Leprosy is a complex infectious disease influenced by genetic and environmental factors. The genetic contributing factors are considered heterogeneous and several genes have been consistently associated with susceptibility like PARK2, tumor necrosis factor (TNF), lymphotoxin-alpha (LTA) and vitamin-D receptor (VDR). Here, we combined a case-control study (374 patients and 380 controls), with meta-analysis (5 studies; 2702 individuals) and biological study to test the epidemiological and physiological relevance of the interleukin-10 (IL-10) genetic markers in leprosy. We observed that the -819T allele is associated with leprosy susceptibility either in the case-control or in the meta-analysis studies. Haplotypes combining promoter single-nucleotide polymorphisms also implicated a haplotype carrying the -819T allele in leprosy susceptibility (odds ratio (OR) = 1.40; P = 0.01). Finally, we tested IL-10 production in peripheral blood mononuclear cells stimulated with Mycobacterium leprae antigens and found that -819T carriers produced lower levels of IL-10 when compared with noncarriers. Taken together, these data suggest that low levels of IL-10 during the disease outcome can drive patients to a chronic and unprotective response that culminates with leprosy.

A hanseníase é doença infecto-contagiosa para a qual, além das condições individuais, outros fatores relacionados aos níveis de endemia e às condições socioeconômicas desfavoráveis influem no risco de adoecer. Objetivou-se reconhecer o perfil socioeconômico e demográfico e o grau de incapacidade instalado dos portadores de hanseníase, atendidos no Centro de Saúde Escola de Botucatu, São Paulo, Brasil. Fizeram parte do estudo 37 pacientes. Os dados foram obtidos por meio do instrumento de consulta de enfermagem. Os resultados mostraram predominância de indivíduos com união estável (78%), brancos (92%), com idade entre 30 e 49 anos (51%), com baixo nível de escolaridade e com renda familiar per capita menor que um salário mínimo. Mais de um terço dos pacientes investigados (35%) apresentavam algum grau de incapacidade física. A associação do baixo perfil socioeconômico com a presença de incapacidades físicas imprime maior vulnerabilidade a essa população, podendo impactar negativamente a sua qualidade de vida.

ObjectiveExplore the presentation, diagnostic criteria and exocrine gland histopathology of paediatric primary Sjogren's syndrome (PPSjS).MethodsA case series of 8 children is reported and American-European Consensus Group (AECG-2002) criteria were examined, as well as minor labial salivary and lachrymal gland biopsies, which were scored by a pathologist blinded to outcome. For all cases, connective tissue diseases and parotid-related infectious disease were excluded.ResultsAge at onset varied from 5-13 years old; 6 were females, all followed from diagnosis up to the last visit (1-10 years). The main features at presentation were recurrent tender parotid swelling and sialectasis imaging, with decreased salivary function assessed by Tc-99 scintigraphy. Mild sicca symptoms were observed in 4/8 cases. Systemic features, including fatigue, myalgia, arthritis, tenosynovitis, joint contractures, transient Raynaud's and high ESR, were recorded at onset. Autoantibody profile was unremarkable for diagnosis, while lymphocytic infiltration of labial salivary glands and sialectasis were observed in all biopsies (8/8). In lachrymal glands, massive lymphocytic infiltration and lymphocytic gastritis were observed during complementary assessment. Flares were treated with low dose steroids and long-term use of hydroxychloroquine (5/8), although only 318 fulfilled AECG-2002 diagnostic criteria, throughout the disease course.ConclusionPPSjS is rare, slowly progressive and its early presentation is variable. Standardised diagnostic algorithms should include recurrent parotid swelling and early diagnosis should rely mostly on salivary and lachrymal gland histopathology in this age group.

Canine visceral leishmaniasis is an endemic infectious disease in some regions from São Paulo state and even though it is a systemic disease, in the dog, the main clinical sign is dermatological. Thirty dogs with positive results in serology (IFI) and parasitological exams (fine needle aspiration) for leishmaniasis from Aracatuba city were evaluated. They only showed dermatological signs and were divided in two groups, one with exfoliative (n=15) and other with ulcerative (n=15) lesions. There was significant statistical difference between CD3 and CD79a population in the same dermatological pattern, and with CD79a population between exfoliative and ulcerative patterns (p<0,05).

Baypamun® é um imunomodulador recomendado na profilaxia e terapêutica de enfermidades. No presente estudo, este produto foi usado em hamsters experimentalmente infectados com Leptospira interrogans, sorogrupo canicola. Estes animais foram divididos em 5 grupos com 20 animais cada. O produto foi usado como terapêutico, profilático e como adjuvante. Quando usado de forma terapêutica, todos os animais vieram a óbito. Quando usado na profilaxia, 8 animais (40%) sobreviveram e foi possível recuperar o agente a partir dos rins desses animais. Baseado na análise do título de anticorpos nos animais que receberam ou não o Baypamun como adjuvante vacinal, o produto não elevou a resposta humoral. Os resultados sugerem, entretanto, outros estudos que utilizem o produto por períodos mais prolongados, além de se verificar a resposta celular para se avaliar o seu efeito como adjuvante nas infecções.

