Introduction. Physical activity can provide long-term benefits for systemic lupus erythematosus (SLE). Objective. This study sought to demonstrate the effects of progressive resistance training on the muscular strength, bone mineral density (BMD) and body composition of pre-menopausal women with SLE undergoing glucocorticoid (GC) treatment. Materials and Methods. This is the case report of a 43-year-old African-South American premenopausal woman with non-extensive SLE and low bone density. A six-month program with three bimonthly cycles of 70%, 80%, and 90% intensity according to the 10 maximum-repetition test was used. Dual-energy X-ray absorptiometry (DXA) was used to measure the BMD, T-scores and body composition, and indirect fluorescence was used to measure the levels of antinuclear antibodies. Student's t-test was used. Results. Statistical improvement was noted in all strength exercises, including the 45° leg press (Δ%=+50%, p<0.001) and knee extension (Δ%=+15%, p=0.003) to maintain the BMD of the L2-L4 lumbar (Δ%=+0.031%; p=0.46) as well as the trochanter (Δ%=+0.037%; p=0.31) and BMI (Δ%=-0.8, p=0.54). Conclusion. In this case study, the presented methodology had a positive effect on strength and contributed to the maintenance of BMD and body composition in a woman with SLE undergoing GC treatment. © 2012 Revista Andaluza de Medicina del Deporte.

Purpose. To verify the effects of resistance training at the electromyographic fatigue threshold (EMGFT) based on one-repetition maximum strength (1RM), heart rate (HR), rate of perceived exertion (PE) and endurance time (EndT). Methods. Nineteen subjects (training group [TG]: n = 10; control group [CG]: n = 9), performed 1-min bicep curl exercises sets at 25%, 30%, 35% and 40% 1RM. Electromyography (biceps brachii and brachiorradialis), HR and PE were registered. Biceps brachii EMGFT was used to create a load index for an eight-week resistance training programme (three sets until exhaustion/session, two sessions/week) for the TG. The CG only attended one session in the first week and another session in the last week of the eight-week training period for EndT measurement. EndT was determined from the number of repetitions of each of the three sets performed in the first and last training sessions. After training, 1RM, EMGFT, EndT, HR and PE at the different bicep curl load intensities were again measured for both groups. Results. Increases in 1RM (5.9%, p < 0.05) and EndT (> 60%, p < 0.001) after training were found. In addition, PE was reduced at all load intensities (p < 0.05), while no changes were found for HR and EMGFT after training. Conclusions. Strength-endurance training based on the EMGFT improved muscular endurance and also, to a lesser extent, muscular strength. Moreover, the reduced levels of physical exertion after training at the same intensity suggest that endurance training exercises may improve comfort while performing strength exercises.

Background. The chronic obstructive pulmonary disease (COPD) is associated with the strength and resistance decreasing in addition to the dysfunction on autonomic nervous system (ANS). The aerobic training isolated or in association with the resistance training showed evidence of beneficial effects on an autonomic modulation of COPD; however, there are no studies addressing the effect of isolated resistance training.Aims. This study aims at investigating the influence of resistance training on an autonomic modulation through heart rate variability (HRV), functional capacity and muscle strength in individuals with COPD.Design. Clinical series study.Setting. Outpatients.Population. The study involved 13 individuals with COPD.Methods. The experimental protocol was composed by an initial and final evaluation that consisted in autonomic evaluations (HRV), cardiopulmonary functional capacity evaluation (6-minute walk test) and strength evaluation (dynamometry) in addition by the resistance training performed by 24 sessions lasted 60 minutes each one and on a frequency of three times a week. The intensity was determined initially with 60% of one maximum repetition and was progressively increased in each five sessions until 80%.Results. The HRV temporal and spectral indexes analysis demonstrates improvement of autonomic modulation, with significant statistical increases to sympathetic and parasympathetic components of ANS representing by SDNN, LF and HF. In addition, it was observed significant statistical increases to shoulder abduction and. knee flexion strength and functional capacity.Conclusion. The exclusive resistance training performed was able to positively influence the autonomic modulation; in addition it promoted benefits on cardiorespiratory functional capacity and strength benefits in individuals with COPD.Clinical Rehabilitation Impact. This study could contribute to clinical and professionals researchers that act with COPD, even though the resistance component of pulmonary rehabilitation presents consensual benefits on several healthy indicators parameters. There is no evidence about the effects on HRV before. Moreover, this study showed, on clinical practice, the HRV uses as an ANS activity on sinus node evaluation and highlights further importance on scientific context.

