CONTEXTO: Os efeitos da levodopa na marcha de pacientes com Doença de Parkinson (DP) em terrenos desobstruídos são conhecidos, mas pouco se conhece sobre seus efeitos na marcha com obstáculos. OBJETIVO: Este estudo objetivou descrever, por meio de ferramenta cinemática, o comportamento locomotor de pacientes com DP e verificar as estratégias locomotoras, sem e sob o efeito da levodopa, durante a ultrapassagem de obstáculos de diferentes alturas. MÉTODO: Cinco pacientes com DP (Hoehn e Yahr= 2±0; idade= 68,4±5,7 anos) percorreram, andando, 10m e ultrapassaram um de dois obstáculos (alto= metade da altura do joelho e baixo= altura do tornozelo) posicionado no meio da passarela em duas sessões (em jejum e no pico de ação do medicamento). As seguintes variáveis foram coletadas e analisadas: distância horizontal pé-obstáculo (DHPO), distância vertical pé-obstáculo (DVPO); distância horizontal obstáculo-pé (DHOP) e velocidades médias, horizontais e verticais, nas fases de abordagem e aterrissagem (respectivamente, VHAO,VVAO; VHDO,VVDO). RESULTADOS: A ANOVA, por tentativa, revelou efeito principal de obstáculo para DVPO (F1,49=15,33; p< 0,001), para VVAO (F1,49= 82,184; p< 0,001), para VHDO (F1,49= 15,33; p< 0,001) e para VVDO (F1,49= 31,30; p< 0,001); e efeito principal de medicamento para DVPO (F1,49= 6,66; p< 0,013) e para VVAO (F1,49= 10,174; p< 0,002). CONCLUSÕES: Pacientes foram mais perturbados pelo obstáculo alto. Os sintomas da DP (bradicinesia e hipocinesia) foram diminuídos com o medicamento, evidenciando aumento geral da velocidade da perna de abordagem e da margem de segurança sobre os obstáculos. Pacientes com DP, independente da condição de medicamento, apresentaram um comportamento que garantiu segurança e estabilidade na marcha.

OBJETIVO: O objetivo deste estudo foi analisar as características cinemáticas, cinéticas e eletromiográficas do andar de adultos jovens em piso fixo sem colete e com suporte parcial de peso (SPP) de 0, 10, 20 e 30% do peso corporal. MÉTODO: Oito jovens com idade média de 22,2 anos foram filmados andando sobre uma passarela que continha uma plataforma de força na região central para registro das componentes da força de reação do solo. Marcadores refletivos foram posicionados nos principais pontos anatômicos dos membros inferiores para registro dos dados cinemáticos, e eletrodos de superfície foram afixados nos músculos tibial anterior e gastrocnêmio medial para registro da atividade elétrica muscular. RESULTADOS: Diferenças significantes entre as cinco condições experimentais foram constatadas nas variáveis espaço-temporal, nos ângulos máximos e mínimos da coxa, joelho e tornozelo e nas amplitudes das componentes horizontal ântero-posterior e vertical da força de reação do solo. de forma geral, as maiores mudanças ocorreram na condição de SPP de 30% do peso corporal. CONCLUSÃO: É importante considerar as compensações que ocorrem no padrão do andar com SPP no planejamento das intervenções terapêuticas. Ainda, para melhor definir a utilização dos sistemas de suspensão de peso na reabilitação, estudos futuros precisam ser realizados para verificar o comportamento do andar em populações com alteração de movimento em piso fixo.

This study examined the daily life and most important physical injuries suffered and reported by the dancers of a professional (contemporary) dance company in S (a) over tilde $o Paulo, Brazil. Through an observational, cross,sectional, retrospective procedure using a questionnaire that collected qualitative and quantitative data, we were able to gather information on 30 dancers who collaborated with the survey. We determined that the injuries considered as most important by dancers were those that prevented dance activity during some months. These injuries occurred mainly during rehearsals (which is the activity occuppying, the most time on the schedule). Articular injuries were the most frequent and mainly involved the knee and ankle. They were related to classical technique, in which most of the company's artists started their dance careers. Medical care usually was sought within 1 day, and the prescribed treatment resolved the problem, but the injury cause was not identified in all cases.

