Introduction: The HELLP syndrome is a severe complication of pregnant women with preeclampsia (PE), characterized by association of hemolysis, changes in liver enzymes and thrombocytopenia. Hemolysis, defined by the presence of microangiopathic hemolytic anemia, is one of the characteristics in this syndrome. However, as hemolysis occurs in a short time there is some difficulty in its laboratory diagnosis. Therefore, the search for a more sensitive and specific method for hemolysis determination may help in the early diagnosis of the HELLP syndrome. Objectives: a) To determine the plasma concentration of haptoglobin in normotensive pregnant women and in pregnant women with PE, classified into mild PE, severe PE and HELLP/partial HELLP syndrome; b) To compare the efficacy of haptoglobin plasma concentration and serum total bilirubin as criteria for hemolysis diagnosis in HELLP/partial HELLP syndrome. Methods: We conducted a cross-sectional analytical and comparative study involving 66 pregnant women diagnosed with PE, being 25 cases with mild PE, 28 with severe PE, and 13 with HELLP/partial HELLP syndrome. Twenty-one normotensive pregnant women were included for comparison of haptoglobin plasma concentration between the groups and to determine the normal values for pregnant women. The variables studied were: maternal age, gestational age, systolic and diastolic blood pressure, proteinuria, hematocrit and hemoglobin values, platelet count, serum total bilirubin, lactate dehydrogenase (LDH), glutamic oxaloacetic transaminase (AST) and glutamic-pyruvic transaminase (ALT), urea, creatinine and uric acid, and also plasma concentrations of haptoglobin. The results were analyzed by nonparametric tests, with a significance level of 5%. A atorvastatina, amplamente utilizada no tratamento de dislipidemias, é fármaco inibidor da enzima 3-hidroxi-3-metilglutaril coenzima A redutase (HMG-CoA) redutase que atua na etapa limitante da síntese do colesterol nas células hepáticas. O transporte hepatocelular da atorvastatina é mediado principalmente pelo polipeptídeo transportador de ânions orgânicos (OATP) 1B1, o qual é altamente polimórfico geneticamente e está sujeito a interação com outros fármacos usados na clínica. O presente estudo teve como objetivo revisar a literatura científica baseada em estudos clínicos, in vitro e experimentais a fim de avaliar o impacto de polimorfismos genéticos de OATP1B1 na disposição cinética, eficácia e toxicidade da atorvastatina. Os estudos clínicos demonstram que o polimorfismo c.521T>C, presente nos alelos SLCO1B1 *5 e *15, resulta em alteração funcional da atividade transportadora, sendo que indivíduos homozigotos para o alelo raro C, apresentam maiores concentrações plasmáticas de atorvastatina e seus metabólitos ativos 2-hidroxiatorvastatina e 4- hidroxiatorvastatina. Entretanto, o aumento na concentração plasmática da atorvastatina e seus metabólitos ativos em indivíduos c.521 CC parece não ser acompanhado de redução na concentração dos mesmos no sítio alvo, tendo em vista que não foram encontradas diferenças significativas na redução da LDL-C ou colesterol total quando comparados aos indivíduos homozigotos para o alelo selvagem. O alelo SLCO1B1*15 mostrou-se significativamente associado a miopatia induzida pela atorvastatina, apesar do pequeno número de pacientes investigados. Estudos in vitro em células expressando OATP1B1 são necessários a fim de investigar o impacto do polimorfismo c.521T>C na concentração de atorvastatina nos hepatócitos. In this article the authors clinically compare the efficacy of two different anti-inflammatory drugs - Etoricoxib (Arcoxia®) and Diclofenac (Olfen®) – in the control of postoperative pain resulting from the surgical removal of impacted lower third molars. Fifteen patients requiring the extraction of bilaterally impacted lower third molars were selected at the Department of Oral and Maxillofacial Surgery at the Araraquara School of Dentistry – UNESP. The drugs were randomly administered during the first and second surgical procedures. Pain was evaluated by means of a visual analogic scale for 72 hours following the surgical procedure. After statistical analysis of the results, the authors concluded that there were no significant differences in terms of postopoerative pain control between the two drugs studied.