Mastitis is the most common infectious disease affecting dairy cattle; in addition, it remains the most economically important disease of dairy industries around the world. Streptococcus agalactiae, a contagious pathogen associated with subclinical mastitis, is highly infectious. This bacterium can cause an increase in bulk tank bacterial counts (BTBC) and bulk tank somatic cell counts (BTSCC). The microbiological identification of S. agalactiae in samples from bulk tanks is an auxiliary method to control contagious mastitis. Thus, there are some limitations for time-consuming cultures or identification methods and additional concerns about the conservation and transport of samples. Bulk tank samples from 247 dairy farms were cultured and compared through polymerase chain reaction (PCR), directed to 16S rRNA genes of S. agalactiae, followed by BTBC and S. agalactiae isolation. The mean value of BTBC was 1.08 x 10(6) CFU mL(-1) and the bacterium was identified through the microbiological method in 98 (39.7%; CI95% = 33.8-45.9%) and through PCR in 110 (44.5%; CI95% = 38.5-50.8%) samples. Results indicated sensitivity of 0.8571 +/- 0.0353 (CI95% = 0.7719-0.9196) and specificity of 0.8255 +/- 0.0311 (CI95% = 0.7549-0.8827). The lack of significant difference between microbiological and molecular results (kappa = 0.6686 +/- 0.0477 and CI95% = 0.5752-0.7620) indicated substantial agreement between the methods. This suggests that PCR can be used for bulk tank samples to detect contagious mastitis caused by S. agalactiae. (C) 2011 Elsevier Ltd. All rights reserved.

They just showed dermatological signs and were divided in two groups, one with exfoliative(n = 15) and other with ulcerative (n = 15) lesions. Histopathological. patterns in the group of exfoliative lesions were: periadnexial dermatitis (5115, 33,3%), superficial perivascular dermatitis (1/5, 6,6%), nodular dermatitis (1115, 6,6%) and mixed dermatitis (8/15, 53,3%), including intersticial/periadnexial dermatitis (1/8, 12,5%). lichenoid/perivascular superficial and deep dermatitis (1/8, 12,5%), perivascular superficial and deep/periadnexial dermatitis (1/8, 12,5%) and superficial perivascular/perianexial dermatitis (5/8, 62,5%). In the group of ulcerative lesions, the histopathological patterns were: perivascular superficial and deep dermatitis (5115, 33,3%), diffuse dermatitis (3/15. 20%), periadnexial dermatitis (2/15, 13,3%), nodular dermatitis (1/15, 6,6%) and mixed dermatitis (4/15, 26,6%), including intersticial/perivascular superficial and deep dermatitis (1/4, 25%), nodular/periadnexial dermatitis (1/4, 25%), fibrosing/perianexial dermatitis (1/4. 25%) and perivascular superficial and deep/periadnexial dermatitis(1/4, 25%). Parasites were found in eight dogs (8/15, 53,3%) with exfoliative dematitis and seven (7/15, 46,6%) with ulcerative dermatitis.

Background: Non-Hodgkin's lymphomas (NHLs) are a heterogeneous group of lymphoproliferative malignancies that may be associated with acquired immunodeficiency syndrome (AIDS). NHL can disseminate to extranodal sites; however, its dissemination to the jaws and mouth is not common. This report presents and discusses two unusual cases of gingival primary extranodal non-Hodgkin's lymphoma (PE-NHL) as the first manifestation of AIDS.Methods: Two mates presented with asymptomatic gingival swelling. They were examined clinically. Biopsies of the gingival tissue were evaluated using routine histologic techniques and immunohistochemistry. The patients were tested for human immunodeficiency virus (HIV) infection.Results: The clinicopathological evaluation and the serological HIV examination of the patients led us to the final diagnosis of gingival PE-NHL as the first manifestation of AIDS. Both patients were referred to an oncologist and to an infectious disease specialist and were given antineoplastic chemotherapy and highly active antiretroviral therapy. Only one patient presented a favorable clinical evolution.Conclusion: The present case reports have important clinical implications; the two unusual presentations of gingival PE-NHL contribute to information about the differential diagnosis of rapidly progressing gingival swelling.

