The development of an accurate diagnosis and appropriate treatment plan can be a complex task, especially in cases of dentoalveolar trauma. The authors present a case report of crown-root fracture caused by trauma and highlight the importance of a multidisciplinary approach for the treatment. An eighteen year-old boy had a bicycle accident resulting in dental trauma. The upper right first molar showed a complicated crownroot fracture and the lower left second pre-molar showed an uncomplicated crown-root fracture. Endodontic treatment, controlled tooth extrusion, periodontal surgery for recovery of biological width, and porcelain crown and onlay restorations were performed. Esthetic and functional results were achieved. At the two-year follow-up it was observed that the tooth/onlay interface of the upper right first molar was stained and the onlay of the left lower second pre-molar was fractured. Therefore, the interface stained was repaired and a porcelain crown was made for the lower second premolar. The clinical case presented herein leads to the conclusion that a multidisciplinary treatment plan is extremely important for a proper resolution in cases of dentoalveolar trauma.

Objective: to evaluated the effect of treating root-adhered necrotic periodontal ligament (PDL) with citric acid on the healing process in delayed rat tooth replantation. Material and Methods: Forty Wistar rats, assigned to 4 groups (n=10), had their upper right incisor extracted and kept dry on a workbench. For Group I (control), the teeth were replanted after a 5 min extra-alveolar time. For the other groups, replantation was done after 60 min. In Group II, the root canals were filled with a calcium hydroxide-based paste and the teeth were replanted with no root surface treatment. In Group III, the teeth were immersed in citric acid (pH 1) for 3 min, the canals were filled with calcium hydroxide and the teeth were replanted. In Group IV, instead of the immersion, the roots were scrubbed with gauze soaked in citric acid and the teeth were replanted. The animals were sacrificed 60 days postoperatively. Results: Regarding replacement resorption, there was statistically significant difference (p < 0.05) between the control group and the other three groups. No statistically significant difference (p > 0.05) was found among the groups regarding the areas of inflammatory resorption. There was also a statistically significant difference (p < 0.05) between the control group and Group IV regarding ankylosis. The control group showed the least replacement resorption percent means compared to the other groups (p < 0.05). The root structure was more affected by replacement resorption and ankylosis in Group IV compared to the Groups II and III, but this difference was not significant statistically (p > 0.05). Conclusion: the treatment of root surface-adhered necrotic periodontal ligament with citric acid was not able to prevent the occurrence of ankylosis, root resorption and inflammatory resorption in delayed rat tooth replantation.

Dental avulsion is the most severe type of traumatic tooth injuries because it causes damageto several structures and results in the complete displacement of the tooth from its socketin the alveolar bone. The ideal situation is to replant an exarticulated tooth immediatelyafter avulsion because the extraoral time is a determinant factor for treatment successand for a good prognosis. However, it is not always possible. The success of replantationdepends on a number of factors that may contribute to accelerate or minimize theoccurrence of root resorption or ankylosis, among which is the type and characteristicsof the medium used for temporary storage during the time elapsed between avulsionand replantation. Maintaining the tooth in an adequate wet medium that can preserve,as longer as possible, the vitality of the periodontal ligament cells that remain on rootsurface is the key to success of replantation. Recent research has led to the developmentof storage media that produce conditions that closely resemble the original socketenvironment, with adequate osmolality (cell pressure), pH, nutritional metabolites andglucose, and thus create the best possible conditions for storage. Although these storagemedia can now be purchased in the form of retail products, the most common scenariois that such a product will not be readily available at the moment of the accident Thispaper reviews the literature on the different storage media that have been investigatedfor avulsed teeth based on full-length papers retrieved from PubMed/Medline, Lilacs, BBOand SciELO electronic databases using the key words storage medium , transportationmedium , avulsion , tooth avulsion , replantation , tooth replantation , milk and propolis .After application of inclusion and exclusion criteria, 39 papers were selected and criticallyreviewed with respect to the characteristics, efficacy and ease of access of the storagemedium. The choice of surgical approach to diseases that affect the pre-auricular region has been subject of much discussion in the literature. Access pre-hearing, have been used with high compliance rate of success, and during the history of surgery, several modifications were made in this approach with the goal of reducing the irreversible consequences especially regarding common in condylar fractures as paralysis and facial scars. The views range from the indication of surgical treatment for all fractures, until the conviction that no fracture of the bone segment should be surgically treated. Therefore, this study is of relevance, since offers surgeons warn about the care on the anatomical structures involved in these surgical approaches and describe, seeking a comparison between them, the advantages of them by means of a literature review covering from the extensive bouts of Bellinger (1940) and Al-Kayat (1979) that provide a broad view of the surgical field to access modified and increasingly smaller as the endaural (2001) which not only allows a satisfactory field of aesthetics as maintenance the patient without any signs of scarring.

Introduction: The treatment of mandibular fractures has undergone many changes, especially after the introduction of rigid internal fixation techniques. Currently, the use of resorbable materials presents some advantages over metallic devices, particularly when applied in children. Objective: The authors discuss the controversies involved in the treatment of mandibular fractures in children, mainly related to fixation of fractures. Case Report: In this paper, we present a case of a 5 years treatment patient with mandibular angle fracture treated by open reduction and internal fixation with plates and screws. Final Comments: Aiming to not interfere in the development of tooth germs and mandibular bone growth a second surgery for removal of the fixation devices is indicated.

