The main objective of the presented study is the development of a predictive interval type-2 fuzzy inference system in order to estimate the mortality risk for a newborn, to be used as an auxiliary tool for decision making in medical centers where there is a lack of professionals for this purpose and, afterwards, to compare its performance to a type-1 fuzzy system. The input variables were chosen due to their acquisition ‘simplicity, not involving any invasive tests, such as blood tests or other specific tests. The variables are easily obtained in the first few minutes of life: birth weight, gestational age at delivery, 5-minute Apgar score and previous report of stillbirth. Databases from the DATASUS were used to validate the model. 1351 records from the city of São José dos Campos, a mid-sized city in the São Paulo state’s countryside, were considered in this study. Finally, an analysis using the ROC curve was performed to estimate the model’s accuracy

In epidemiological studies, when the data is collected by interview, it is of importance to analyse the reliability of the information. This study was carried out with the purpose of examining the self-agreement of mothers in answering questions about variables of interest in oral cleft researches on two different occasions. A sample of 91 mothers of oral cleft babies were interviewed on two different occasions. The capacity of giving the same answer to questions related to heredity, type of clefts, period of gestation of the mother and birthweight were studied. The statistics type kappa (k) and intraclass correlation coefficient (r) by point and by 95% of confidence interval were applied. The intra-observer agreement for the variables history of oral clefts in the family, type of cleft, period of gestation of the mother and birthweight of the newborn was, respectively, k=0.9492, k=1.0000, k=0.9281 and r=0.9996. We concluded that the background on oral cleft in the family history of patients with this anomaly is a variable with an excellent degree of reliability. Also, the information given by the mothers related to the period of gestation, type of the baby’s cleft and birthweight are reliable.

Objetivo: Examinar o padrão de consumo do tabaco e conhecimentos sobre as doenças relacionadas ao tabaco, assim como identificar os tipos mais populares de mídias entre gestantes para aprimorar estratégias para a prevenção e a cessação do tabagismo entre essas mulheres. Métodos: Estudo transversal com 61 gestantes atendidas em um hospital universitário e em unidades básicas de saúde em Botucatu, SP. A Escala Hospitalar de Ansiedade e Depressão foi aplicada a todas as participantes. Para aquelas com história de tabagismo, também foi aplicado o Teste de Fagerström para Dependência de Nicotina, e foi avaliado o grau de motivação para cessação tabágica nas fumantes. Resultados: Das 61 gestantes avaliadas, 25 (40,9%) eram fumantes (média de idade, 26,4 ± 7,4 anos), 24 (39,3%) eram ex-fumantes (média de idade, 26,4 ± 8,3 anos), e 12 (19,8%) nunca fumaram (média de idade, 25,1 ± 7,2 anos). A exposição passiva foi relatada por 39 gestantes (63,9%). Das 49 fumantes/ex-fumantes, 13 (29,6%) conheciam as consequências pulmonares do tabagismo; somente 2 (4,9%) conheciam os riscos cardiovasculares; 23 (46,9%) acreditavam que fumar não causa nenhum problema para o feto ou o recém-nascido; 21 (42,9%) consumiram álcool durante a gestação; 18 (36,7%) relataram aumento no consumo de cigarros quando bebiam; 25 (51,0%) experimentaram cigarros com sabores; e 12 (24,5%) fumaram narguilé. Entre as 61 gestantes avaliadas, a televisão foi o tipo de mídia mais disponível e favorita (85,2%), assim como a mais preferida (49,2%). Conclusões: Entre gestantes, o fumo ativo, o fumo passivo e o uso de formas alternativas de consumo de tabaco parecem ser altamente prevalentes, e tais mulheres parecem possuir poucos conhecimentos sobre as consequências do uso de tabaco. Programas educacionais que incluam informações sobre as consequências de todas as formas de uso de tabaco, utilizando formatos novos e efetivos para esta população específica, devem ser desenvolvidos para promover a prevenção/cessação do tabagismo entre gestantes. Amostras adicionais para explorar diferenças culturais e regionais são necessárias.

Objective: To understand the practices related to late-onset sepsis (LOS) in the centers of the Brazilian Neonatal Research Network, and to propose strategies to reduce the incidence of LOS.Methods: This was a cross-sectional descriptive multicenter study approved by the Ethics Committee. Three questionnaires regarding hand hygiene, vascular catheters, and diagnosis/treatment of LOS were sent to the coordinator of each center. The center with the lowest incidence of LOS was compared with the others.Results: All 16 centers answered the questionnaires. Regarding hand hygiene, 87% use chlorhexidine or 70% alcohol; alcohol gel is used in 100%; 80% use bedside dispensers (50% had one dispenser for every two beds); practical training occurs in 100% and theoretical training in 70% of the centers, and 37% train once a year. Catheters: 94% have a protocol, and 75% have a line insertion team. Diagnosis/treatment: complete blood count and blood culture are used in 100%, PCR in 87%, hematological scores in 75%; oxacillin and aminoglycosides is the empirical therapy in 50% of centers. Characteristics of the center with lowest incidence of LOS: stricter hand hygiene; catheter insertion and maintenance groups; use of blood culture, PCR, and hematological score for diagnosis; empirical therapy with oxacillin and aminoglycoside.Conclusion: The knowledge of the practices of each center allowed for the identification of aspects to be improved as a strategy to reduce LOS, including: alcohol gel use, hand hygiene training, implementation of catheter teams, and wise use of antibiotic therapy. Early diagnosis of hearing loss minimizes its impact on child development. We studied factors that influence the effectiveness of screening programs. To investigate the relationship between gender, weight at birth, gestational age, risk factors for hearing loss, venue for newborn hearing screening and pass and fail results in the retest. Prospective cohort study was carried out in a tertiary referral hospital. The screening was performed in 565 newborns through transient evoked otoacoustic emissions in three admission units before hospital discharge and retest in the outpatient clinic. Gender, weight at birth, gestational age, presence of risk indicators for hearing loss and venue for newborn hearing screening were considered. Full-term infants comprised 86% of the cases, preterm 14%, and risk factors for hearing loss were identified in 11%. Considering the 165 newborns retested, only the venue for screening, Intermediate Care Unit, was related to fail result in the retest. Gender, weight at birth, gestational age and presence of risk factors for hearing loss were not related to pass and/or fail results in the retest. The screening performed in intermediate care units increases the chance of continued fail result in the Transient Otoacoustic Evoked Emissions test.
