FUNDAMENTOS: A leishmaniose tegumentar americana permanece doença endêmica em diversas regiões brasileiras. A sobrevivência do parasita no interior dos macrófagos se deve, em parte, pela atividade de uma K+/H+-ATPase de membrana que pode ser inibida pelo omeprazol. OBJETIVOS: Avaliar a eficácia do omeprazol na prevenção do desenvolvimento de lesões de leishmaniose em hamsters. MÉTODOS: Empregaram-se 18 hamsters, divididos em três grupos: o grupo L recebeu apenas a inoculação de L. brasiliensis na pata anterior direita, o grupo O recebeu apenas doses diárias de 0,4mg de omeprazol subcutâneo, e o grupo L+O recebeu o inóculo de leishmanias e o tratamento com omeprazol desde o dia da inoculação. O estudo foi conduzido por 42 dias, realizaram-se medidas dos diâmetros das patas semanalmente, e, ao final do estudo, foram realizados esfregaços das lesões para verificação dos parasitas. RESULTADOS: Os hamsters dos grupos L e L+O desenvolveram lesões de leishmaniose tegumentar havendo ulceração em duas patas do grupo L e uma do grupo L+O. Ao final do estudo, a mobilidade e vitalidade no grupo L foram menores que em L+O, e estas menores que no grupo O. Os diâmetros das patas inoculadas nos grupos L e L+O foram significativamente maiores que no início do estudo (p<0.05). Não houve diferença significativa entre os diâmetros das patas dos grupos L e L+O ao final do estudo (p0,05), sendo detectados parasitas no esfregaço das lesões dos dois grupos. CONCLUSÕES: Omeprazol, no protocolo utilizado, não evitou o desenvolvimento de lesões de leishmaniose tegumentar em hamsters.

FUNDAMENTOS: O queratoacantoma é neoplasia cutânea benigna que incide preferencialmente em indivíduos de pele clara, faixa etária elevada, acometendo áreas fotoexpostas. Além da exposição à radiação ultravioleta, sua etiologia é relacionada a diversos carcinógenos, entre eles a infecção pelo papilomavírus humano (HPV). OBJETIVOS: Avaliar a prevalência do DNA do HPV, bem como seus genótipos, em lesões de queratoacantoma solitário de pacientes imunocompetentes. MÉTODOS: Foram estudados queratoacantomas de pacientes sem evidências de imunocomprometimento, excisados entre 1996 e 2000 em hospital universitário. Realizaram-se cortes histológicos, desparafinização e extração de DNA desses fragmentos. Os espécimes positivos para DNA de HPV foram submetidos ao seqüenciamento gênico, para determinação do genótipo. RESULTADOS: Foram estudados 58 pacientes com idade média de 64,5±13,8 anos. A proporção entre os sexos foi semelhante, e as localizações mais comuns foram os membros superiores (50%) e a face (27,6%). Detectou-se DNA de HPV em 48 (82,7%) fragmentos de queratoacantomas, sendo os genótipos 6, 11 e 16 os prevalentes. CONCLUSÕES: A alta prevalência do achado de DNA de HPV em lesões de queratoacantoma solitário pode sugerir a participação viral em sua oncogênese.