Alveolar wall fenestrations are common during implant placement. The aim of this paper is to report a case where a peri-implant bone fenestration was reconstructed immediately after implant placement by an autogenous mandibular bone block. A male patient was referred to the Department of Surgical and Integrated Clinics to substitute his Kennedy´s Class IV removable partial denture for an implantsupported fixed prosthesis. A peri-implant bone fenestration at the buccal wall was seen at the region of 12, being reconstructed by a mandibular bone block secured by a bicortical screw. Six months later the surgical procedures, an implant-supported complete fixed partial prosthesis was developed. The autogenous bone block harvested from the mandibular ramus was a safe alternative to reconstruct the peri-implant bone defect such as fenestration types.

Background: The principles of tissue regeneration to repair alveolar bone defects are based on the fabrication of a biologic barrier with different biomaterials. Therefore, the present study aimed to investigate the guided bone regeneration (GBR) by using membrane of demineralized bovine bone matrix (DBBM) on experimental defects in tibia of dogs. Methods: Four dogs were used and after anesthesia, shaving and antisepsis, two standardized bone defects were created on the right tibia of each animal with trephine drill. In the Control Group, the defects were filled with blood coagulum, while in the Treated Group, a membrane of DBBM was used to cover the defects. After 90 days, animals were sacrificed. Results: In the Treated Group, 67.4% of new bone formation was observed and, in the Control Group, 32.6% of the bone tissue reabsorbed when compared with initial bone volume. The membrane remained intact and no inflammatory tissue was observed on membrane/ bone interface. Conclusion: It was concluded that the use of DBBM is an osteoconductive material, presents biocompatibility and may be promise option to repair bone defects.

The florid cemento-osseous dysplasia is an asymptomatic lesion present in the fibro-osseous maxilla and mandible of uncertain etiology. It has higher expression in females, and patients melanoderm, middle-aged to elderly. This dysplasia is an asymptomatic condition that can be discovered when a radiograph is performed. A biopsy is contraindicated to avoid infection difficult to treat. We report the case of a white woman 52 years old, who searched the Clinic of Surgery and Traumatology Bucco-maxillofacial surgery, Faculty of Dentistry of Araçatuba with pain in the posterior portion of left mandible. After radiographic examination was diagnosed with florid cementoosseous dysplasia. Treatment was instituted clinical and radiographic.

The design of implant structure is still a questioning to the professionals, while their variations can to influence of stress distribution favorably to bone. Therefore, the aim of study was to review addressing the topic of osseointegrated implants design establishing answers for the patient oral rehabilitation, according variations existing on the market. It was conducted a detailed search strategy by the PubMed/ Medline and Bireme, it was used as descriptors: “dental implants cylinder”; “dental implants thread”; “dental implants geometry”, until May 2014. From 798 articles, after review were selected 18 articles and 1 specific area book. The results were divided to compare design of implant and threads for further discussion and conclusions. Thus, conclude that the threads implants are best used because of its advantages with respect to the stress distribution and stability primary. The variations of the differents types of threads, have influence for stress distribution.

The purpose of this literature review was to evaluate the use of fibrin tissue adhesive in implant dentistry. Materials and Methods: A literature search of Medline-PubMed for articles published, describing the use of fibrin tissue adhesive in implant dentistry was performed and articles were critically reviewed. Results: The literature review reveals clinical trials and experimental studies with regard to the use of fibrin tissue adhesive in implant dentistry. This material consists of 2 components: highly purified, freeze-dried human fibrinogen with factor XIII and a starter solution containing human thrombin. Clotting factor XIII is admitted for reinforcement of the fibrin network. The components are reconstituted before use and when mixed form a clot by mimicking the terminal phase of the physiological clotting cascade. Several studies showed that fibrin tissue adhesive is fully absorbed by macrophages within 2 weeks of application. Adhesive fibrin tissue may be used for to prevent bone loss, to create contour in the periimplant soft tissue and osseous tissue, to sculpt emergence profile for prosthetic components and to mimic tissue architecture. In the last years fibrin tissue adhesive also find use as material for the controlled delivery of drugs and biologics. Conclusions: The fibrin tissue adhesive presents good properties such as biocompatibility, hemostatic properties and ability to break down like the physiologic clot. This material, alone or associated with other materials, can be used with the implants immediately after extraction. In this condition it brings the necessary anchoring and efficient maintenance of osseous/mucosal contour, which it is important for the clinical success.

The short dental implants represent an option of previsible treatment, its main indication lives in the possibility of avoiding techniques surgical invasive. The literature in the display that the geometry of the implants is of vital importance, combined with wide diameters and surface treatment, mainly when the bone quality is unfavorable. The mechanical resistance can be enlarged, increasing the number of implants and being used the spliting among them. A rigorous protocol prosthodontist should be following with intention of to avoid oblique loads and to control habits parafuncionais. We aimed at to discuss, through a wide bibliographical revision the short dental implants, their characteristics and indications, giving emphasis to the aspects biomechanic of the same ones. Material and Method: The bases given: Pubmed, ISI and Dentistry Oral Science from 1990 to 2009, were consulted to identify researches related to the length of the dental implants, with emphasis to the considered short and the biomechanics related to the same ones. They were located 568 articles, being selected 69 that gathered the inclusion criteria. Result: Among these 69 selected, 26 articles agreed the evidence level. Conclusion: Inside of the limits of appraised data, it can be concluded that the short implants represent a good treatment alternative, especially for cases with reduced bone bed. However a rigorous indication protocol and use should be followed for warranty of the success of the treatment.
