Este estudo pesquisou sintomas de stress em adultos jovens, relacionando-os com o sexo e ano escolar em curso. Investigou também o tipo e a freqüência de sintomas. Participaram 295 estudantes de 15 a 28 anos, sendo 150 mulheres e 145 homens, que cursavam o primeiro e terceiro anos do ensino médio, curso pré-vestibular e primeiro e quarto anos de ensino superior. A avaliação do stress foi realizada através do Inventário de Sintomas de Stress de Lipp. Os resultados acusaram correlação significativa entre sexo e nível de stress (p<0,0001) sendo que as mulheres apresentaram maior nível de stress em todos os grupos avaliados. Maior índice de stress surgiu em estudantes do curso Pré-Vestibular, seguidos do terceiro ano do ensino médio. A sintomatologia apresentada foi predominantemente psicológica e os sintomas mais prevalentes foram sensibilidade emotiva excessiva, para as mulheres e, para os homens, pensamento recorrente.

Megaesophagus, an affection characterized by aperistalses of the esophageal body and deficient relaxation of lower esophageal sphincter, has disphagia as main symptom. The aim of this study was to evaluate the nutritional status of patients with non advanced megaesophagus in pre and post-operative periods of cardiomyotomy. Ten patients were evaluated at 5 moments (pre-operative - M 1 and post-operative - 1, 3, 6 ant 12 months after surgery). The anthropometric, hematimetric and biochemical parameters were studied in the 5 moments. Conclusions: 1. Most 017 the patients with non-advanced megaesophagus are eutrophic; 2. Surgical treatment determines an improvement in nutritional status and an increase in HDL cholesterol values.

OBJETIVO: Avaliar a qualidade de vida relacionada à saúde de pacientes com doença obstrutiva crônica das vias aéreas recebendo oxigenoterapia domiciliar prolongada (ODP) por meio de cilindros de oxigênio e comparar estes resultados com os obtidos após seis meses de modificação do sistema de fornecimento para concentradores de oxigênio. MÉTODOS: Um total de 45 pacientes, 24 com hipoxemia crônica e 21 sem evidências de hipoxemia, foram avaliados. Os pacientes com hipoxemia crônica estavam recebendo ODP regularmente durante pelo menos os últimos seis meses e foram avaliados no momento basal, em uso de cilindro, e após seis meses de transição para concentradores. Os pacientes não hipoxêmicos foram avaliados no mesmo intervalo de tempo que os pacientes hipoxêmicos. Para avaliar a qualidade de vida foi utilizada a versão validada para língua portuguesa (Brasil) do Questionário Respiratório Saint George (Saint George's Respiratory Questionnaire - SGRQ). RESULTADOS: No momento inicial, os pacientes hipoxêmicos apresentaram maior comprometimento da qualidade de vida, avaliada pelo escore total e pelos escores dos domínios sintomas e impacto do SGRQ, que os pacientes não hipoxêmicos. Após seis meses, houve melhora significativa da qualidade de vida dos pacientes hipoxêmicos e, neste momento, não foi encontrada diferença entre os pacientes com e sem hipoxemia. CONCLUSÃO: Nossos achados mostraram que os pacientes com doença obstrutiva crônica das vias aéreas e hipoxemia crônica apresentam prejuízo da qualidade de vida, que essa qualidade de vida pode ser melhorada com o uso regular de ODP e que o sistema de fornecimento de oxigênio tem influência nessa melhora.