The aim of this study was to verify the effects of aerobic and combined training on the body composition and lipid profile of obese postmenopausal women and to analyze which of these models is more effective after equalizing the training load. Sixty five postmenopausal women (age=61.0±6.3 years) were divided into three groups: Aerobic Training (AT,n= 15), Combined Training (CT,[strength+aerobic],n=32) and control group (CG,n=18). Their body composition: upper body fat (TF), fat mass (FM), percentage of fat mass and fat free mass (FFM) were estimated by DXA. The lipid profile, total cholesterol, HDL-cholesterol and LDL-cholesterol were assessed. There was a statistically significant difference in the TF (AT= -4.4 %, CT= -4.4%, and CG= 1.0%, p= 0.001) and FFM (AT= 1.7%, CT= 2.6%, and CG= -1.4%, p= 0.0001) between the experimental and the control groups. Regarding the percentage of body fat, there was a statistically significant difference only between the CT and CG groups (AT= -2.8%, CT= -3.9% and CG= 0.31%, p= 0.004). When training loads were equalized, the aerobic and combined training decreased core fat and increased fat-free mass, but only the combined training potentiated a reduction in percentage of body fat in obese postmenopausal women after the training program. HDL-c levels increased in the combined group and the chol/HDL ratio (atherogenic index) decreased in the aerobic group, however, there were no significant differences between the intervention programs. Taken together, both the exercise training programs were effective for improving body composition and inducing an anti-atherogenic status.

Study objectives: This study was developed to investigate the influence of thoracic and upperlimb muscle function on 6-min walk distance (6MWD) in patients with COPD.Design: A prospective, cross-sectional study.Setting: the pulmonary rehabilitation center of a university hospital.Patients: Thirty-eight patients with mild to very severe COPD were evaluated.Measurements and results: Pulmonary function and baseline dyspnea index (BDI) were assessed, handgrip strength, maximal inspiratory pressure (Pimax), and 6MWD were measured, and the one-repetition maximum (1RM) was determined for each of four exercises (bench press, lat pull down, leg extension, and leg press) performed on gymnasium equipment. Quality of life was assessed using the St. George Respiratory Questionnaire (SGRQ). We found statistically significant positive correlations between 6MWD and body weight (r = 0.32; p < 0.05), BDI (r = 0.50; p < 0.01), FEV, (r = 0.33; p < 0.05), PImax (r = 0.53; p < 0.01), and all values of 1RM. A statistically significant negative correlation was observed between 6MWD and dyspnea at the end of the 6-min walk test (r = -0.29; p < 0.05), as well as between 6MWD and the SGRQ activity domain (r = -0.45; p < 0.01) and impact domain (r = -0.34; p < 0.05) and total score (r = -0.40; p < 0.01). Multiple regression analysis selected body weight, BDI, Pimax, and lat pull down IRM as predictive factors for 6MWD (R-2 = 0.589).Conclusions: the results of this study showed the importance of the skeletal musculature of the thorax and upper limbs in submaximal exercise tolerance and could open new perspectives for training programs designed to improve functional activity in COPD patients.

Background: Pulmonary rehabilitation (PR) programs are beneficial to patients with chronic obstructive pulmonary disease (COPD), and lower-extremity training is considered a fundamental component of PR. Nevertheless, the isolated effects of each PR component are not well established. Objective: We aimed to evaluate the effects of a cycle ergometry exercise protocol as the only intervention in a group of COPD patients, and to compare these results with a control group. Methods: 25 moderate-to-severe COPD patients were evaluated regarding pulmonary function, respiratory muscle strength, exercise capacity, quality of life and body composition. Patients were allocated to one of two groups: (a) the trained group (TG; n=13; 6 men) was submitted to a protocol of 24 exercise sessions on a cycle ergometer, with training intensity initially set at a heart rate (HR) close to 80% of maximal HR achieved in a maximal test, and load increase based on dyspnea scores, and (b) the control group (CG; n=12; 6 men) with no intervention during the protocol period. Results: TG showed within-group significant improvements in endurance cycling time, 6-min walking distance test, maximal inspiratory pressure and in the domain 'dyspnea' related to quality of life. Despite the within-group changes, no between-group significant differences were observed. Conclusion: In COPD patients, the results of isolated low-to-moderate intensity cycle ergometer training are not comparable to effects of multimodality and high-intensity training programs. Objective: To associate changes of body composition, muscle strength (MS) and plasma hormones (PH) in resistance-training protocol in sedentary postmenopausal women (PMW).Design: This randomized controlled trial, Brazilian 43 PMW (45-70-year-old) able for physical exercises were selected after they have accomplished medical and ethical criteria. They were assigned in two groups: RT, resistance training (n = 22); and CT, not trained control (n = 2 1); with supervision sessions of two to three exercise for large and one exercise for smaller groups in three series of 8-12 rep. (60-80% 1RM) for each exercise. The training period lasted 16 weeks and was preceded by low-load exercise (40-50% 1RM) adaptation period of 4 weeks (3/(times week)). Body weight, height, body mass index (BMI), and composition (BIA) along with fast-PH (FSH, LH, estrachol, cortisol, IGF-1 and testosterone) were assessed before (MO) and after (M 16) the 4 weeks period with the MS (1RM) determined also at 8 weeks (W). The values were correlated by Person's test and the means compared by Student's t-test and ANOVA.Results: At baseline both groups were similar in age, time of PMW, body composition, MS and fast-PH. However after 16 weeks, RT presented higher BMI (2. 1 %), IGF- 1 (37.8%) and MM gain (1.8 +/- 0.8 kg) than CT. MM correlated positively with IGF-1 (r = 0.45, p < 0.05) and MS progressively increased in all exercise greater in pectoral than legs and upper arms. Conclusion: Former sedentary postmenopausal women submitted to resistance training gained MM and MS irrespectively of fat mass changes but significantly associated with IGF-1 increase.
