OBJECTIVE: to review the evolution of acute respiratory distress syndrome (ARDS) definitions and present the current definition for the syndrome. DATA SOURCE: a literature review and selection of the most relevant articles on ARDS definitions was performed using the MEDLINE®/PubMed® Resource Guide database (last ten years), in addition to including the most important articles (classic articles) that described the disease evolution. DATA SYNTHESIS: the review included the following subjects: introduction; importance of definition; description of the first diagnostic criterion and subsequently used definitions, such as acute lung injury score; definition by the American-European Consensus Conference, and its limitations; description of the definition by Delphi, and its problems; accuracy of the aforementioned definitions; description of most recent definition (the Berlin definition), and its limitations; and practical importance of the new definition. CONCLUSIONS: ARDS is a serious disease that remains an ongoing diagnostic and therapeutic challenge. The evolution of definitions used to describe the disease shows that studies are needed to validate the current definition, especially in pediatrics, where the data are very scarce.

Realizou-se adaptação transcultural da Escala de Satisfação com o Suporte Social (ESSS) para a língua portuguesa. As qualidades psicométricas foram avaliadas numa amostra de 1.023 estudantes do ensino superior do Brasil e de Portugal. A partir dos resultados obtidos propõe-se uma versão modificada da ESSS com 12 itens que avaliam 4 dimensões. A versão modificada revelou adequada confiabilidade, validade fatorial, validade concorrente, divergente e discriminante com exceção dessa última para Satisfação com as Amizades e a Intimidade. A validade convergente esteve no limite do aceitável. Observou-se invariância dos pesos fatoriais entre Brasil e Portugal, permitindo sua utilização para a avaliação da Satisfação com o Suporte Social em estudantes do ensino superior de ambos os países.

INTRODUÇÃO:Testes incrementais de corrida permitem a determinação de limiares metabólicos e neuromusculares. O objetivo do presente estudo foi comparar índices eletromiográficos e metabólicos entre dois protocolos incrementais de corrida com diferentes intervalos entre cada estágio de velocidade.MÉTODOS:Participaram do estudo 14 voluntários do sexo masculino. Os protocolos incrementais de corrida em esteira iniciaram em 8 km.h-1, com incremento de 1 km.h-1 a cada três minutos até a exaustão voluntária. Os dois protocolos diferiram quanto aos intervalos entre cada estágio de velocidade: 30 segundos (protocolo 1) e 120 segundos (protocolo 2). O limiar de fadiga eletromiográfico (LFEMG) foi determinado para os músculos reto femoral, bíceps femoral, tibial anterior e gastrocnêmio lateral. Para tanto, o comportamento do valor RMS foi correlacionado em função do tempo de corrida, sendo realizada regressão linear para determinação dos coeficientes de inclinação. O limiar de lactato foi identificado por meio do ponto de inflexão na curva lactato-intensidade e o limiar anaeróbio foi determinado por meio de interpolação linear. Foi aplicado um teste t de Student para dados pareados (p<0,05).RESULTADOS:Foi verificado que o protocolo 2 apresentou velocidade de LFEMG maior do que o protocolo 1, apenas para o músculo BF (p=0,023), o que caracteriza uma resposta específica deste músculo em protocolos incrementais de corrida.CONCLUSÃO:Protocolos de corrida com intervalos de até dois minutos entre os estágios incrementais apresentaram resultados semelhantes para determinação do LFEMG da maioria dos músculos estudados e dos limiares metabólicos.

Diatraea saccharalis Fabr. (Lepidoptera: Crambidae) is a major sugarcane pest in Brazil. The management of infested areas is based on the release of Cotesia flavipes (Cameron) (Hymenoptera: Braconidae), a parasitoid of D. saccharalis larvae, but there are doubts about the effectiveness of C. flavipes, primarily regarding its rate of dispersal in sugarcane fields. Thus, the objective of this study was to evaluate the dispersal of C. flavipes in a sugarcane field and suggest a release method that provides higher parasitoid efficiency. The study was carried out in four areas of approximately 1 ha, in which stalk pieces containing 20 D. saccharalis larvae were distributed in a rectangular grid, and 12,000 C. flavipes adults were released at four points, that were 50 m apart and 25 m from the field border. Three days later, the D. saccharalis larvae were recovered and kept in the laboratory until they reached pupal stage or C. flavipes emergence. Parasitism varied from 13.2% to 42.8%. The random distribution of parasitized larvae was found in one assay. In three areas, the parasitized larvae showed an aggregated distribution, with a range of 15 to 25 m. Since the parasite's success is directly linked to parasitoid dispersion, it would be interesting to move the release points to 30 m from each other because the dispersal may happen in a 15 m radius.

