Background: Tolerance and response to antiviral HCV treatment is poor in advanced fibrosis. The aim of this study was to assess SVR rate and its predictive factors in HCV advanced fibrosis patients treated in real life with full dose PEG-IFN plus RBV and to evaluate the adverse events related to treatment. Methods: A multicentric, retrospective study was conducted at six university hospitals. METAVIR F3 and F4 HCV monoinfected patients who were treated with PEG-IFN and RBV had their data analyzed. A stepwise logistic regression analysis was performed to identify the variables independently related to SVR. Adverse events were recorded during treatment. Results: 308 patients were included, 75% genotype 1 and 23% genotype 3. METAVIR F3 was present in 39% and F4 in 61% of patients. The median Child Pugh score for F4 patients was 5 (5–9). The global SVR rate was 34%, 11% were relapsers and 55% were nonresponders. SVR rates were similar between patients treated with PEG-IFN alfa 2a or alfa 2b (p = 0.24). SVR rates according to Child–Pugh score were 26% (Child A) and 18% (Child B). The independent factors related to SVR in F4 patients were genotype 3, RVR and fewer Child Pugh score points. Treatment interruption occurred in 31% patients and death occurred in 1.9%, all with liver cirrhosis. Conclusion: Treatment of HCV in patients with advanced fibrosis should not be postponed. However, a very careful evaluation of cirrhotic patients must be performed before treatment is indicated and careful monitoring is required during treatment.

O sistema de irrigação por aspersão convencional é muito utilizado na suplementação de água na cultura da batata. Pressionado, devido à baixa eficiência de aplicação de água, este têm sido substituído pela irrigação por pivô central e localizada, objetivamente mais eficiente. A adoção de sistemas mais eficientes a programas de manejo de irrigação podem aumentar a eficiência do uso de água da batateira. Diante disso, o presente trabalho objetivou a avaliação do efeito de diferentes lâminas e regimes de irrigação no cultivo de batata durante a estação outono/inverno de 2007, na região Sul do estado de Minas Gerais. O experimento foi conduzido sob delineamento estatístico de blocos casualizados, com parcelas subdivididas e quatro repetições. Nas parcelas as duas frequências de irrigação [F4(quatro dias) e F6(seis dias)], e nas subparcelas as quatro lâminas de irrigação em função do percentual da irrigação total necessária (ITN) [L1: 0,8 ITN (80%); L2: 1,00 ITN (100%); L3: 1,10 ITN (110%) e L4: 1,25 ITN (125%)]. As variáveis qualitativas foram avaliadas pelo teste t, com nível de probabilidade (≤10%) e análise de regressão, considerando os valores dos coeficientes de regressão (R²). O ciclo total foi de 119 dias com evapotranspiração média de 181,0 a 186,4 mm. As menores lâminas aplicadas foram 129,5 mm e 133,3 mm em L1, nas frequências de 4 e 6 dias e as maiores 203,4 mm e 209,3 mm em L4 para as frequências de 4 e 6 dias, respectivamente. As maiores produções de batata graúda ocorreram nas lâminas de irrigação de 0,99 e 1,05 ITN para as frequências F4 e F6. A maior produção total foi obtida em F6, com a lâmina de 1,06 ITN. O maior valor de uso eficiente de água foi de 38,05 kg de tubérculo m-3 de água, obtido com a combinação de tratamentos F6/L2 e o menor 32,89 tubérculo m-3 de água na combinação F4/L1.

Osteoclasts and macrophages share progenitors that must receive decisive lineage signals driving them into their respective differentiation routes. Macrophage colony stimulation factor M-CSF is a common factor; bone is likely the stimulus for osteoclast differentiation. To elucidate the effect of both, shared mouse bone marrow precursor myeloid blast was pre-cultured with M-CSF on plastic and on bone. M-CSF priming prior to stimulation with M-CSF and osteoclast differentiation factor RANKL resulted in a complete loss of osteoclastogenic potential without bone. Such M-CSF primed cells expressed the receptor RANK, but lacked the crucial osteoclastogenic transcription factor NFATc1. This coincided with a steeply decreased expression of osteoclast genes TRACP and DC-STAMP, but an increased expression of the macrophage markers F4/80 and CD11b. Compellingly, M-CSF priming on bone accelerated the osteoclastogenic potential: M-CSF primed cells that had received only one day M-CSF and RANKL and were grown on bone already expressed an array of genes that are associated with osteoclast differentiation and these cells differentiated into osteoclasts within 2 days. Osteoclastogenesis-insensitive precursors grown in the absence of bone regained their osteoclastogenic potential when transferred to bone. This implies that adhesion to bone dictates the fate of osteoclast precursors. Common macrophage-osteoclast precursors may become insensitive to differentiate into osteoclasts and regain osteoclastogenesis when bound to bone or when in the vicinity of bone. J. Cell. The aim of this study was to develop a potentially synbiotic beverage fermented with Lactobacillus casei LC-1 based on aqueous extracts of soy and quinoa with added fructooligosaccharides (FOS). Five formulations with differing proportions of soy and quinoa extracts were tested. The viability of the microorganism, the pH, and the acidity of all formulations were monitored until the 28th day of storage at 5 ℃. The chemical composition of the extracts and beverages and the rheological and sensory properties of the final products were analyzed. Although an increase in acidity and a decrease in pH were observed during the 28 days of storage, the viability of the probiotic microorganism was maintained at 10(8) CFU·mL(-1) in all formulated beverages throughout the storage period. An increase in viscosity and consistency in the formulations with higher concentrations of quinoa (F1 and F2) was observed. Formulation F4 (70% soy and 30% quinoa extracts) showed the least hysteresis. Formulations F4 and F5 (100% soy extract) had the best sensory acceptance while F4 resulted in the highest intention to purchase from a group of 80 volunteers. For chemical composition, F3 (50% soy and 50% quinoa extracts) and F4 showed the best results compared to similar fermented beverages. The formulation F4 was considered the best beverage overall.