FUNDAMENTOS - Melasma é hipermelanose comum caracterizada por máculas acastanhadas em áreas fotoexpostas, cuja fisiopatogenia não é totalmente esclarecida. OBJETIVOS - Caracterizar e comparar morfologica e funcionalmente os melanócitos da epiderme sã com os da pele afetada por melasma. MÉTODOS - Avaliaram-se 12 pacientes portadores de melasma facial, sendo realizadas biópsias da pele lesada e pele sã adjacente. Os cortes foram corados por hematoxilina-eosina, Fontana-Masson, marcados pelo Melan-A e submetidos à microscopia eletrônica. A quantificação epidérmica de melanina e melanócitos foi estimada a partir de análise citomorfométrica digital. RESULTADOS - Todas as pacientes eram mulheres com média de idade 41,3±2,8 anos. Ao Fontana-Masson evidenciou-se importante aumento da melanina epidérmica na pele lesada em relação à pele sã. A marcação pelo Melan-A demonstrou melanócitos maiores com dendritos proeminentes na pele lesada. Observou-se maior densidade de melanina epidérmica na pele lesada, e a análise digital do número de melanócitos da epiderme não demonstrou diferença significativa entre pele lesada e sã. À microscopia eletrônica, observaram-se número aumentado de melanossomas maduros nos ceratinócitos e melanócitos com organelas citoplasmáticas proeminentes na pele lesada. CONCLUSÕES - Melanogênese aumentada na epiderme com melasma em relação à epiderme normal adjacente.

Avaliou-se, por meio de exames radiográficos e densitométricos, a influência de um biomaterial desenvolvido pela indústria brasileira, no processo de reparação de fraturas diafisárias instáveis do rádio. Foram utilizados 15 coelhos Norfolk, machos, idade entre cinco e seis meses e peso corpóreo médio de 3,5kg. Induziu-se fratura transversa na porção média da diáfise de ambos os rádios direito, por meio de serra giratória. Ao redor das extremidades fraturadas do rádio direito (tratado) aplicou-se BMPs adsorvidas à hidroxiapatita e aglutinante de colágeno em grânulos, ambos de origem bovina. O rádio esquerdo não recebeu tratamento, sendo considerado controle. Realizaram-se estudos radiográficos e de densitometria óptica por imagens radiográficas no pós-operatório imediato e aos 30, 60 e 90 dias de pós-operatório. Somente no 30º dia de pós-operatório ocorreu maior porcentagem de restabelecimento cortical na fratura tratada com biomaterial. Pela análise estatística não se verificou diferença nos exames densitométricos quer entre momentos ou entre membros. Foi possível concluir que apenas no exame radiográfico ao 30 dias de pós-operatório detectou-se efeito positivo do biomaterial.

This study was designed to evaluate retrospectively the frequency and etiology of the gastrointestinal (GI) lesions in 45 consecutive necropsies of adult patients with Acquired Immunodeficiency Syndrome (AIDS). Gross descriptions and histological sections of the GI tract, from mouth to anus, were reviewed. The slides were H&E stained, and when necessary special stains and immunohistochemical methods were also employed. There were lesions in GI tract in 37 (82.3%) patients; the mouth was the segment most frequently involved (73.3% of the cases), followed by the colon (55.5%). Multiple lesions occurred in 17 (37.7%) cases. Cytomegalovirus caused colonic lesions in 35.7% of the cases. Candidiasis was observed in 26.6% mainly in the mouth and herpes simplex (8.8%) was the important agent of esophageal lesions. Oral hairy leukoplasia associated with HPV was found in 16 (35.5%) cases. Neoplasia was diagnosed in 7 (15.5%) cases: four Kaposi's sarcoma, two anal intramucosal carcinomas and one gastric lymphoma. Our data confirm the high frequency and variety of GI tract alterations in AIDS.

Cara inchada of cattle (CI) is a periodontitis which occurs in calves in several regions of Brazil. Its progressive lesions, caused mainly by Bacteroides melaninogenicus and Actinomyces pyogenes, could lead to loss of teeth, malnutrition and possibly death of affected animals. For further analysis of the pathogenesis of this disease the influence of streptomycin on the adherence of the causative bacteria to oral epithelial cells of cattle was determined. Production of streptomycin by soil bacteria (Actinomycetes) in Brazil had been suggested by several authors. In our studies, streptomycin in subinibitory concentrations increased tenfold the rate of adherence with B. melaninogenicus (from 0.4 to 4.0 bacteria per bovine epithelial cell). On the contrary, streptomycin did not influence significantly the adherence of A. pyogenes nor streptococci of the viridans group. Thus, streptomycin could be a contributing factor to the development of the periodontal infections with B. melaninogenicus.

