PURPOSE: To report a case of idiopathic macular hole, with vitreoretinal traction confirmed by optical coherence tomography that was successfully treated by a single intravitreous perfluoropropane (C3F8) gas bubble injection.METHODS: Case report. A 65-year-old patient with idiopathic macular hole (stage 2, one eye) received an intravitreous gas injection and was prospectively followed with optical coherence tomography.RESULTS: A complete posterior vitreous detachment was achieved within 6 weeks after gas injection. Visual acuity improved from 20/80 to 20/25 by 10 months of followup. Optical coherence tomography disclosed vitreoretinal traction release and macular hole closure. No complications were related to the procedure.CONCLUSION: This simple procedure can assist a complete posterior vitreous detachment with relief of the hyaloid-foveolar traction, facilitating macular hole closure. Euonymus japonica Thunb. (euonymus) plants were submitted for 9 months to two irrigation treatments using water from different sources: a control (C) water with electrical conductivity (EC) less than 1.2 dS.m(-1) and reclaimed wastewater (RW) with EC approximate to 4 dS.m(-1). At the end of the experiment, no differences in the total dry weight were observed between treatments, whereas the leaf dry mass increased (to the detriment of the root part in RW plants). Throughout the day, the stem water potential (Psi(stem)) of the RW plants was lower than in C, whereas stomatal conductance (g(S)) was slightly reduced in RW from 0800 HR to 1200 HR, but no significant variation in photosynthesis (P-n) or energy conversion efficiency (F'(v)/F'(m)) in photosystem II was detected through the effect of salinity. Gas exchange and fluorescence showed a tendency to increase after midday in plants treated with RW. The photosynthetic behavior and fluorescence of RW plants may have been related to the nitrogen and chlorophyll content of the leaves, confirming the resistance of the photosynthetic mechanism to salinity in this species in these conditions. The toxic effects produced by high concentrations of boron (B), sodium (Na+) and chloride (Cl-) were offset by the effect of other ions like magnesium (Mg2+), potassium (K+), and phosphorus (P) in plants irrigated with RW, thus improving their physiological status without decreasing their ornamental value.

The search for materials with higher properties and characteristics (wear resistance, oxidation, corrosion, etc.) has driven research of various materials. Among the materials that are being studied with such properties and characteristics are super alloys based on nickel which has an important role in the aeronautical, automotive, marine, production of gas turbines and now in space vehicles, rocket engineering , experimental aircraft, nuclear reactors, steam-powered plants, petrochemical and many other applications because besides having all the characteristics and properties mentioned above also have an excellent performance at high temperatures. The super alloy based on nickel studied in this work is the super alloy Pyromet 31v normally used in the manufacture of exhaust valves in common engines and diesel engines of high power by cater requirements such as mechanical strength and corrosion resistance at temperatures of approximately 815 ° C. The objective of this work is to produce results to demonstrate more specific information about the real influence of coatings on cutting tools and cutting fluids in turning and thus promote the optimization of the machining of these alloys. The super alloy Pyromet 31v was processed through turning, being performed with various machining parameters such as cutting speed, feed rate, depth in conditions of Minimum Amount of Fluid (MAF), abundant fluid, cutting tools with coating and without coating in early in his work life and with wear. After turning were obtained several samples of chips and the part generated during the machining process, was measured roughness of the material, subsequently made macrostructural analysis of the tools used order to detect possible wear and microstructural analysis of samples collected being that the latter was used for Optical Microscopy, Scanning Electron Microscopy (SEM) and ... Nowadays cancer is one of the main causes of death and many efforts worldwide have been driven to find out new treatments and approaches in order to extinguish or reduce this group of disorder. Chemotherapy is the main treatment for cancer, however, conventional schedule based on maximum tolerated dose (MTD) show several side effects and frequently allow the development of drug resistance. In this review we present the evidence that metronomic chemotherapy, based on the frequent administration of low or intermediate doses of chemotherapeutics is as efficient as MTD and works better in some situations. Finally, we present some data indicating that noncytotoxic concentrations of antineoplastic agents are able to both up-regulate the immune system and increase the susceptibility of tumor cells to cytotoxic T lymphocytes. Taken together, data from the literature provide us the evidence that low concentrations of selected chemotherapeutics agents, rather than conventional high doses, should be chosen for combination with immunotherapy

Objectives: The effectiveness of noninvasive positive-pressure ventilation in preventing reintubation due to respiratory failure in children remains uncertain. A pilot study was designed to evaluate the frequency of extubation failure, develop a randomization approach, and analyze the feasibility of a powered randomized trial to compare noninvasive positive-pressure ventilation and standard oxygen therapy post extubation for preventing reintubation within 48 hours in children with respiratory failure.Design: Prospective pilot study.Setting: PICU at a university-affiliated hospital.Patients: Children aged between 28 days and 3 years undergoing invasive mechanical ventilation for greater than or equal to 48 hours with respiratory failure after programmed extubation.Interventions: Patients were prospectively enrolled and randomly assigned into noninvasive positive-pressure ventilation group and inhaled oxygen group after programmed extubation from May 2012 to May 2013.Measurements and Main Results: Length of stay in PICU and hospital, oxygenation index, blood gas before and after tracheal extubation, failure and reason for tracheal extubation, complications, mechanical ventilation variables before tracheal extubation, arterial blood gas, and respiratory and heart rates before and 1 hour after tracheal extubation were analyzed. One hundred eight patients were included (noninvasive positive-pressure ventilation group, n = 55 and inhaled oxygen group, n = 53), with 66 exclusions. Groups did not significantly differ for gender, age, disease severity, Pediatric Risk of Mortality at admission, tracheal intubation, and mechanical ventilation indications. There was no statistically significant difference in reintubation rate (noninvasive positive-pressure ventilation group, 9.1%; inhaled oxygen group, 11.3%; p > 0.05) and length of stay (days) in PICU (noninvasive positive-pressure ventilation group, 3 [116]; inhaled oxygen group, 2 [1-25]; p > 0.05) or hospital (noninvasive positive-pressure ventilation group, 19 [7-141]; inhaled oxygen group, 17 [8-80]).Conclusions: The study indicates that a larger randomized trial comparing noninvasive positive-pressure ventilation and standard oxygen therapy in children with respiratory failure is feasible, providing a basis for a future trial in this setting. No differences were seen between groups. The number of excluded patients was high.
