A 35-year-old African Brazilian patient had sickle cell anemia complicated with recurrent vasoocclusive (VOC) crises and refractory painful leg ulcers for 16 years. The ulcers started over both medial malleoli and expanded gradually. The ulcer on the left leg spread from the foot to the knee circumferentially and was refractory to all forms of therapy within the frame work of multi-disciplinary care. The patient agreed to a below the knee amputation of the left leg. He felt much better after the amputation but developed severe neuropathic phantom pain that was well controlled medically. He could differentiate the sickle cell anemia and ulcer pain from the neuropathic pain. About 6 months after the amputation he had dengue fever with fatal outcome. This is the first report of treatment of refractory sickle cell anemia leg ulcer with amputation and probably the first report of a Brazilian patient with sickle cell anemia and dengue fever.

Background. The chronic obstructive pulmonary disease (COPD) is associated with the strength and resistance decreasing in addition to the dysfunction on autonomic nervous system (ANS). The aerobic training isolated or in association with the resistance training showed evidence of beneficial effects on an autonomic modulation of COPD; however, there are no studies addressing the effect of isolated resistance training.Aims. This study aims at investigating the influence of resistance training on an autonomic modulation through heart rate variability (HRV), functional capacity and muscle strength in individuals with COPD.Design. Clinical series study.Setting. Outpatients.Population. The study involved 13 individuals with COPD.Methods. The experimental protocol was composed by an initial and final evaluation that consisted in autonomic evaluations (HRV), cardiopulmonary functional capacity evaluation (6-minute walk test) and strength evaluation (dynamometry) in addition by the resistance training performed by 24 sessions lasted 60 minutes each one and on a frequency of three times a week. The intensity was determined initially with 60% of one maximum repetition and was progressively increased in each five sessions until 80%.Results. The HRV temporal and spectral indexes analysis demonstrates improvement of autonomic modulation, with significant statistical increases to sympathetic and parasympathetic components of ANS representing by SDNN, LF and HF. In addition, it was observed significant statistical increases to shoulder abduction and. knee flexion strength and functional capacity.Conclusion. The exclusive resistance training performed was able to positively influence the autonomic modulation; in addition it promoted benefits on cardiorespiratory functional capacity and strength benefits in individuals with COPD.Clinical Rehabilitation Impact. This study could contribute to clinical and professionals researchers that act with COPD, even though the resistance component of pulmonary rehabilitation presents consensual benefits on several healthy indicators parameters. There is no evidence about the effects on HRV before. Moreover, this study showed, on clinical practice, the HRV uses as an ANS activity on sinus node evaluation and highlights further importance on scientific context.

Children with cerebral palsy due to movement and posture disorders might have an injured nutritional state. This study's objective was to classify the body mass index of children with cerebral palsy and verify its relation with the injury level of gross motor function. Twenty children, male and female sex, with cerebral palsy with ages between 2 and 14 years participated in the study. They were chosen from a rehabilitation center with physiotherapy, occupational and speech therapy accompaniment. Children were classified according to the gross motor function classification scale. The anthropometric measures of each child weight, heel-knee length, estimated stature and body mass index were gauged. The body mass index was classified according to the Center for Disease Control and Prevention's body mass index percentile calculator for children and teenagers. The obtained data didn't show any relation between the gross motor function and body mass index. The reduced number of participants and the fact of all children be accompanied in dysphagia clinics and, when necessary, nutrition clinics, might have influenced these results.

Introduction: The pulmonary rehabilitation (PR) is composed of aerobic and resisted exercises that improve the functional capacity to the exercise, life quality and decrease respiratory symptoms in subjects with chronic pulmonary disease. Objective: Assess the effects of a combined PR program in the cardiorespiratory function and peripheral muscle strength in subjects with chronic pulmonary disease. Method: Patients with chronic pulmonary disease were submitted to the PR program, which was developed on 24 sessions of 60 minutes (three times per week). The program was composed of aerobic exercises (two times per week) and resisted exercises (once a week). Before and after the PR the patients were submitted to manovacuometry in order to measure the maximum inspiratory pressure (MIP) and the maximum expiratory pressure (MEP), ventilometry, peek expiratory flow (PEF), six minute walking test (6MWT) and one maximum repetition (1RM). The data are presented in absolute frequency, percentage and mean±standard deviation. The t Student test was used to compare data before and after the PR and the ANOVA test to compare before, after and predicted distances in the 6MWT (p<0.05). Results: Seven patients were part of this study, 85.70% of women, 71.40% with pulmonary emphysema diagnosis. The mean age was 69.43±5.59 years old, the height was 1.61±0.07 m, the mean weight was 66.20±8.40 kg and the body mass index mean was 25.50±2.48 kg/m². From the variables assessed, the MEP increased from 79.71±13.69 to 84.42±12.83 cmH2O (p=0,03), the PEF increase from 255.71±66.3 to 320.00±93.63 l/min (p=0,03) and the distance in the 6MWT from 415.28±47.90 to 483,79±79,77 m (p=0,02). The load in the 1RM test in the reverse peck deck exercise (before - - 17.10±8.10kg; after – 210.40±9.00kg), knee in leg extension machine (before – 17.10±9.50kg; after – 26.40±13.10kg) and hip extensors (right before – 48.60±22.10kg; after – 62.90±19.30kg; and left before – 46.40±20.10kg; after – 62.10±18.20kg) increased significantly (p<0,05). Conclusion: After the PR program there was improvement in the expiratory muscular strength, in the lower limbs strength and in the functional capacity. Besides that, there was a reduction in the airflow obstruction of the subjects with chronic pulmonary disease.

