Background: Excessive gingival display (EGD) has a negative impact on a pleasant smile. Minimally invasive therapeutic modalities have become the standard treatment in many dentistry fields. Therefore, the aim of this study is to compare the clinical outcomes of open-flap (OF) and minimally invasive flapless (FL) esthetic crown lengthening (ECL) for the treatment of EGD.Methods: A split-mouth randomized controlled trial was conducted in 28 patients presenting with EGD. Contralateral quadrants received ECL using OF or FL techniques. Clinical parameters were evaluated at baseline and 3, 6, and 12 months post-surgery. The local levels of receptor activator of nuclear factor-kappa B ligand (RANKL) and osteoprotegerin (OPG) were assessed by enzyme-linked immunosorbent assay at baseline and 3 months. Patients' perceptions regarding morbidity and esthetic appearance were also evaluated. Periodontal tissue dimensions were obtained by computed tomography at baseline and correlated with the changes in the gingival margin (GM).Results: Patients reported low morbidity and high satisfaction with esthetic appearance for both procedures (P > 0.05). RANKL and OPG concentrations were increased in the OF group at 3 months (P < 0.05). Probing depths were reduced for both groups at all time points, compared with baseline (P < 0.05). There were no differences between groups for GM reduction at any time point (P > 0.05).Conclusions: FL and OF surgeries produced stable and similar clinical results up to 12 months. FL ECL may be a predictable alternative approach for the treatment of EGD.

Foram analisados 100 dentes primeiros molares e 100 dentes segundos molares superiores, direitos e esquerdos, permanentes, extraídos de humanos, com o objetivo de explorar a região anatômica da furca, mais especificamente realizar medidas das distâncias das aberturas das furcas vestibular, mesial, e distal até à junção esmalte-cemento, e das distâncias das referidas aberturas até às faces radiculares pertinentes: vestibular, mesial, distal e lingual. Utilizou-se para mensuração um microcomputador 486 DX-66Mhz, no qual estava instalado um software analisador de imagens Mocha-Jandel Scientific SigmaScan Pro, version 2.0. Em relação às distâncias das aberturas das furcas até à junção esmalte-cemento foi observado que as distâncias médias para a furca vestibular foram de 3,50mm e 3,03mm; para a furca mesial de 4,44mm e 4,69mm; para a furca distal de 4,26mm e 3,73mm, respectivamente para os primeiros e segundos molares superiores. Em relação às distâncias das aberturas das furcas até às faces radiculares pertinentes foi observado que as distâncias médias da furca vestibular até à face mesial da raiz mesio-vestibular foram de 3,78mm e 3,72mm; da furca vestibular até a face distal da raiz disto-vestibular 3,62 e 3,64mm; da furca mesial até à face vestibular da raiz mesio-vestibular 7,33mm e 6,89mm; da furca mesial até a face lingual da raiz lingual de 4,13mm e 4,33mm; da furca distal até à face vestibular da raiz disto-vestibular de 5,61mm e 5,09mm; da furca distal até a face lingual da raiz lingual de 4,73mm e 4,65mm, respectivamente para os primeiros e segundos molares superiores.

Individuals with facial paralysis of 6 months or more without evidence of clinical or electromyographic improvement have been successfully reanimated utilizing an orthodromic temporalis transfer in conjunction with end-to-side cross-face nerve grafts. The temporalis muscle insertion is released from the coronoid process of the mandible and sutured to a fascia lata graft that is secured distally to the commissure and paralyzed hemilip. The orthodromic transfer of the temporalis muscle overcomes the concave temporal deformity and zygomatic fullness produced by the turning down of the central third of the muscle (Gillies procedure) while yielding stronger muscle contraction and a more symmetric smile. The muscle flap is combined with cross-face sural nerve grafts utilizing end-to-side neurorrhaphies to import myelinated motor fibers to the paralyzed muscles of facial expression in the midface and perioral region. Cross-face nerve grafting provides the potential for true spontaneous facial motion. We feel that the synergy created by the combination of techniques can perhaps produce a more symmetrical and synchronized smile than either procedure in isolation.Nineteen patients underwent an orthodromic temporalis muscle flap in conjunction with cross-face (buccal-buccal with end-to-side neurorrhaphy) nerve grafts. To evaluate the symmetry of the smile, we measured the length of the two hemilips (normal and affected) using the CorelDRAW X3 software. Measurements were obtained in the pre- and postoperative period and compared for symmetry.There was significant improvement in smile symmetry in 89.5 % of patients.Orthodromic temporalis muscle transfer in conjunction with cross face nerve grafts creates a synergistic effect frequently producing an aesthetic, symmetric smile.This journal requires that authors assign a level of evidence to each article. This work demonstrates the technique of perineural anesthetic blocks made in the distal forelimb of the horse, and also covers aspects relating to the interpretation of the results obtained with the procedure. The limit of the subject the distal forelimb of horses is explained by the distribution of common lameness, being more frequent in this region. The procedure is intended to provide analgesia to a region of the limb, causing the animal to stop limping or decrease the intensity of claudication temporarily, since this region is responsible for the claudication, allowing thus, the likely location of the lesion, which may be investigated by other helper methods to confirm the diagnosis. Besides the description of the technique, were approached concerning aspects to the location of the anatomical structures involved, materials needed, cares with the animals, and possible complications from the procedure
