O emprego da trombectomia na angioplastia primária demonstra bons resultados clínicos e angiográficos imediatos, por meio de redução da embolização distal e do fenômeno do no-reflow e otimização da perfusão miocárdica. Essa técnica, principalmente usada em artérias coronárias, em raras oportunidades é descrita quando empregada em pontes de safena aortocoronarianas. Relatamos o caso de um paciente de 67 anos com infarto agudo do miocárdio relacionado a oclusão de ponte de safena e submetido a trombectomia com bons resultados.

There is a high prevalence of leprosy in the Amazon region of Brazil. We have developed a distance education course in leprosy for training staff of the Family Health Teams (FHTs). The course was made available through a web portal. Tele-educational resources were mediated by professors and coordinators, and included the use of theoretical content available through the web, discussion lists, Internet chat, activity diaries, 3-D video animations (Virtual Human on Leprosy), classes in video streaming and case simulation. Sixty-five FHT staff members were enrolled. All of them completed the course and 47 participants received a certificate at the end of the course. At the end of the course, 48 course-evaluation questionnaires were answered. A total of 47 participants (98%) considered the course as excellent. The results demonstrate the feasibility of an interactive, tele-education model as an educational resource for staff in isolated regions. Improvements in diagnostic skills should increase diagnostic suspicion of leprosy and may contribute to early detection.

Risco de queda em idosos pode ser avaliado por meio de um teste simples de mobilidade. OBJETIVO: Avaliar o equilíbrio de idosos usando o teste Timed up and go. MÉTODO: Os indivíduos tiveram o tempo medido desde o momento em que se levantaram de uma cadeira, caminharam 3 metros para a frente e retornaram à cadeira. Os idosos também responderam questões sobre desequilíbrio, tontura e queda. RESULTADOS: Cerca de 69% dos sujeitos realizaram o teste em até 19 segundos. Houve correlação significativa entre desequilíbrio, tempo dispendido e queda, assim como entre tontura e queda. CONCLUSÃO: A maior parte dos idosos mostrou baixos valores no teste, o que sugere boa mobilidade funcional. Entretanto, um número expressivo de indivíduos com valores mais elevados estão, provavelmente, mais propensos a quedas e à dependência menor ou maior nas atividades da vida diária.

In order to determine if patients with a history of previous urothelial cell carcinoma (UCC) but with current normal urinary cytology have DNA damage in urothelial cells, the single-cell gel electrophoresis (comet) assay was conducted with cells obtained by urinary bladder washings from 44 patients (28 with a history of previous UCC). Increased DNA damage was observed in cytologically normal urothelial cells of patients with a history of UCC when compared with referents with no similar history and after correcting the data for smoking status and age (P < 0.018). Increased DNA damage also correlated with the highest tumor grade, irrespective of time or course of the disease after clinical intervention (Kendall tau correlation, 0.37, P = 0.016). Moreover, aneuploidy, as assessed by DNA content ratio (DCR; 75th/25th percentile of total DNA fluorescence of 50 comets/patient) was unaltered by smoking status, but increased with UCC grade: 1.39 +/- 0.12 (median +/- 95% confidence interval; referents); 1.43 +/- 0.11 (Grade I UCC; P = 0.264, against referents); 1.49 +/- 0.16 (Grade II UCC; P = 0.057); 1.57 +/- 0.16 (Grade III UCC; P = 0.003). Micronucleated urothelial cells (MNC) were also scored on Giemsa-stained routine cytological smears and were found not to correlate with DNA damage or DCR. MNC frequencies were higher for patients with a history of UCC and/or smoking than referents with neither history, but there was no statistical difference between groups. Taken together, these results suggest that the normal-appearing urothelium of patients resected for UCC still harbor genetically unstable cells. (C) 2002 Wiley-Liss, Inc.

Amostras fecais de 203 bezerros com diarréia, idade inferior a 30 dias, de ambos os sexos e de diferentes propriedades do Estado de São Paulo foram examinadas num período de dois anos. Cultivos para pesquisa bacteriana foram feitos em agar acrescido de 10% de sangue bovino e agar Levine. As placas foram incubadas por até 96 horas, em condições aeróbias, a 37°C, com observação dos aspectos de colônia e estudos morfológicos, bioquímicos e realização de outros testes, quando pertinentes. O teste de ELISA foi aplicado para pesquisa de Rotavirus. Cryptosporidium spp. também foi pesquisado e identificado. Resultados revelaram envolvimento de vários patógenos de forma isolada, assim como associados. Rotavirus foi encontrado em 51 (25,1%) das amostras, sendo em 58,8% só, em 41.7% associado a outros microrganismos. Cryptosporydium spp foi isolado em 43 (21.3%) das amostras, sendo só em 65,1% delas e associado a outros enteropatógenos em 34,9%. No exame parasitológico foram encontrados ovos de estrongilídeos em 5 (2,5%) das amostras, não excedendo mais de dois ovos por campo examinado. Ao exame microbiológico, um ou mais microrganismos foram isolados. Escherichia coli foi encontrada em 100% das amostras. As pesquisas de toxina termoestável e do antígeno de aderência K99 realizada nas 73 amostras de E.coli foram negativas, e o grupo sorológico das mesmas foi determinado, sendo 34,2%, 17,8% e 47,9% das amostras pertencentes aos sorogrupos O8, O11 e O101, respectivamente. Salmonella Dublin e Salmonella typhimurium foram isoladas em 5,4% e 6,1% das amostras examinadas, respectivamente.

A protocol for DNA damage assessment by the single-cell gel (SCG)/comet assay in human urinary bladder washing cells was established. Modifications of the standard alkaline protocol included an increase to 2% of sodium sarcosinate in the lysis solution, a reduction in the glass-slide area for comet analysis, and a cutoff value for comet head diameter of at least 30 mum, to exclude contaminating leukocytes. Distinguishing cell populations is crucial, because significant differential migration was demonstrated for transitional and nontransitional cells, phenomena that may confound the results. When applying the modified protocol to urinary bladder cells from smokers without urinary bladder neoplasia, it was possible to detect a significant (P = 0.03) increase in DNA damage as depicted by the tail moment (6.39 +/- 3.23; mean 95% confidence interval; n = 18) when compared with nonsmokers (1.94 +/- 1.41; n = 12). No significant differences were observed between ex-smokers and current smokers regarding comet parameters. Inflammation was not a confounding factor, but DNA migration increased significantly with age in nonsmokers (r = 0.68; P = 0.014). Thus, age matching should be a concern when transitional cells are analyzed in the SCG assay. As it is well known, DNA damage may trigger genomic instability, a crucial step in carcinogenesis. Therefore, the present data directly support the classification of individuals with smoking history as patients at high risk for urinary bladder cancer.

Fifteen cases of dogs with prostatic abscesses are reported. The animals were underwent to physical examination prioritizing the digital rectal exam with transabdominal palpation of the prostate gland followed by blood cell count, measurement of serum urea, creatinine, alanine aminotransferase and alkaline phosphatase levels, abdominal's radiographic and ultrasound exams, and culture of the prostate. The animals were underwent to different treatments being related to the animal's general state, location, size and quantity of prostatic abscesses. This description reiterates the importance of prostatic disease in canine medicine once the detailed physical examination associated with specific complementary tests, particularly x-ray and ultrasound, are valuable for the detection of dog's prostatic diseases. Surgical procedures used here are effective to the treatment of prostatic abscesses with low mortality rate.
