Pre-eclampsia (PE) is a complication of human pregnancy characterized by hypertension and proteinuria after 20 weeks of gestation. Its incidence varies from 5% to 7% of pregnancies and is a major cause of morbidity and maternal and fetal mortality. This is a multisystemic disease, with focus on vascular dysfunction and is closely related to the exacerbated activation of the immune system. In addition to increased activation of monocytes and granulocytes, there is an elevated production of proinflammatory cytokines in pregnant women with PE. The nuclear transcription factor-kB (NF-kB) is present in the cells of the immune system and is responsible for transcription of genes related to inflammation. Whereas the PE is associated with intense inflammatory response, the use of substances modulating the activity of NF-kB factor could be useful in alleviating the inflammation present in these patients. Silibinin is the main component of silymarin, a polyphenolic extract obtained from fruits and seeds of Sylibum marianum with potent hepatoprotective, anti-inflammatory and anti-fibrotic activities. The silibinin mechanism of action includes the ability to inhibit NF-kB activation and, consequently, its migration to the nucleus. The objective of this study was to assess whether silibinin modulates the activity of NF-kB and the production of inflammatory cytokines in mononuclear cells of patients with PE. We evaluated 34 pregnant women with PE, 20 normotensive pregnant women (GN) and 15 non-pregnant women (NG). Peripheral blood mononuclear cells (PBMC) were obtained from those groups of women and cultured in the presence or absence of silibinin (5 uM or 50 uM) and stimulated or not with lipopolysaccharide (LPS) for 18 h to obtain supernatant for determination of tumor necrosis factor-alpha (TNF-α) and interleukin-1 (IL-1β) by enzyme immunoassay (ELISA). The Rooming favors the permanence of the mother with the newborn, thus establishing a stronger link between the mother and baby, in addition, care assistance and guidance are essential to both through the health care team because they recognize the moments critical that their interventions are necessary to ensure the health of both. The educational lecture was an option to have the opportunity to assist, guide and answer possible questions that may arise for mothers, and also to assess the knowledge of puéperas regarding the care of babies and self care. There was participation from 40 patients. In the study methodology was developed a questionnaire containing two subjects (self-care and care for newborns) was given to patients before and after the lecture held by the author in a rooming Maternity HC de Botucatu. The results were analyzed statistically and studied by the author, allowing note that many significant results were obtained after the lecture, which we realize that the same intervention on self-care was significant in respect of: bleeding, infection in the genital tract and urinary tract, surgical dehiscence , care of the lower limbs and constipation, how to care for the newborn was the significance to the questions about the hygiene of the umbilical stump, cramps, hunger, weeping and pacifiers. In the pre-lecture questions 73.00% of 27.00% were right and wrong. The humanization of childbirth implies the understanding of this event as an important experience, and requires the redefinition of human relationships from the review of the assistance project, the understanding of pregnant women condition and human rights. To evaluate child birth assistance using a score that was developed by Botucatu Cuesta Regional Management and Jurumirim Valley Collegiates. This is an epidemiological, descriptive study that is inserted in the field of evaluation of services, programs or health projects. The data has been obtained by direct observation of deliveries, emphasizing the humane care. Results: Approximately one third of the women gave birth in a tertiary hospital (34.1%) and it was their first babies (33%).24.8% of the women received prenatal care in high-risk services. 67.1% of the births were normal, and 84.7% had no companions in the delivery room. In 47.1% of the cases the delivery was performed by obstetrician who used anesthesia in 44.7% and episiotomy in 48.2% of the deliveries. More than half of the newborns were attended by the pediatrician in the delivery room and had a delivery graph completed. Although the present study shows that 67.1% of the births were normal, caesarean rate can be considered excessive, as the WHO points out that c-sections above 15% are unlikely to be justifiable. It is important to emphasize that the Ministry of Health has to have a commitment with all women to promote safe motherhood, even in cases when the pregnancy involves a risk for both the mother and the fetus. It is noteworthy that the created score allowed us to assess variables related to the humanization of childbirth and only average and quite similar situations among the three services were evidenced. We hope that with this study, managers and professionals that work in this area can be subsidized in order to offer effective humane assistance and quality service in the delivery

Introductions: A supernumerary tooth is one that is additional to the normal series and can be found in almost any region of the dental arch. Its etiology is not fully understood. With regard to its prevalence, it occurs more commonly in permanent dentition and twice as often in men than in women. Supernumerary teeth are classified according to their morphology and location. Their presence can cause problems such as failure of eruption, displacement of teeth, crowding and odontogenic cysts and tumors. The diagnosis is usually by routine radiographs, for the majority of such teeth are impacted and asymptomatic. Objective and Case report: The purpose of this study was to make a brief review of the relevant literature and report one clinical case of the female patient, 23 years, melanoderma, featuring three quarters molars. Final considerations: The early diagnosis proved to be important for the resolution of the case in order to minimize or even prevent the development of complications and thus establish a proper plan of treatment.