Observations on the epizootiology of cara inchada in cattle (CI) indicated that animals affected by the disease recover spontaneously when they are transferred from the CI-prone area to CI-free pastures. In the presente study 13 young bovines with actively progressing periodontal CI-lesions were transferred to a CI-free area, in order to investigate the clinical course of the disease and the composition of the microflora in the periodontal lesions in two distinct situations: (1) in the actively progressing lesions, and (2) in the healed periodontal pouches of clinically recovered animals. The semi-quantitative and qualitative bacteriological study was undertaken in regard to the percentage of black-pigmented Bacteroides in cultures. In the actively progressing lesions of the 13 CI-diseased bovines the percentage of these bacteria was 71.3% in average. The evaluation of the composition of the microflora in the healed periodontal pouches of the clinically recovered bovines showed that bacteria of the black-pigmented Bacteroides group existed only at an average of 1.7%. The results of this investigation give further support for the infectious nature of the CI-periodontitis and the primary involvement of these bacteria.

The intestinal ischemia is unusual in vascular surgery emergency. Its main causes are embolisms and arterial thrombosis. In addition to severe ischemia, reperfusion of the ischemic tissues can lead to several complications that may worsen the ischemic lesion and produce a life threatening situation caused by systemic alterations. Intestinal tissue injuries due to ischemia and reperfusion have been demonstrated in clinical and experimental studies, in which pathophysiology and adequate treatment were also studied. The great variety of experimental models used and results achieved reflect the need for an intestinal ischemia and reperfusion experimental model that is simple, reproducible and consistent, in order to search for treatments that can reduce the damage caused by this situation. Purpose: To evaluate the conjunctival epithelial malign lesions carriers and the recurrence rate using exeresis and radiotherapy. Methods: A retrospective study was conducted observing 52 conjunctival epithelial malign lesions carriers treated from 1989 by 2003. The subjects were assessed to evaluation according: age, gender, time of start, classification of the lesion and recurrence rate. The lesion were surgically removed and radiotherapy was indicated when the exeresis was incomplete. Results: The majority of the patients were male, with 61 year old median age, white. The conjunctival squamous carcinoma was presented in 86,5% of the patients. The recidive rate was 11,5%, happened between 1 to 60 month postoperative. Conclusion: according our results the conjunctival epithelial malign lesions were more often observed in older, males and whites. The majority of the patient had conjunctival squamous carcinoma (86,5%). The recidive rate after exeresis and radiotherapy in patients with incomplete exéresis was 11,5%.

To stimulate the bone callus development process from distal third of radius, 24 adult mongrel dogs used were from both sexes. These dogs were separated in two experimental groups of 12 animals each, named control and treated, divided in 4 moments (M1=15 days; M2=30 days; M3=45 days; M4=60 days), who underwent were performed surgical fractures. In treated group, it was performed bone perforations on proximal and distal edges, craniolateral and mediolateral to the fracture site. At the end of each moment, control and treated animals were evaluated by radiography, histology, and bone mineral densitometry (BMD) was determined on fracture site. According to the radiographic data of treated dogs, it was verified on days 15 and 30 more intense bone regeneration than control group. During M3 and M4, it wasn't detected any difference in bone reparation process betweencontrol and treated groups. In densitometric study, BMD values were greater in treated animals than in control dogs. Histological studies revealed at 15 and 30 days chondrocyte hyperplasia and initial endochondral ossification on drilled limbs; control group showed sustainment connective tissue and initial chondrocyte hiperplasia. At M3 and M4 of the treated group, were verified development and remodeling of periosteal callus in more advanced phases when comparing with limbs from control group. It can be concluded that using perforations enhances blood flow supply and activation of osteogenic cells on fracture site, stimulating the beginning of fracture consolidation process.