A dynamical systems approach to the study of locomotor intralimb coordination in those with hemiparesis led to an examination of the utility of the shank-thigh relative phase (RP) as a collective variable and the identification of potential constraints that may shape this coordination. Eighteen non-disabled individuals formed three groups matched to the age and gender of six participants with chronic right hemiparesis. The three groups differed in the constraints imposed on their walking: (1) walking at their preferred walking speed; (2) walking as slowly as those with hemiparesis; and, (3) walking slowly with a right ankle-foot orthosis (AFO). The results revealed an asymmetry in intralimb coordination between the unaffected and affected leg of those with hemiparesis localized to the latter third of the gait cycle when the limb is advanced from the end of stance to the reestablishment of a new stance. Walking slowly with or without an AFO resulted in no measureable effect in the non-disabled, but accounts for 22% of the variance in the intralimb coordination of the hemiplegic's affected limb and 16% in the unaffected limb. The AFO offered little additional contribution. These results derive from shank-thigh RP that is shown to provide more information about intralimb coordination than knee angle displacement. Objective. - In a pioneer way, we investigated the morbidity of sports injuries referred by judo athletes from São Paulo State Championship.Material and methods. - Data collection from 93 senior judokas in State of São Paulo Judo Championship, through Referred Morbidity Inquiry about last year.Results. - One hundred and ten events were registered with a distribution frequency gradient of sprain > contusion > strain > ligament injury > partial and total dislocation. The most injured body areas were: knee (26.3%), shoulder (21.8%), fingers (17.3%), and ankle (10.0%). The most risky situation was when the athlete was training (standing, applying a blow) and it can be explained by the frequent and unprepared exposition of the athletes. Yearly lesion rate was 1.18 injuries per athlete/year.Conclusion. - Sprains constitute the most common judo injury, and athletes are more susceptible when they apply blows; discussion about the direction that the prevention should be applied remains opened. Purpose. Fatigue has been pointed as a fall risk in the elderly; however, the effects of prolonged gait on neuromuscular recruitment and on its pattern remain unknown. The aim of this study was to evaluate the effects of prolonged gait on neuromuscular recruitment levels and spatial-temporal gait variables. Methods. Eight healthy older women (age: 72.63 ± 6.55 years) walked at their preferred walking speed for twenty minutes on a treadmill. The Root Mean Square (RMS) from the vastus-lateralis, femoral biceps, tibialis anterior and lateral gastrocnemius muscles were determined at the first and last minute of the test during the moments of Heel Strike (HS), Terminal Stance and Terminal Swing (TS). In addition, coactivation in the knee and ankle as well as the stride cadence and length were measured in the test. The two RMS data (taken at the first and last minute) were compared by means of a Student's t-test. Results. Twenty minutes of walking induced fatigue in the subjects, as observed through an increase in RMS, notably during the HS and TS. Coactivation was also influenced by the prolonged gait test. The only gait phase where a risk of falling was enhanced was the HS. Nonetheless, subjects developed strategies to maintain a safe motor pattern, which was evidenced by an increase in stride length and a decrease in stride cadence. Conclusion. Tests lasting just twenty minutes on a treadmill were enough to induce fatigue in older adults. However, the level of fatigue was not enough to present a danger or fall risk to elderly individuals.

Single Limb Stance under visual and proprioceptive disturbances is largely used in clinical settings in order to improve balance in a wide range of functional disabilities. However, the proper role of vision and proprioception in SLS is not completely understood. The objectives of this study were to test the hypotheses that when ankle proprioception is perturbed, the role of vision in postural control increases according to the difficulty of the standing task. And to test the effect of vision during postural adaptation after withdrawal of the somesthetic perturbation during double and single limb stance. Eleven males were submitted to double (DLS) and single limb (SLS) stances under conditions of normal or reduced vision, both with normal and perturbed proprioception. Center of pressure parameters were analyzed across conditions. Vision had a main effect in SLS, whereas proprioception perturbation showed effects only during DLS. Baseline stability was promptly achieved independently of visual input after proprioception reintegration. In conclusion, the role of vision increases in SLS. After proprioception reintegration, vision does not affect postural recovery. Balance training programs must take that into account. © 2011 Elsevier Ltd.

Objective of this study was to analyze the postural balance of neuropathic diabetic individuals through baropodometry, related to losses in the sensorimotor system. Twenty-eight healthy and 25 diagnosed with diabetic neuropathy were subjected to static evaluation (measurement of displacement of body center of pressure) and dynamic (temporal analysis of the stance phase of gait cycle). The tactile sensitivity of the feet was assessed by Semmes Weinstein monofilaments and isometric muscle strength of ankle dynamometry. Analyses of multivariânvia (MANOVAs) and variance (ANOVAs) indicated lower performance in tactile sensitivity, muscle strength and dynamic balance, but showed no difference for static equilibrium of diabetic neuropathy. With this study by regression analysis, one can infer that the equilibrium differences in gait of neuropathic insensitivity may result from tactile and muscle strength. © FTCD/FIP-MOC.

Objective: To determine the nervous activation, muscle strength, and biomechanical parameters that influence the cost of walking in older fallers and non-fallers. Methods: Maximal voluntary isokinetic torque was measured for the hip, knee and ankle of older women. Oxygen consumption was measured at rest and during 8 min of walking at self-selected speed. An additional minute of walking was performed to collect kinematic variables and the electromyographic signal of trunk, hip, knee, and ankle muscles, which was analyzed by the linear envelope. Cost of walking was calculated by subtracting resting body mass-normalized oxygen consumption from walking body mass-normalized oxygen consumption. Stride time and length, and ankle and hip range of motion were calculated from kinematic data. Findings: Older adult fallers had 28% lower knee extensor strength (p = 0.02), 47% lower internal oblique activation at heel contact (p = 0.03), and higher coactivation between tibialis anterior and gastrocnemius lateralis in each of the gait phases (p < 0.05). For fallers, a higher activation of gluteus maximus was associated with a higher cost of walking (r = 0.55, p < 0.05 and r = 0.71, p < 0.01, before and after heel contact, respectively). For non-fallers, an association between cost of walking and age (r = 0.60, p = 0.01) and cost of walking and thigh muscle coactivation (r = 0.53, p = 0.01) existed. Interpretation: This study demonstrated that there may be links between lower-extremity muscle weakness, muscle activation patterns, altered gait, and increased cost of walking in older fallers. © 2013 Elsevier Ltd.
