The use of multimodal neuroimaging techniques has been helpful in the investigation of epileptogenic zone in patients with refractory epilepsies. This work aims to describe an ictal event during EEG-fMRI performed simultaneously in a 39-year-old man with refractory epilepsy. The EEG data were recorded at a sampling rate of 5 kHz, using a BrainAmp (BrainProducts, München, Germany) amplifier, with 64 MR (magnetic resonance) compatible Ag/AgCl electrodes. MR images were acquired using a 3T scanner in 3 sequences of 6 minutes of echo-planar images (EPIs), with TR = 2s, being the last sequence stopped after the ictal event. The EEG was corrected for gradient and pulse artifacts using the Brain Vision Analyzer2 software (BrainProducts), and the functional images were realigned, slice-timing corrected, normalized and smoothed. The start of the ictal changes was used for the evaluation of the BOLD response in MR images, using a t-test with a minimum cluster of 5 voxels, p <0.005 (T>2.5). The patient had a partial complex seizure, as noted by neurologist. The fMRI data showed positive BOLD responses (activation) in dysplastic areas, but showed the most significant activation outside the lesion, in areas compatible with secondary spread of the epileptic focus, probably caused by motor reaction also observed during the seizure. As a conclusion, we note that the technique of EEG-fMRI can detect the epileptogenic zone in patients with refractory epilepsy, but areas of dissemination of primary epileptogenic focus may show significant activation, introducing additional difficulties to the interpretation of the results

Seizure is a clinical manifestation of abnormal neuronal hyperactivity of the cerebral cortex, presentation clinical varies according to the location and extent of the brain. The classic seizure begins with an inicial period, named prodrome, followed by the aura, the ictus and finally the post-ictus phase. Seizures can be generalized or focal/partial types. Focal seizures with secondary generalization are the most common type in dogs. The seizure can be divided etiologically in intracraneal (idiopathic epilepsy; sintomatic; probably sintomatic epilepsy) and extracraneal disorders. After determining the etiology of seizures and critical analysis of the frequency, severity, and worry they cause to the owner, you should make the decision whether to launch an anticonvulsant drug therapy. Although the ideal is the elimination of seizures, the real goal should be to decrease the frequency and intensity of crises, without affecting the quality of life of patients taking the drugs used side effects acceptable. The veterinary should consider the bioavailability of the drug, contraindications, toxicity, drug interactions, the amount of daily administration, the availability of the owner and the cost of therapy. Seizures can be controlled in about 70- 80% of the dogs and in the majority of the cats when treated with Phenobarbital alone. Addition of a second drug (usually potassium bromide), decreases the number of seizures in more that 50% in about 70-80% of dogs. However, approximately 20-30% of dogs are refractory to this treatment. In these cases, it will be necessary to think in other pharmacological alternatives, like felbamate, gabapentin, levetiracetam, zonisamid, among others. It has been concluded that... This paper concerns about the durability of new material in construction. It is noteworthy the fact that increases increasingly searching for alternative materials that do not depend only of natural resources and at the same time be an alternative for reuse of industrial waste. Since the construction materials have a long life and a high cost of civil works and maintenance, it is crucial to estimate the behavior of a new product. Thus , this work discuss the durability of mixed mortar lining , made with waste from the process Kraft pulp production , known as dregs and grits , in partial replacement of sand. Tests were conducted to simulate conditions as adverse environments of constant heat and fire, with the aim of analyzing the behavior of mortar mixed matched the behavior of standard mortar

Photodynamic Therapy (PDT) is a therapeutic method which employs a photosensitizer and light to cause cellular death. The chemical compounds have low or none toxicity for hosts cells. Under the incidence of light, in an appropriated wavelength, these chemical compounds produce reactive oxygen which affects the biomolecules of the target-cells. The specific illumination of the affected area increases the selectivity of the therapy, since the photodynamic process occurs only in the irradiated area. Pythiosis, for instance, is a life-threatening emerging disease caused by a fungus-like organism called Pythium insidiosum. The disease occurs in man and other animals, being mostly observed in horses. Human pythiosis may present as ophthalmic, cutaneous-subcutaneous and systemic forms of lesions. Due to the fact that P. insidiosum is not a true fungus, it is refractory to most antifungal drugs and the treatment of the disease is difficult. Extensive surgery procedures, such as limb amputation, are the treatment of choice, however relapses may occur frequently. Although not totally effective, the use of immunotherapy associated to surgery have shown some results. Considering that pythiosis is an emerging disease few explored in its etiological and therapeutic aspects, which are limited and few effective, it is of great importance to encourage the development of researches for new strategies of treatment. In this sense, it was evaluated the effect of PDT on in vitro growth of the pathogen employing two chemical compounds as photosensitizer, porphyrin and chlorine, at different concentrations in combination with several energetic dosages. Porphyrin showed inhibition of growth at 25mg/mL with 100J/cm2 of energetic dosage and chlorine showed similar results employing low concentrations (0,7, 1,0 and 1,3mg/mL) with 70J/cm2 of energetic dosage... Prolonged intermittent renal replacement therapy (PIRRT) has emerged as an alternative to continuous renal replacement therapy in the management of acute kidney injury (AKI) patients. This trial aimed to compare the dialysis complications occurring during different durations of PIRRT sessions in critically ill AKI patients. We included patients older than 18 years with AKI associated with sepsis admitted to the intensive care unit and using noradrenaline doses ranging from 0.3 to 0.7 mu g/kg/min. Patients were divided into two groups randomly: in G1, 6-h sessions were performed, and in G2, 10-h sessions were performed. Seventy-five patients were treated with 195 PIRRT sessions for 18 consecutive months. The prevalence of hypotension, filter clotting, hypokalemia, and hypophosphatemia was 82.6, 25.3, 20, and 10.6%, respectively. G1 was composed of 38 patients treated with 100 sessions, whereas G2 consisted of 37 patients treated with 95 sessions. G1 and G2 were similar in male predominance (65.7 vs. 75.6%, P=0.34), age (63.6 +/- 14 vs. 59.9 +/- 15.5 years, P=0.28) and Sequential Organ Failure Assessment score (SOFA; 13.1 +/- 2.4 vs. 14.2 +/- 3.0, P=0.2). There was no significant difference between the two groups in hypotension (81.5 vs. 83.7%, P=0.8), filter clotting (23.6 vs. 27%, P=0.73), hypokalemia (13.1 vs. 8.1%, P=0.71), and hypophosphatemia (18.4 vs. 21.6%, P=0.72). However, the group treated with sessions of 10h were refractory to clinical measures for hypotension, and dialysis sessions were interrupted more often (9.5 vs. 30.1%, P=0.03). Metabolic control and fluid balance were similar between G1 and G2 (blood urea nitrogen [BUN]: 81 +/- 30 vs. 73 +/- 33mg/dL, P=1.0; delivered Kt/V: 1.09 +/- 0.24 vs. 1.26 +/- 0.26, P=0.09; actual ultrafiltration: 1731 +/- 818 vs. 2332 +/- 947mL, P=0.13) and fluid balance (-731 +/- 125 vs. -652 +/- 141mL, respectively) . In conclusion, intradialysis hypotension was common in AKI patients treated with PIRRT. There was no difference in the prevalence of dialysis complications in patients undergoing different durations of PIRRT.