Objective: The purpose of this research was to evaluate the histological changes of the periodontal ligament and alveolar bone during dental movement in diabetic rats subjected to low level laser therapy (LLLT).Methods: The movement of the upper molar was performed in 60 male Wistar rats divided into four groups (n = 15): CTR (control), DBT (diabetic), CTR/LT (irradiated control) and DBT/LT (irradiated diabetic). Diabetes was induced with alloxan (150 mg/kg, i.p.). LLLT was applied with GaAlAs laser at 780 nm (35 J/cm(2)). After 7, 13 and 19 days, the periodontal ligament and alveolar bone were histologically analyzed.Results: The mean of osteoblasts (p < 0.01) and blood vessels (p < 0.05) were significantly decreased in DBT compared with CTR at 7 days, whereas the mean of osteoclasts was lower at 7 (p < 0.001) and 13 days (p < 0.05). In DBT/LT, only the mean of osteoclasts was lower than in CTR (p < 0.05) at 7 days, but no difference was observed at 13 and 19 days (p > 0.05). The collagenization of the periodontal ligament was impaired in DBT, whereas DBT/LLT showed density/disposition of the collagen fibers similar to those observed in CTR.Conclusions: LLLT improved the periodontal ligament and alveolar bone remodeling activity in diabetic rats during dental movement. We have compared the results of the external irrigation technique with those of a double irrigation technique with continuous intermittent movement. Maximum thermal measurements were made in the cortical part of 10 samples of bovine ribs during osteotomy to simulate the preparation of a surgical bed for the installation of dental implants at a depth of 10 mm Twenty specimens were drilled for each group: external irrigation and continuous movement (control group 1, CG1); external irrigation and intermittent movement (control group 2, CG2); double irrigation and continuous movement (test group 1, TG1); and double irrigation and intermittent movement (test group 2, TG2). The double irrigation technique gave significantly better results regardless of the drilling movement used. Thermal increases between samples was 19.2% in group CG1, 10.4% in CG2, 5.4% in TG1, and 3.4% in TG2. The double irrigation technique produced a significantly smaller increase in temperature in the cortical bone during both types of drilling (p = 0.001), which illustrated its greater efficiency compared with that of the external irrigation technique. Dawson and Murray-Clay (Dawson and Murray-Clay [2012]. Astrophys. J., 750, 43) pointed out that the inner part of the cold population in the Kuiper belt (that with semi major axis a < 43.5 AU) has orbital eccentricities significantly smaller than the limit imposed by stability constraints. Here, we confirm their result by looking at the orbital distribution and stability properties in proper element space. We show that the observed distribution could have been produced by the slow sweeping of the 4/7 mean motion resonance with Neptune that accompanied the end of Neptune's migration process. The orbital distribution of the hot Kuiper belt is not significantly affected in this process, for the reasons discussed in the main text. Therefore, the peculiar eccentricity distribution of the inner cold population cannot be unequivocally interpreted as evidence that the cold population formed in situ and was only moderately excited in eccentricity; it can simply be the signature of Neptune's radial motion, starting from a moderately eccentric orbit. We discuss how this agrees with a scenario of giant planet evolution following a dynamical instability and, possibly, with the radial transport of the cold population. (C) 2014 Elsevier Inc. Background: There are no reported cases of factitious or simulated obsessive compulsive disorder (OCD). However, over the last years, our clinic has come across a number of individuals that seem to exaggerate, mislabel or even intentionally produce obsessive and/or compulsive symptoms in order to be diagnosed with OCD.Methods: In this study, experienced clinicians working on a university-based OCD clinic were requested to provide clinical vignettes of patients who, despite having a formal diagnosis of OCD, were felt to display non-genuine forms of this condition.Results: Ten non-consecutive patients with a self-proclaimed diagnosis of OCD were identified and described. Although patients were diagnosed with OCD according to various structured interviews, they exhibited diverse combinations of the following features: (i) overly technical and/or doctrinaire description of their symptoms, (ii) mounting irritability, as the interviewer attempts to unveil the underlying nature of these descriptions; (iii) marked shifts in symptom patterns and disease course; (iv) an affirmative yes pattern of response to interview questions; (v) multiple Axis I psychiatric disorders; (vi) cluster B features; (vii) an erratic pattern of treatment response; and (viii) excessive or contradictory drug-related side effects.Conclusions: In sum, reliance on overly structured assessments conducted by insufficiently trained or naive personnel may result in invalid OCD diagnoses, particularly those that leave no room for clinical judgment.