Freshwater stingrays are very common in the Parana, Paraguay, Araguaia, and Tocantins Rivers and tributaries in Brazil. This study presents the clinical aspects of 84 patients injured by freshwater stingrays. Intense pain was the most conspicuous symptom. Skin necrosis was observed in a high percentage of the victims, mostly fishermen and bathers. The initial therapeutic procedures, like immersion of the affected member in hot water were effective in the initial phases of the envenoming, especially in the control of the acute pain; however, they did not prevent skin necrosis. By SDS-PAGE, the freshwater stingray (Potamotrygon falkneri) venom extract presented a major band of approximately 12 kDa. Several other components distributed between 15 and 130 kDa were detected in the venom extract. Many components with molecular mass above 80 and 100 kDa have gelatinolytic and caseinolytic activities, respectively. Hyaluronidase activity was detected only in a component around 84 kDa in P. falkneri venom extract. Our results demonstrated that the presence of these enzymes could explain partially the local clinical pictures presented by patients wounded by freshwater stingray. (C) 2004 Elsevier Ltd. BackgroundLower limb venous ulceration ranks among the 10 most common medical problems in Western countries and has significant socioeconomic impact. The aim of this study was to identify the risk factors for unhealed, recurrent, and large ulcers and to characterize patients with active or recently healed venous ulcers.MethodsWe identified 97 patients and assessed 103 ulcerated limbs in 90 patients. All patients underwent clinical examination, arterial and venous system evaluation, ankle-brachial index determination, and ultrasound of the affected limb. Clinical characteristics included age, gender, race, ulcer duration, time since first episode, history of recurrence, localization of ulcer, ulcer area, eczema, ochre dermatitis, lipodermatosclerosis, pain, body mass index, and medical history data. Risk factors were identified by univariate analysis and estimated odds ratios.ResultsWe assessed 90 patients (103 limbs) with active or healed venous leg ulcers, of whom 84.4% were Caucasian and 68.9% were female. Mean age was 56.0 +/- 13.3 years. Ulcers had remained unhealed for < 1 year in 40.7%. Lipodermatosclerosis, lower limb hyperpigmentation, edema, and eczema were seen in 96.7%, 95.6%, 94.4%, and 51.1% of patients, respectively. Pain was a frequent symptom in 74.4%. Body mass index was assessed in 85 patients: 30.6% were slightly, 36.5% moderately, and 7% severely obese. Patient age > 60 years (odds ratio [OR] 4.0), extensive lipodermatosclerosis (OR 8.7), and previous history of ulceration (OR 19.9) were risk factors for unhealed ulcers. Time since first ulcer episode >= 2 years (OR 29.2) and incompetence of venous systems (OR 1.6) were risk factors for recurrence.ConclusionsLongstanding and large ulcers and recurrences are the main problems encountered by venous ulcer patients. Severe lipodermatosclerosis, previous ulcer history, and time since first ulcer episode >= 2 years are significant risk factors.

O estudo pretende identificar características demográficas e epidemiológicas em homens com câncer de próstata, atendidos no Hospital das Clínicas da Faculdade de Medicina de Botucatu/UNESP, entre 01/01/2000 e 31/01/2003. Foram colhidos dados em 78 de 94 prontuários, observando-se escolaridade, uso de tabaco, estado civil, idade, diagnóstico e sintomatologia. Utilizou-se metodologia quantitativa associada à estatística descritiva. Foram encontrados 50% com 1º grau incompleto, 27,39% não tabagistas, 87% casados, 43% entre 64 e 73 anos. Observou-se que 27,58% foram diagnosticados em 2000, sendo 63,20% aposentados e 20,70% com sintoma de jato urinário fraco. Apenas 20% tinham realizado exames preventivos e, desses, 77,90% possuíam 1º grau incompleto. O estudo foi feito nesse hospital por ser considerado de referência terciária da Direção Regional de Saúde de Botucatu -DIR XI¹. da amostra, 80% procuraram o serviço apresentando sintomas, o que leva a crer que ainda haja falta de orientação da população quanto ao câncer de próstata, principalmente quanto à incidência e prevalência relacionadas à idade, como fator de risco. O estudo evidencia subsídios para um melhor direcionamento da assistência a esses pacientes, permitindo um diagnóstico precoce e conseqüente possibilidade de cura, além da melhora na qualidade de vida.