Background: Coactivation may be both desirable (injury prevention) or undesirable (strength measurement). In this context, different styles of muscle strength stimulus have being investigated. In this study we evaluated the effects of verbal and visual stimulation on rectus femoris and biceps femoris muscles contraction during isometric and concentric. Methods: We investigated 13 men (age =23.1 ± 3.8 years old; body mass =75.6 ± 9.1 kg; height =1.8 ± 0.07 m). We used the isokinetic dynamometer BIODEX device and an electromyographic (EMG) system. We evaluated the maximum isometric and isokinetic knee extension and flexion at 60°/s. The following conditions were evaluated: without visual nor verbal command (control); verbal command; visual command and; verbal and visual command. In relation to the concentric contraction, the volunteers performed five reciprocal and continuous contractions at 60°/s. With respect to isometric contractions it was made three contractions of five seconds for flexion and extension in a period of one minute. Results: We found that the peak torque during isometric flexion was higher in the subjects in the VVC condition (p > 0.05). In relation to muscle coactivation, the subjects presented higher values at the control condition (p > 0.05). Conclusion: We suggest that this type of stimulus is effective for the lower limbs.

OBJECTIVES: Treatments for injured articular cartilage have not advanced to the point that efficient regeneration is possible. However, there has been an increase in the use of platelet-rich plasma for the treatment of several orthopedic disorders, including chondral injuries. Our hypothesis is that the treatment of chondral injuries with platelet gel results in higher-quality repair tissue after 180 days compared with chondral injuries not treated with gel.METHODS: A controlled experimental laboratory study was performed on 30 male rabbits to evaluate osteochondral injury repair after treatment with or without platelet gel. Osteochondral injuries were surgically induced in both knees of each rabbit at the medial femoral condyle. The left knee injury was filled with the platelet gel, and the right knee was not treated. Microscopic analysis of both knee samples was performed after 180 days using a histological grading scale.RESULTS: The only histological evaluation criterion that was not significantly different between treatments was metachromasia. The group that was treated with platelet gel exhibited superior results in all other criteria (cell morphology, surface regularity, chondral thickness and repair tissue integration) and in the total score.CONCLUSION: The repair tissue was histologically superior after 180 days in the study group treated with platelet gel compared with the group of untreated injuries.

Individuals with spastic cerebral palsy show muscle weakness, difficulties in the control of agonist and antagonist muscles, decreased range of motion and tonus and sensibility alterations, especially in knee joint. These problems can interfere on the performance of functional activities such gait. The aim of this study was to analyze the contribution of knee range of motion on gait of hemiplegic and diplegic children considering their asymmetries. Twelve children, 6 hemiplegics e 6 diplegics from 7 to 12 years of age (age average= 9,5 ± 1,93) took part. Spasticity was assessed by the Ashworth’s Modified Scale and the passive knee range of motion by an eletrogoniometer. The task was to walk on a walkway of 8m long, in their preferred speed, in 6 attempts, been 3 on right and 3 on left sagital planes. Eigth passive markers were bilaterally fixed for the kinematic record. Orthogonally to the walkway, two digital camcorders were assembled on the sagital plane. The fotogrametric procedures were performed by the Dvideow 6.3 software. The Matlab 7.0.1 software was used to filter and to calculate the dependent variables. The U test of Mann- Whitney found differences to the cerebral palsy type for knee extension/hiperextension (U = - 2.943; p= 0.003), knee relative angle at heel contact (U = - 5.992; p= 0.001) and knee range during stride (U = - 4.099; p= 0.001). The Wilcoxon’s test revealed differences according to the asymmetries for the hemiplegics only for the knee relative angle at heel contact (T= - 2.635; p<0.008). The contributions of passive knee range of motion, revealed by the Spearman correlations, for the more afected limb of the diplegics, showed that the knee extension/hiperextension interfere on the cadence, stride duration and step width; the knee relative angle at heel contact change the stride length and duration and cadence; and the...(Complete abstract click electronic access below)

This experimental study aimed the evaluation of the use of anthropomorphic phantoms in the analysis of the influence of the acquisition parameters in the contrast of radiographic images. The analyzed factors were the variation of the peak kilovoltage (kVp) and the product of the filament circuit by the time of exposition (mAs). The influence of these factors was verified for different anthropomorphic phantoms: foot, knee and chest. The image contrast behavior with the simulators was compared to values obtained with the use of an aluminum ladder being the behavior of this reference for analysis and discussion. To assure the reproducibility of images, quality control tests were made and evaluation of procedure conditions before the experiments. The results obtained are shown in a scale of images where it was possible to evaluate the impact in the darkness of images. Regions with different compositions were analyzed which were different in image, this way the values of optical density and contrast are represented as charts and graphics. We conclude that the use of anthropomorphic phantoms in the evaluation of the influence of tension of the tube and time of exposition in the contrast of the radiographic images is not indicated for a quantitative analysis through optical density, once they present incompatible results with the data as reference as the aluminum ladder even so these simulators present a great property in qualitative analysis regarding the differentiation of structures and subjective evaluations

