The Urbanization is a notable process in our society, being part of it since around 3500 BC in Mesopotamia, currently Iraq area. Since the Industrial Revolution, cities had a significant increase in their growth and began to concentrate increasingly labor and capital. Brazil, despite having had a late industrialization, also had a high population growth in cities, especially in the XVIII century, which were not provided with any kind of planning that targets the ordering and implementation of the urban essential apparatus. Therefore, Brazilian cities started to exhibit countless structural and environmental problems. Associated with inadequate infrastructure, the modifications in the urban atmosphere, like increased heat, air pollution and increased rainfall and storm frequency, cause different types of impacts in cities. Among the most worrisome are those from the intense rain, which cause human and material damage. The city of Rio Claro (SP) can be considered an example of this reality. Thereby, the objective of this study was to analyze the impacts on the population originated from rainfall in the urban area during the period 2005-2010, by the mapping of events and analysis of the active atmospheric systems. The information of the events were collected in the Fire Department and the Press and the synoptic maps were obtained in the library of INPE, in Cachoeira Paulista (SP) and in the site of the Navy of Brazil. The data showed that among 247 occurrences, during the six years, 233 occurred in the spring-summer period. Moreover, after finalizing the maps, it was found the locations considered critical regarding the frequency of occurrences, as: Visconde do Rio Claro Avenue and central area of the city, both with a total of 23 occurrences; Jardim Inocoop, with 20; Tancredo Neves Avenue, with 12 and Vila Paulista, with 10 occurrences. Introduction: Preterm Labor (PTL) and Preterm Premature Rupture of Membranes (PPROM) cause severe complications for both mother and fetus. Among the risk factors associated with preterm labor and PPROM, genetic predisposition has been gaining importance. However, the association between polymorphic genes and the pathogenesis of PTL and PPROM remains elusive. A better understanding of the genetic mechanisms underlying these adverse pregnancy outcomes may enable the identification of high risk patients and allow new approaches to minimize the deleterious effects of prematurity. Aim: To determine the association between maternal IL-6 polymorphism gene and the occurrence of PTL and PPROM. Patients and Methods: The study included 109 patients with prior history of PL and/or PPROM that delivered prematurely at the Obstetrical Unit Care of Botucatu Medical School, UNESP between 2003 and 2012. The control group consisted of 68 patients that delivered at term, matched to the case group by age, ethnicity, and sex of the newborn. Oral swabs (Cath-AllTM – Epicentre Biotechnologies) were collected for analysis of genetic polymorphisms by PCR. Statistical tests were performed to compare genotype, clinical and socio-demographic data from the groups. A p-value of <0.05 was considered significant. Results: The sociodemographic characteristics in both groups were homogeneously distributed. The frequency of the polymorphic allele C, associated with less production of IL-6, and therefore thought to be protective against PTL and PPROM, was 32,5% in the study group and 30,9% in the control group, without statistically significant differences. Conclusion: Considering the sample size included in this study, the frequency of the mutated allele is similar in pregnant women who delivered at term and gestational complications as PTL and PPROM

Devido à grande importância médico-veterinária dos carrapatos e a ampla utilização de produtos químicos sintéticos para o seu controle, tais como a permetrina, o presente estudo trouxe uma análise morfofisiológica, citoquímica e da reorganização do citoesqueleto durante os processos de degeneração e de morte celular que ocorreram nas glândulas salivares de fêmeas semi-ingurgitadas de carrapatos Rhipicephalus sanguineus quando expostas a 206, 1031 e 2062 ppm de permetrina. Os resultados obtidos mostraram que a permetrina é um potente agente químico que interfere na morfofisiologia do tecido glandular desta espécie, causando severas alterações na forma dos ácinos, bem como intensa vacuolização citoplasmática nas células acinares e acentuada desorganização do tecido glandular, culminando num avançado processo de degeneração, com consequente formação de corpos apoptóticos. Além disso, este piretróide comprometeu a capacidade de secreção das glândulas salivares, visto a drástica redução de proteínas, lipídeos e polissacarídeos nas diferentes células acinares. O material genético do tecido glandular também mostrou severas alterações induzidas pela permetrina, tais como: na forma e no tamanho do núcleo das células acinares, marginalização da cromatina, fragmentação nuclear, bem como surgimento de núcleos picnóticos, principalmente quando aplicadas as maiores concentrações do produto. Com relação à reorganização do citoesqueleto, os resultados revelaram que a permetrina foi capaz de modificar o citoesqueleto das glândulas salivares de R. sanguineus por meio de um processo gradual de desintegração à medida que as concentrações do acaricida aumentaram. Nestas células glandulares, os processos de morte celular ocorreram num ritmo mais acelerado do que em condições naturais, nas quais esse processo dar-se-ia somente no final do ingurgitamento da fêmea. O maior rio do Estado de São Paulo, o Rio Tietê, teve, na Região Metropolitana de São Paulo, suas áreas de várzeas tomadas pelo processo de urbanização e metropolização da população. Parte de suas terras inundáveis passou por processos de aterramento e impermeabilização para que a instalação de indústrias e infraestruturas fosse possível. A APA da Várzea do Rio Tietê foi criada para controlar o processo de ocupação do entorno do rio e na tentativa de amortecer as enchentes causadas por tempestades, cada vez mais constantes. Essas chuvas torrenciais são causadas pela influência das Ilhas de Calor da RMSP com o encontro de brisas marítimas, e causam inundações e destruições

The prophylactic effect of ondansetron on subarachnoid morphine-induced pruritus is controversial, while evidence suggests that droperidol prevents pruritus. The aim of this study is to compare the effects of droperidol and ondansetron on subarachnoid morphine-induced pruritus. 180 ASA I or II patients scheduled to undergo cesarean sections under subarachnoid anesthesia combined with morphine 0.2mg were randomized to receive, after the child's birth, metoclopramide 10mg (Group I - control), droperidol 2.5mg (Group II) or ondansetron 8mg (Group III). Postoperatively, the patients were assessed for pruritus (absent, mild, moderate or severe) or other side effects by blinded investigators. Patients were also blinded to their group allocation. The tendency to present more severe forms of pruritus was compared between groups. NNT was also determined. Patients assigned to receive droperidol [Proportional odds ratio: 0.45 (95% confidence interval 0.23-0.88)] reported less pruritus than those who received metoclopramide. Ondansetron effect was similar to metoclopramide [Proportional odds ratio: 0.95 (95% confidence interval 0.49-1.83)]. The NNT for droperidol and ondansetron was 4.0 and 14.7, respectively. Ondansetron does not inhibit subarachnoid morphine-induced pruritus.

Are called panfacial fractures when the upper, middle and lower facial thirds present fractures concurrently. In clinical practice, came to imply the involvement of two facial thirds. Panfacial fractures are usually accompanied by other systemic lesions that impair the patient's life and therefore require primary treatment. Almost invariably are associated with damage to soft tissues and severe losses of bone structures which may lead to severe facial deformations and malocclusions. The panfacial fractures treatment is complex because often there isn’t a stable bone structure to guide the reduction of various fractures. Several orders of treatment have been proposed, but they are variations of the two classical approaches "bottom to top and inside-out" and "top to bottom and out-inside". The aim of this paper is to discuss the principles of management and panfacial fractures treatment, emphasizing the sequence of fracture reduction and highlighting its indications, advantages and disadvantages, through literature review and reports of surgical clinic cases. We conclude that the exact sequence of fracture reduction is not as important as developing a treatment plan that allows accurate positioning of the fractured segments.