Objective: To compare the efficacy of the mouthwashes 0.12% chlorhexidine, Listerine, and 0.5% and 2% Melaleuca Alternifolia oil against the salivary levels of Streptococcus mutans and total microorganisms. Methods: This study was double-blind controlled and paired clinical assay. Twenty-six volunteers aged 21 to 35 years old were enrolled. At baseline, 1 mL of unstimulated saliva was collected from each subject, 1 and 15 min after mouthrinsing with the following solutions: sterile distilled water, 0.12% chlorhexidine digluconate, Listerine (©Johnson & Johnson do Brasil), 0.5% and 2% concentrations of Melaleuca Alternifolia (Sigma-Aldrich). The volunteers used all the evaluated mouthrinses with a 15-day interval between the solutions. Immediately after rinsing, saliva was collected and serial dilutions were performed, followed by plating in blood agar culture medium for growth of total microorganisms and SB-20 (Sucrose-Bacitracin agar) for growth of S. mutans, and incubation at 37 °C for 48 h in microaerophilia. After incubation, the number of colonies was counted and expressed as colony forming units (UFC/mL). Results: Chlorhexidine showed antimicrobial action by reducing total microorganisms and S. mutans, while the action of 0.5% Melaleuca Alternifolia was similar to that of distilled water. Listerine and 2% Melaleuca Alternifolia oil reduced total microbial counts by 11% and 9% respectively, and S. mutans by 20% and 11%. Conclusion: A single rinse with 0.12% chlorhexidine is effective in reducing the levels of total microorganisms and S. mutans present in saliva. Under the same testing conditions, Listerine and 0.5% and 2% Melaleuca Alternifolia oil presented lower efficacy than chlorhexidine.

The irrigation of root canals aims to their cleaning and disinfection, improving endodontic treatment success. OBJECTIVE: To investigate the influence of the diameter and type of irrigation needle and the root canal enlargement on the mechanical efficacy of endodontic irrigation. MATERIAL AND METHODS: Twelve human single-rooted mandibular incisors were used. During some instrumentation stages (enlargement by #20, #30, and #40 K file), root canals were filled with radiographic contrast solution mixed to propyleneglycol and zinc oxide. Needles with different diameters and designs were employed: G1 – 23G and lateral opening; G2 – 22G and apical opening; G3 – 30G and lateral opening; G4 – 30G and apical opening. The needles were inserted up to resistance, with 1 mm step-back to avoid root canal obliteration. The irrigation was performed with 2 mL of distilled water. Before and after irrigation/aspiration, teeth were radiographed at bucco-lingual and mesiodistal direction, using a digital radiographic system. Then, root canal areas, before (filled by contrast solution) and after irrigation (remnant of contrast solution), were submitted to image subtraction with Adobe Photoshop CS4 software. Subsequently, the areas were measured by Image Tool 3.0 software, allowing the obtaining of the cleaning percentage for each modality. Data were analysed by using Anova and Tukey's test. The level of significance was set at P < 0.05. RESULTS: For all root canal enlargements, 30G needles (G3 e G4) presented a better cleaning efficacy. In all groups, higher cleaning efficacy percentage was observed at #30 and #40 K file enlargement. CONCLUSION: Regardless their design, thinner needles were more effective; a better cleaning efficacy occurred in more enlarged root canals.

O carcinoma espinocelular é uma das quatro neoplasias malignas que mais acomete gatos, principalmente os de pele clara e que residem em países tropicais onde a incidência de raios solares é intensa. O tratamento cirúrgico é o mais indicado, mas nem sempre é possível de ser realizado seja pelo estágio avançado do tumor, pela sua localização ou por opção do proprietário que não quer ver seu animal mutilado. O objetivo desta revisão sistemática foi verificar a eficácia da quimioterapia com ou sem associações terapêuticas para o tratamento do carcinoma espinocelular superficial, multicêntrico e de cabeça e pescoço em gatos. Os melhores resultados para o tratamento do carcinoma espinocelular superficial foram observados com a aplicação intralesional do quimioterápico associado à radioterapia ou à eletroterapia, para o tipo multicêntrico faltam trabalhos sobre o assunto e o único encontrado não apresentou resultados satisfatórios. No caso do carcinoma espinocelular de cabeça e pescoço que alberga o pior prognóstico, a terapia multimodal com associação de quimioterapia, radioterapia e excisão cirúrgica demonstrou ser eficiente. Mais estudos sobre o assunto devem ser realizados utilizando um número maior de animais e com um tempo de seguimento mais longo