Large motor dysfunctions are observed in older adults with the age advance. Parkinson’s disease (PD) patients have motor deficits to perform daily living activities. To raise from a chair, a daily task necessary to live independently, requires both large muscle recruitment and large joint range of motion to achieve the vertical position safely. Normally, we initiate gait after raise from a chair. The aim of this study was to analyze the PD patients’ behavior when rising from a chair and initiating gait and to compare it according to the age advance. In order to do that, 23 PD patients (66.61±7.64 years old) were distributed in three age groups: Young group, between 51 and 60 years of age (n=7); intermediary group, between 61 and 70 years of age (n=7); and elderly group, over 70 years of age (n=9). There were no statistical differences among groups either for the disease evolution stage or for it compromising. The task was to stand from a chair and to initiate gait forward in three attempts. The dependent variables were: spatial and temporal (first step length and duration, and stride length, duration and velocity) and angular (flexion and extension of head, shoulder, hip, knee, and ankle). The motion of standing from a chair was divided in two phases. The data was statistically treated by means of Analyses of Variance with group as the only factor. The Scheffé’s post hoc test was used to localize differences among groups and the significance level was adjusted to p≤0.017. Several studies have shown that different stretching routines can lead to decreases on acute neuromuscular system performance. Although the deficit in muscle strength mediated by different methods of stretching has been systematically observed, few studies have investigated the possible existence of a dose-response relationship between the amount of stretching and muscle strength deficit in older adults. In this context, the objective of this study was to investigate the acute effect of two different stretching volumes on isometric force-time curve (Cf-t) in elderly women. The study included 13 older women (64.08 ± 4.27 years, 69.98 ± 10.56 kg, 157.90 ± 8.66 cm, 28.25 ± 4.22 kg/m²). The participants visited the laboratory for five consecutive days, among which the first two were used for familiarization. During the other three days the participants underwent experimental conditions: control (C) stretch 30 seconds (AE30) and stretch 60 seconds (AE60). For the AE30 and AE60 conditions, three series of passive static stretching were performed, with duration of 30 and 60 seconds, respectively. The experimental conditions were performed with an interval of at least 24 hours between them and the order of execution was randomized. The recording of isometric Cf-t of the knee extensor muscles was performed in extensor chair connected to a force transducer. Measurements were recorded immediately after each experimental condition, for five seconds. For statistical analysis, descriptive procedures were used and ANOVA one way to check possible changes on the Maximal Voluntary Contraction (CVM) and Peak Rate of Force Development (TDFP) among the three conditions (p <0.05). The ANOVA showed no statistically significant difference for CVM and TDFP, between the three conditions. Throughout the history of medicine surgeons realized the importance of the patella to the functioning of the knee. The main function of the patella is to increase the mechanical efficiency of the quadriceps tendon and knee extensor mechanism. It was found that 50% to 80% of the fractures without deviation of the patella have the transversal pattern, possibly due to excessive tensile forces applied to the extensor mechanism. The purpose of this study is to analyze the loads to which a patella is submitted during a normal extension movement of knee. This analysis will be done by modeling a 3D patella and subsequent load simulation as, described in medical literature, using the finite element method

The objective of this study was to investigate the influence of previous active static stretch on the isometric peak torque (PT) and rate of force development (RFD) measured from different time intervals from the beginning of muscle contraction. Participated of this study 15 male individuals, apparently healthy, with ages between 18 and 25 years, without regular physical activity practice. The individuals were submitted in different days to the following tests: 1) Familiarization session to the isokinetic dynamometer; 2) Two maximal isometric concentric contractions for knee extensors in isokinetic dynamometer to determine PT and RFD (Control), and; 3) Two active static stretching exercises for the dominant leg extensors (10 x 30 s for each exercise, with 20 s of rest). After the stretching, the isokinetic test was repeated (Post-Stretching). The conditions 2 and 3 were performed in random order. The RFD was considered as the mean slope of the moment-time curve at time intervals of 0-30, 0-50 and 0-100ms relative to the beginning of muscle contraction. It was verified significant reduction for both maximal RFD and PT after the stretching (p < 0.05). At intervals of 0- 30ms, 0-50ms and 0-100ms, the RFD at the conditions with stretching was similar to the RFD without stretching (p > 0.05). At intervals of 0-150ms and 0-200ms, the RFD obtained at the contraction without stretching was significantly higher that that obtained at the contraction with stretching (p < 0.05). It can be concluded that the static stretching, performed with duration of 600 s diminish isometric PT, maximal RFD and RFD measured at late phase (> 100 ms) of muscle contraction.