It was evaluated the radiographs alteration of the castor oil bean polymer applied for the filing of induced bone fault in horses. Five animals were evaluated, subjected to a local anesthesia and sedation, proceeding to a surgical induction of a hole in the radio bone, in both members with three centimeters depth perforations. It was used a drill of eight millimeters of diameter. The right member served as a control, without filling induced holes. The holes in left radio were filled with the commercial polymer derived from the castor oil bean. We evaluated the localized swelling and radiological aspects as: the establishment of scores for bone filling, sclerosis and a morphometric analysis of the sclerosis area, in 15, 45, 90 and 180 days of lesion evolution. There was more swelling in local members whose injuries were filled with polymer from castor oil bean. By comparison, the radiographic examination showed better bone filling in the gaps of members control. We concluded that the completion of castor oil bean polymer in bone holes induced in radio of horses, delayed the bone healing, in the radiographic evaluation.

In the majority of cases of bone fracture requiring surgery, orthopedic implants (screw-plate and screw) are used for osteosynthesis and the infections associated with such implants are due to the growth of microorganisms in biofilms. The objective of this study was to identify microorganisms recovered from osteosynthesis implants used to fix bone fractures, to assess the viability of the cells and the ability of staphylococci to adhere to a substrate and to determine their sensitivity/resistance to antimicrobials. After surgical removal, the metal parts of austenitic stainless steel (ASTM F138/F139 or ISO NBR 5832-1/9) were transported to the Laboratory of Clinical Microbiology, washed in buffer and subjected to ultrasonic bath at 40±2 kHz for 5 minutes. The sonicated fluid was used to seed solid culture media and cell viability was assessed under the microscope by with the aid of a fluorescent marker. The production of extracellular polysaccharide by Staphylococcus spp. was investigated by means of adhesion to a polystyrene plate. The profile of susceptibility to antimicrobials was determined by the disk diffusion assay. The most frequently isolated bacteria included coagulase-negative Staphylococcus resistant to erythromycin, clindamycin and oxacillin. Less frequent were Pseudomonas aeruginosa resistant to trimethoprim/sulfamethoxazole and ampicillin, Acinetobacter baumannii resistant to ceftazidime, Enterobacter cloacae resistant to cephalothin, cefoxitin, cefazolin, levofloxacin and ciprofloxacin, Bacillus spp. and Candida tropicalis. The observation of slides by fluorescence microscope showed clusters of living cells embedded in a transparent matrix. The test for adherence of coagulase-negative Staphylococcus to a polystyrene plate showed that these microorganisms produce extracellular polysaccharide. In conclusion, the metal parts were colonized by bacteria related to orthopedic implant infection, which were resistant to multiple antibiotics.

Visceral leishmaniasis in dogs is described as a chronic disease whose main symptoms are progressive weigth loss, cachexy and dermatologic lesions. Recently, the disease has been associated to neurologic disorders. A total of 40 dogs with visceral leishmaniasis were divided into two groups. The first composed of dogs without neurological signs (n=30) and the second by dogs with neurological disorders (n=10). Brain samples were collected, stored in 10% buffered formalin and subjected to immunohistochemical examination for amastigotes forms of Leishmania (Leishmania) infantum chagasi, CD3+, CD4+ and CD8+ T lymphocytes and macrophages. Imunnohistochemistry evaluation revealed no amastigote forms of the parasite. CD3+ T lymphocytes were present in 24/30 (80%) dogs without neurological signs and in all dogs from the second group (p=0.0011). CD4+ and CD8+ were rarely observed, with CD4+ immunostaining in 10/40 (25%) dogs, from which half of them had neurological disease (p=0.0090). The presence of CD8+ was detected only in 4/10 (40%) dogs from neurological group (p=0.0021). Macrophages were detected in 38/40 (95%) dogs, without significant differences between groups (p=0.7664).