In bacteria, fungi, plants, and apicomplexan parasites, the aromatics compounds, such as aromatics amino acids, are synthesized through seven enzymes from the shikimate pathway, which are absent in mammals. The absence of this pathway in mammals make them potential targets for development of new therapy against infectious diseases, such as tuberculosis, which is the world's second commonest cause of death from infectious disease. The last enzyme of shikimate pathway is the chorismate synthase (CS), which is responsible for conversion of the 5-enolpyruvylshikimate-3-phosphate to chorismate. Here, we report the crystallographic structure of CS from Mycobacterium tuberculosis (MtCS) at 2.65 angstrom resolution. The MtCS structure is similar to other CS structures, presenting beta-alpha-beta sandwich structural topology, in which each monomer of MtCS consists of a central helical core. The MtCS can be described as a tetramer formed by a dimer of dimers. However, analytical ultracentrifugation studies suggest the MtCS is a dimer with a more asymmetric shape than observed on the crystallographic dimer and the existence of a low equilibrium between dimer and tetramer. Our results suggest that the MtCS oligomerization is concentration dependent and some conformational changes must be involved on that event. (c) 2005 Elsevier B.V. All rights reserved.

Uma versão condensada em português de um artigo de revisão sobre a periodontite da cara inchada dos bovinos, publicado em inglês, está apresentada com algumas informações adicionais. A doença foi responsável por grandes perdas de bovinos jovens, principalmente nas décadas de 1970 e l980 no Brazil Central. em face da periodontite progressiva e a perdas de dentes, os animais não podem se alimentar convenientemente, tornam-se emaciados e podem morrer. A doença foi tida como uma deficiência ou desequilíbrio mineral. Mas as pesquisas de campo e de laboratório, realizadas durante 30 anos, mostraram que trata-se de doença infecciosa multifatorial a ser definida como Periodontite Epizoótica Bovina. Chegou-se à conclusão que os fatores principais para o seu desenvolvimento são: (1) a idade dos bovinos na fase de erupção dos dentes premolares e molares; (2) a presença de bactérias do grupo Bacteroides spp nos espaços subgengivais; e (3) a ingestão com a forragem de concentrações subinibitórias de antibióticos, sobretudo de estreptomicina, produzidos por actinomicetos cujo número é aumentado em solos virgens recém-cultivados na formação de pastagens após a derrubada da mata ou da vegetação de Cerrado; isto leva a um aumento da aderência dos bacteróides ao epitélio gengival e à destruição dos tecidos peridentários. Hoje em dia, a doença perdeu a sua importância e praticamente desapareceu, porque a microbiota do solo entrou novamente em equilíbrio e a abertura de grandes áreas virgens para a pecuária cessou. Porém, novos surtos podem ocorrer em áreas anteriormente positivas para a doença quando, na reforma de pastagens ou capineiras, houver um novo desequilíbrio da microbiota do solo. Outros antibióticos, como a espiramicina e virginiamicina, administrados por via oral ou adicionado a misturas minerais, podem controlar a periodontite.

The in vitro activity of cefepime was compared to that of ceftazidime, ceftriaxone, and cefotaxime in a multicenter study involving 10 clinical microbiology laboratories and clinical isolates from 18 Brazilian hospitals from 7 cities (4 states). A total of 982 isolates consecutively collected between December 1995 and March 1996 were susceptibility tested by using Etest and following the NCCLS procedures for agar diffusion tests. The cefepime spectrum was broader than that of the other broad-spectrum cephalosporins against both Gram-negative rods and Gram-positive cocci. Cefepime tons particularly move active against Enterobacter sp. (MIC90, 2 mu g/ml), Serratia sp. (MIC90, 2 mu g/ml) and oxacillin-susceptible Staphylococcus aureus (MIC90, 3 mu g/ml). Against Pseudomonas aeruginosa, cefepime (MIC90 16 mu g/ml) was slightly more active than ceftazidime (MIC90 32 mu g/ml) and 8- to 16-fold more active than ceftriaxone or cefotaxime (MIC90 >256 mu g/ml). Our results show that nosocomial bacteria, especially Gram-negative rods, have a high rate of cephalosporin resistance in Brazil. However, part of these resistant bacteria remains susceptible to cefepime. The Etest was shown to be an excellent method for multicenter studies of the in vitro evaluation of new antimicrobial agents. (C) 1997 Elsevier B.V.

For the molecular diagnosis of Plasmodium vivax variants (VK210, VK247, and P. vivax-like) using DNA amplification procedures in the laboratory, the choice of rapid and inexpensive identification products of the 3 different genotypes is an important prerequisite. We report here the standardization of a new polymerase chain reaction/restriction fragment length polymorphism technique to identify the 3 described P. vivax circumsporozoite protein (CSP) variants using amplification of the central immunodominant region of the CSP gene of this protozoan. The simplicity, specificity, and sensitivity of the system described here is important to determine the prevalence and the distribution of infection with these P. vivax genotypes in endemic and nonendemic malaria areas, enabling a better understanding of their phylogeny.