Introduction: The association of gender with health status (HS) response to long-term oxygen therapy (LTOT) in very severe COPD is unclear. The aims of this study were: (1) to compare dyspnea perception and HS between male and female with very severe COPD at baseline and (2) to provide a prospective assessment of HS response to LTOT, according to gender.Patients and methods: Hypoxemic COPD (n =97, age: 65.5 +/- 9.6 years, 53% males) were enrolled in a prospective longitudinal study over 12 months or until death. St. George's Respiratory Questionnaire (SGRQ) and baseline dyspnea index (BDI) were assessed.Results: At baseline, HS impairment and dyspnea sensation were similar between genders. After 12 months of LTOT, women presented improvement in symptom (64.1 +/- 120.6 versus 40.6 +/- 122.9; P < 0.0001) and total SGRQ scores. Men also showed improvement in symptoms after 12 months (62.7 +/- 23.3 versus 49.6 +/- 22.8; P < 0.0005); however, they presented deterioration of activity, impact and total scores during the study period, with markedly decline of activity domain (68.5 +/- 20.0 versus 75.9 +/- 16.9; P = 0.008). BDI did not show significant difference by gender over the study period.Conclusions: Our results show that the HS course in very severe COPD patients differs according to gender, as females show greater response longitudinally to LTOT. OBJETIVO: estudar os efeitos de técnicas fisioterápicas sobre os desconfortos músculo-esqueléticos na gestação. MÉTODOS: estudo coorte prospectivo, com 71 gestantes nulíparas, de baixo risco, distribuídas conforme a participação (estudo; n=38) ou não (controle; n=33). O Programa Multidisciplinar de Preparo para o Parto e Maternidade constou de 10 encontros (da 18ª à 38ª semana), com atividades educativas, fisioterápicas e de interação. Compararam-se, por questionário específico, no início e final do programa, a ocorrência (presença ou ausência), as características (local, tipo, intensidade, freqüência e duração) e a evolução dos desconfortos músculo-esqueléticos. As médias aferidas na avaliação inicial foram comparadas pela análise de variância (ANOVA) seguida pelo teste do F. Para o estudo entre as proporções, verificadas no início e no final do programa, utilizou-se o teste do c². A significância estatística foi definida pelo limite de 5% (p<0,05). RESULTADOS: no início do programa, 63,6% das gestantes do grupo controle e 84,2% do Estudo relataram sintomas músculo-esqueléticos (p=0,05), caracterizados por dor na região lombossacra. No grupo controle predominaram a intensidade leve (18,2%) e grave (18,4%), e no grupo estudo, a grave (36,8%) e isolada ou associada (31,6%). No final, o grupo controle tinha sintomas de intensidade grave (60,6%), com freqüência diária (42,4%) e duração maior que três horas (69,7%) (p<0,05). O grupo estudo referia intensidade leve (57,9%) e freqüência quinzenal (50,0%) com duração máxima de uma hora (55,3%) (p<0,05). A evolução dos sintomas foi diferenciada, confirmando-se piora em 63,6% das gestantes do Controle e melhora em 65,8% das participantes do Programa (p<0,05). CONCLUSÕES: as técnicas fisioterápicas do Programa Multidisciplinar de Preparo para o Parto e Maternidade se relacionaram à diminuição da intensidade, freqüência e duração e à melhor evolução dos desconfortos músculo-esqueléticos na gestação.

Some patients with obsessive-compulsive disorder (OCD) exhibit an unsatisfactory reduction in symptom severity despite being treated with all the available therapeutic alternatives. The clinical variables associated with treatment-refractoriness in OCD are inconsistently described in the literature.Methods: To investigate factors associated with treatment-reftactoriness of patients with OCD, we conducted a case-control study, comparing 23 patients with treatment-refractory OCD to 26 patients with treatment-responding OCD.Results: the factors associated with refractoriness of OCD were higher severity of symptoms since the onset of OCD (P < 0.001), chronic course (p=0.003), lack of a partner (p=0.037), unemployment (p=0.025), low economic status (p=0.015), presence of obsessive-compulsive symptoms of sexual/religious content (p=0.043), and higher scores on family accommodation (P < 0.001). Only the three latter variables remained significantly associated with treatment-reftactoriness after regression analyses. Limitations: small sample size, the biases and drawbacks inherent to a case-control study, and the inclusion criteria used to define the study groups may have limited the generalisation of the results.Conclusion: A major strength of this study is the systematic and structured evaluation of a vast array of variables related to the clinical expression of OCD, including epigenetic factors and ratings derived from instruments evaluating family accommodation. The presence of sexual/religious symptoms, low economic status and high modification on family function due to OCD were independently associated with, treatment-refractoriness. Future longitudinal studies are warranted to verify if these variables represent predictive factors of treatment non-response. (c) 2006 Elsevier B.V. All rights reserved.

Background: An impairing preoccupation with a nonexistent or slight defect in appearance is the core symptom of body dysmorphic disorder (ODD), a psychiatric condition common in dermatology settings.Objective: We sought to determine the prevalence of ODD in dermatologic patients, comparing general and cosmetic settings, and describing some demographic and clinical characteristics.Methods: In all, 300 patients were consecutively assessed. Screening and diagnoses were performed with validated instruments plus a best estimate diagnosis procedure. The final sample comprised 150 patients in the cosmetic group, 150 patients in the general dermatology group, and 50 control subjects. Standard statistical analyses were performed (chi(2), nonparametric tests, logistic regression).Results: The current prevalence was higher in the cosmetic group (14.0%) compared with general (6.7%) and control (2.0%) groups. No patient had a previous diagnosis. Frequently the reason for seeking dermatologic treatment was not the main ODD preoccupation. Patients with ODD from the cosmetic group were in general unsatisfied with the results of dermatologic treatments.Limitations: Cross-sectional study conducted in a university hospital is a limitation. It is uncertain if the findings can be generalized. Retrospective data regarding previous treatments are not free from bias.Conclusions: BUD is relatively common in a dermatologic setting, especially among patients seeking cosmetic treatments. These patients have some different features compared with general dermatology patients. Dermatologists should be aware of the clinical characteristics of ODD to identify and refer these patients to mental health professionals. (J Am Acad Dermatol 2010;63:235-43.)
