao quadro clínico, acidentes não relacionados ao medicamento e pacientes propositalmente em regime de sedação. Hospitalization is usually steeped in fantasies that generate high levels of anxiety, especially for children. Therefore, the hospital psychotherapeutic process must propose actions to enable them to develop internal resources to cope with situation that causes anxiety. This article aims to demonstrate the importance of psychological intervention through drawings as mediators in the pre-surgical context of orthopedic correction in the hospital. This patient made seven drawings that at first expressed her anguish towards the surgery and at the end showed her attempt to elaborate. We conclude that drawing is an important therapeutic tool in the hospital.

Abstract A fuzzy linguistic model based on the Mamdani method with input variables, particulate matter, sulfur dioxide, temperature and wind obtained from CETESB with two membership functions each was built to predict the average hospitalization time due to cardiovascular diseases related to exposure to air pollutants in São José dos Campos in the State of São Paulo in 2009. The output variable is the average length of hospitalization obtained from DATASUS with six membership functions. The average time given by the model was compared to actual data using lags of 0 to 4 days. This model was built using the Matlab v. 7.5 fuzzy toolbox. Its accuracy was assessed with the ROC curve. Hospitalizations with a mean time of 7.9 days (SD = 4.9) were recorded in 1119 cases. The data provided revealed a significant correlation with the actual data according to the lags of 0 to 4 days. The pollutant that showed the greatest accuracy was sulfur dioxide. This model can be used as the basis of a specialized system to assist the city health authority in assessing the risk of hospitalizations due to air pollutants.

Early diagnosis of hearing loss minimizes its impact on child development. We studied factors that influence the effectiveness of screening programs. To investigate the relationship between gender, weight at birth, gestational age, risk factors for hearing loss, venue for newborn hearing screening and pass and fail results in the retest. Prospective cohort study was carried out in a tertiary referral hospital. The screening was performed in 565 newborns through transient evoked otoacoustic emissions in three admission units before hospital discharge and retest in the outpatient clinic. Gender, weight at birth, gestational age, presence of risk indicators for hearing loss and venue for newborn hearing screening were considered. Full-term infants comprised 86% of the cases, preterm 14%, and risk factors for hearing loss were identified in 11%. Considering the 165 newborns retested, only the venue for screening, Intermediate Care Unit, was related to fail result in the retest. Gender, weight at birth, gestational age and presence of risk factors for hearing loss were not related to pass and/or fail results in the retest. The screening performed in intermediate care units increases the chance of continued fail result in the Transient Otoacoustic Evoked Emissions test.

Hospital Dentistry can be understood by the care of the oral abnormalities that require intervention of multidisciplinary team for highly complex treatment. Dentistry integrated into the hospital, allows a better performance in the commitment to patient care and also expanded oral health care to the population. However, even among dentists, opinions differ on the subject. The participation of the dentist in an outpatient or in hospital, aims to collaborate, deliver and add more strength to what characterizes the new identity of the hospital. The aim of this study was to review the literature about the importance of ministering concepts of Hospital Dentistry for the exercise of this profession and how the situation is nowadays.

The aim of this research was to investigate the knowledge, judgment, monitoring and execution of the dentists regarding Hospital Dental Care. Five hundred dentists from Araçatuba (São Paulo state) answered a questionnaire. The results showed that 49% of professionals have never had experience at hospitals and 64% affirmed that this content is poorly approached in graduation. Also 46% of respondents have no interest or don’t think necessary acting in a hospital and 24% believe that this care is done by specialists only. It can be suggested that there is a lack of knowledge about Hospital Dental Care, thus the dentist shall be instructed since academic training for insertion into the hospital team.

Contexto: O manuscrito descreve o ataque não provocado de um jacaré-do-Pantanal a um pescador. Ataques por jacarés são pouco relatados e a espécie do Pantanal raramente causa acidentes. Descrição do caso: A vítima foi mordida na mão direita quando limpava peixes nas margens do rio Paraguai, por um jacaré de cerca de 1,5 m que saiu debaixo da vegetação aquática. O paciente apresentou dor, secreção purulenta e febre e procurou auxílio médico, apresentando ferimentos perfurocontusos e perfurocortantes e fratura-arrancamento no segundo metacarpiano direito. Após cinco dias de internação, houve significativa regressão do processo inflamatório, quando obteve alta hospitalar para posterior acompanhamento ambulatorial. Discussão: Os acidentes por jacarés podem ser muito graves, devido à dentição e à potência das mandíbulas desses répteis. Embora algumas espécies amazônicas possam predar seres humanos, o jacaré-do-Pantanal não causa mortes, mas mesmo espécimes de pequeno tamanho podem causar lesões graves e infecção secundária importante, como observado no caso. Conclusões: Devido ao potencial traumático e capacidade de inoculação de micro-organismos, as mordidas de jacarés devem ser encaradas como ferimentos de alto risco pelas equipes de saúde.

The Patient Classification System has become essential concerning to the practice in management and care from a nurse. This study analyzed the implementation of Assistance Sites in an intensive care unit for adults as a way of organization and classification of patients, as well as the impact of this process on the quality of care according to the Nursing Activities Score and the relation with the Hospital Infection. This is a quantitative, prospective, descriptive and transversal study. The data collection was realized from July until October 2010. The sample was consisted of 214 patients, mostly male, neurosurgical and with a mid age of 57 years. The NAS was on the average of 71.72%. Regarding the Hospital Infection before and after implantation, there was a reduction in the rates of pneumonia. However, the nursing workload remained the same. Moreover, It was evident the importance of using the Nursing Activities Score and the implementation of new ways for classification of patients to improve the organization of the care.

Objective. To describe behaviors related to hand cleaning in health professionals who work in the pediatric inpatient unit of a university hospital. Methodology. Descriptive study, in which the studied population was the sanitary staff who worked in a pediatric inpatient unit of a university hospital, there were a total of 43 people. Information was collected through an observation process and a survey applied on hand washing techniques, at the beginning of the study and a month later of the first one. Results. In just 7% of the observations, participants washed their hands before developing the procedure; one out of two followed the steps described in the technique. The most frequent failure in hand washing was presented in the step of rubbing the right hand palm over the left had dorsum, crossing the fingers and vice versa (18%). Conclusion. Participants of the study didn’t have the habit to wash their hands according to the recommended technique. Data suggest the necessity to promote educational actions to change staff behaviors and attitudes towards the steps and techniques of hand washing before and after performing any procedure.

Objective. To estimate the prevalence of bacteria isolated in samples from the hands of school-age children at a hospitalization unit. Methodology. In 2009, strains were cultured from the hands of 90 school-age children at the pediatric hospitalization unit of Hospital Estadual Bauru (São Paulo, Brazil). After culture of the samples, the isolated bacteria were identified. Results. In 98% of the samples taken from the children, bacteria were isolated. Coagulase-negative Staphilococcus was isolated in 64% of the samples, followed by Staphilococcus aureus (5%) and Pseudomonas aeruginosa (1%). Conclusion. In most of the samples from the children’s hands, bacteria were isolated. Therefore, educative actions about hygiene habits in- and outside the hospital environment should be reinforced, aimed at children and their companions.
