Tempo para acesso ao tratamento do câncer de mama no Distrito Federal, Brasil Central

PURPOSE: To analyze the time between the first symptom and treatment in patients treated for breast cancer in public hospitals in the Federal District. METHODS: This was a cross-sectional analysis. We interviewed 250 women diagnosed with breast cancer treated in six hospitals of the State Department of Health of the Federal District from November 2009 to January 2011. The time intervals studied were the time between the detection of the symptoms and treatment subdivided into intervals until and after the first medical appointment. The variables were: age, menopausal status, color, educational level, average monthly household income, origin, reason for the initial consultation, staging, tumor size, laterality, metastasis to axillary lymph nodes, neoadjuvant chemotherapy, and type of surgery. The Mann-Whtney test was used to assess the association of these variables with the time intervals until treatment. RESULTS: The mean age was 52 years, with a predominance of white women (57.6%), from the Federal District (62.4%), with a family income of up to 2 minimum wages (78%), and up to four years of schooling (52.4%). The staging of the disease ranged from II to IV in 78.8% of the women. The time between the first symptom and treatment was 229 days (median). After detection of the first symptom, 52.9% of the women attended a consultation within 30 days and 88.8% took more than 90 days to start treatment. Women with elementary school education had a greater delay to the start of treatment (p=0.049). CONCLUSIONS: There was a significant delay to start treatment of women with breast cancer in public hospitals of the Federal District, suggesting that efforts should be made to reduce the time needed to schedule medical appointments and to diagnose and treat these patients.

Caracterização do perfil motor de escolares com transtorno autístico

Thematic focus: The motor abnormalities may be part of so-called comorbidities that can coexist with autistic disorder. Objective: To characterize the motor profile of students with autistic disorder. Method: the study included six children with years and 9 months. After signing the consent form by parents or guardians, the students were submitted to the Motor Development Scale for assessment of fine motor, gross motor performance, balance, body scheme, spatial organization, temporal organization and laterality. Results: The results revealed a significant difference between the motor age and chronological age. According to the classification of the Scale of Motor Development, students in this study showed motor development lower than expected for age. Conclusion: The students with autistic disorder in this study presented a profile of Developmental Coordination Disorder in comorbidity, showing that participants of this research presented difficulties in activities that required skills such as handwriting. Thus, motor and psychomotor needs of these students were focused on educational and clinical environment to reduce the impact of behavioral and social manifestations.

Regimes Militares e a Segurança Nacional no Cone Sul

The implantation of military regimes in South America, from the mid-1960s, happened in the context of the Cold War. As a result, both the occurrence of military coups and the plans and actions for the national defense of the Southern Cone States occurred following the logic of that period and the concept of hemispheric defense preached by the United States, with priority in combating the so-called internal subversion. Thus, the need to combat the “internal enemy” has led these countries to participate in hemispheric mechanisms or create his own arrangements for coordination. The article starts from the conflict hypothesis used by the military governments in their military planning to analyze the actions carried out against leftist movements, including coordination between the governments of the Southern Cone. The understanding of the defense concepts against Soviet communism allows us to understand that the groups placed against the military governments were seen as a danger to the State and, in that “state of war”, the means justified the ends.

Relação entre a disparidade de força entre membros inferiores e estabilidade postural em atletas de voleibol

Volleyball is a sport in which the laterality dominant limb shows superior strength and coordination because of its preferential use. Asymmetrical tendencies during the landing after the jump when striking or blocking actions are predominant for most part of game (ARRUDA; EDUARDO, 2008). Adaptations include imbalance of forces in static and dynamic motions at the knee joint, which increases risk for injury. Also, asymmetries in balance control during jumping and landing associate with a general postural instability that can be observed during static balance tasks. The purpose of this study was to investigate relationship between unequal lower limb strength (muscle imbalance) and postural stability levels in volleyball athletes and non-athletes. Nine female volleyball athletes and 10 active non-athletes participated in this study. Four encouters with participants were scheduled: three encounters in the bodybuilding gymnasium to collect anthropometric measures (weight, height for BMI, thigh circumference, which provided an initial diagnosis about asymmetry), and to perform the isometric strength test (i.e., leg press using a load cell and a force transducer to calculate uni an bilateral strength). The last encounter was in the laboratory where a balance test on a force platform was administered under five test conditions, with three repetitions each: baseline (natural standing position), one-leg standing, right side, with full vision (D_CV), and blindfolded (D_SV), one-leg standing, left side, with full vision (E_CV), and blindfolded (E_SV). The stability levels were evaluated using the path length parameters which was based on the total displacement of the center of pressure (DTCP). . Both groups shows asymmetric strength levels between legs, with better performance for the right leg. The hypertrophy of the mandibular coronoid process is a rare condition, progressive and of unknown etiology, characterized by the over development of the coronoid process determining their size increased, resulting in progressive and painful limitation of mouth opening. The mandibular hypomobility is a present symptom in several diseases and associates it to a specific cause requires detailed clinical and imaginologic examination. The cone-beam computed tomography has the potential to provide complementary informations above than conventional radiography. It also has the advantage of eliminate overlaps, provide high resolution image, helping not only in diagnosis, but also in planning the surgical procedure. Here we report a case of male patient, who appeared at the clinic for an evaluation of a trismus. He had no pain, only discomfort for progressive limitation of mouth opening. On physical examination, only a limited mouth opening has been found. Although the movements of laterality and protrusión were posible, they were very limited. Cone-beam computed tomography examination was performed, that showed an image compatible with bilateral hypertrophy of the mandibular coronoid process.