Introduction: Inflammatory bowel disease (IBD) consists of Crohn's disease, ulcerative colitis and an unspecific IBD. The unclear etiology of IBD is a limiting factor that complicates the development of new pharmacological treatments and explains the high frequency of refractory patients to current drugs, including both conventional and biological therapies. In view of this, recent progress on the development of novel patented products to treat IBD was reviewed.Areas covered: Evaluation of the patent literature during the period 2013 - 2014 focused on chemical compounds, functional foods and biological therapy useful for the treatment of IBD.Expert opinion: Majority of the patents are not conclusive because they were based on data from unspecific methods not related to intestinal inflammation and, when related to IBD models, few biochemical and molecular evaluations that could be corroborating their use in human IBD were presented. On the other hand, methods and strategies using new formulations of conventional drugs, guanylyl cyclase C peptide agonists, compounds that influence anti-adhesion molecules, mAbs anti-type I interferons and anti-integrin, oligonucleotide antisense Smad7, growth factor neuregulin 4 and functional foods, particularly fermented wheat germ with Saccharomyces cerevisiae, are promising products for use in the very near future.

Carbon fiber reinforced carbon composites can be made by iterative liquid impregnation or gas phase carbon deposition routes. In both cases, at the final processing stage the carbon fiber is embedded in carbon matrix which results in unique properties such as low density, high thermal conductivity and thermal shock resistance, low thermal expansion and high modulus, in relation to other refractory materials. In the present study assembled three-directional and four-directional preforms, having 50% volume of pores, were densified by iterative cycles of thermoset resin impregnation followed by pyrolysis under inert atmosphere, until appropriate densities were achieved. The thermoset resin is converted in a carbon matrix during pyrolysis. The iterative manufacturing process of the carbon fiber reinforced carbon composites is evaluated by means of nondestructive techniques based on X-ray computed tomography and electrical resistivity. X-ray computed tomography gives a general mapping view of the filling pores of the preforms which impacts results of the electrical resistivity. After six processing cycles and heat treatments up to 2000?, the final densities of the three-directional and four-directional carbon fiber reinforced carbon composites were 1.16g/cm(3) and an electrical resistivity of approximate to 0.07m. The configuration of preforms, three-directional or four-directional, did not alter the densification profile, in terms of increasing density and reducing porosity during the processing cycles.

Automotive heat shields are usually composed of two metal sheets enclosing an insulating material with a paper-like texture that contains refractory ceramic particles. This article discusses the results achieved by recycling the scrap automotive insulation that is discarded in landfills, using the same concept as paper recycling. For comparison with the original product, tests of thickness, bulk density, weight loss on ignition, tensile strength, compressibility, and recovery were performed on recycled materials produced in a so-called "manual" process (involving little automation and performed in adapted facilities) without pressing, and pressed once, twice, and four times. Materials recycled in a so-called "industrial" process (in a paper recycling plant) without pressing, and pressed once were also tested. The recycled materials can be considered approved with respect to the main requirement, thermal insulation, since they dissipated the under-hood temperature by more than 300 A degrees C (like the original product). Like the heat insulation tests, the thermogravimetric analysis suggested that the recycled materials showed higher stability than the original product. Thermogravimetric, microscopy, and energy dispersive spectroscopy analyses indicated that the structural and compositional characteristics of the original product were preserved after recycling.

Ependymomas are glial tumors derived from ependymal cells lining the ventricles and the central canal of the spinal cord. It may occur outside the ventricular structures, representing the extraventicular form, or without any relationship of ventricular system, called ectopic ependymona. Less than fifteen cases of ectopic ependymomas were reported and less than five were anaplastic. We report a rare case of pure cortical ectopic anaplastic ependymoma.
