The reconstruction of bilateral osteoradionecrosis (ORN) of mandibular defects using a single free bone flap is rarely performed because extensively radiated neck tissue with severe fibrosis is usually unsuitable for vascularized reconstruction. Thirty-one patients with nasopharyngeal carcinoma (NPC) underwent bilateral reconstruction of advanced ORN in the mandible using a single fibular osteocutaneous flap. Clinical factors associated with the operation were assessed, including classification of mandible defects, types of recipient vessels, perioperative complications, and postoperative outcomes. All of the fibular osteocutaneous flaps survived completely, with the exception of 1 inner skin paddle that presented partial necrosis in a reconstruction of through-and-through defects. All patients experienced an improvement in cosmetic results 6 months after the reconstruction, whereas 23 patients experienced improved mouth opening compared to the preoperative condition. Advanced bilateral ORN in patients with NPC could be synchronously reconstructed with a single fibular osteocutaneous flap. © 2015 Wiley Periodicals, Inc. Head Neck, 2015.

High-frequency ultrasound is a non-invasive tool used in skin ageing research to assess dermis thickness and echogenicity. This study evaluated the reliability of a range of high-frequency ultrasound parameters and tested their correlation with age and a validated clinical scale for the assessment of forearm skin photoageing; the difference between two body sites according to environmental exposition patterns was also investigated. Twenty-three volunteers aged 28-82 years were divided into three groups according to forearm photoageing degree. A 20 MHz ultrasound unit was used to obtain cross-sectional images of the skin by two trained investigators on two different sites: the dorsal forearm (chronically photoexposed skin) and the proximal medial arm (non-photoexposed skin). Several echogenicity parameters were studied for each skin compartment: total dermis, upper dermis and lower dermis, and the ratio between upper and lower dermis. The intraclass correlation coefficient (for complete agreement) between investigators was higher for upper and total dermis echogenicity measures compared with the lower dermis. At the non-photoexposed site, the upper and lower dermis parameter ratio was better correlated with age. At the photoexposed area, total dermis parameters demonstrated higher correlations with clinical score. The authors discuss the choice of parameters for forearm photoageing assessment using high-frequency ultrasound.

The zygomatic-maxillary complex due to its projection framed as one of the areas hardest hit by injuries, as well as the nasal bones. Component important in this context, the zygomatic arch fracture is under the direct action of forces due to its structure fragile, resulting in loss of normal convex curvature. Therefore, it is aimed to report a clinical case of male patient, who had leucoderma zygomatic bone fracture using access transcutaneous and intra-oral fracture reduction body of zygoma and zygomatic arch. In addition to evidence combination of closed and open techniques for solving the case. The technique provided the patient excellent cosmetic and functional results.

Dental changes that compromise facial harmony can have serious consequences on the social image of the patient. Depending on the emotional significance for the individual it can change interpersonal relationships, causing profound changes in their patterns of self-acceptance and self-image, with profound effects on their self-esteem of the patient. The search for tooth whitening as cosmetic factor for a harmonic smile grew sharply in the last decade. Although the color of the tooth represents only one aspect in the set of determinants of facial harmony, it is a strong factor because it is quickly perceived. This study aims to report the clinical case of a patient with spots on their upper central incisors. For the patient these spots have compromised her smile, causing harm and discomfort in their everyday social and professional relationships.

Enamel microabrasion can eliminate enamel irregularities and discoloration defects, improving the appearance of teeth. This article presents the latest treatment protocol of enamel microabrasion to remove stains on the enamel surface. It has been verified that teeth submitted to microabrasion acquire a yellowish color because of the thinness of the remaining enamel, revealing the color of dentinal tissue to a greater degree. In these clinical conditions, correction of the color pattern of these teeth can be obtained with a considerable margin of clinical success using products containing carbamide peroxide in custom trays. Thus, patients can benefit from combined enamel microabrasion/tooth bleaching therapy, which yields attractive cosmetic results. Esthetics plays an important role in contemporary dentistry, especially because the media emphasizes beauty and health. Currently, in many countries, a smile is considered beautiful if it imitates a natural appearance, with clear, well-aligned teeth and defined anatomical shapes.1-3 Enamel microabrasion is one technique that can be used to correct discolored enamel. This technique has been elucidated and strongly advocated by Croll and Cavanaugh since 1986,4 and by other investigators1,2,5-13 who suggested mechanical removal of enamel stains using acidic substances in conjunction with abrasive agents. Enamel microabrasion is indicated to remove intrinsic stains of any color and of hard texture, and is contraindicated for extrinsic stains, dentinal stains, for patients with deficient labial seals, and in cases where there is no possibility to place a rubber dam adequately during the microabrasion procedure.1,2 It should be emphasized that enamel microabrasion causes a microreduction on the enamel surface,3,6,10 and, in some cases, teeth submitted to microabrasion may appear a darker or yellowish color because the thin remaining enamel surface can reveal some of the dentinal tissue color. In these situations, according to Haywood and Heymann in 1989,14 correction of the color pattern of teeth can be obtained through the use of whitening products containing carbamide peroxide in custom trays. A healthy and attractive smile is very valued in today s society. Yet this requires better aesthetic and cosmetic solutions from dental surgeons.The present paper presents a case report of diastema closure using an association of tooth movement induced by orthodontic elastics,dental cosmetics and resin-bonded prosthesis. The female patient L.B.A.J., aged 19 years, visited the Integrated Clinic of Araçatuba DentalSchool UNESP mainly complaining of missing teeth and poor esthetics. After anamnesis, clinical and radiographic examination revealedhypodontia of the maxillary right and left first premolars, abnormal maxillary right lateral incisor shape and diastema between the maxillaryright lateral incisor and canine and between the maxillary left lateral incisor and canine. Planning was done which included indication ofdiastema closure with composite resin restoration and metal-free resin-bonded prosthesis, with a structure made of Vectris (Ivoclar VivadentInc., Amherst, NT, USA). The option for dental cosmetics associated with resin-bonded prosthesis after movement with orthodontic elasticsresulted in an excellent aesthetic outcome of low cost and short treatment duration.Indexing terms: composite resins; diastema; fixed resin bonded partial denture.

The skin aging is a matter of discomfort verified in the population. Thus, every day, new products are launched on the market to offer different manners to prevent the premature aging of the skin. In this context, active substances, as alpha and beta hidroxyacids (AHA/BHA), beyond the sunscreens, are considered a way of prevention and amelioration of the effects caused in the skin due to the time. The aim of this study was to develop and evaluate a cosmetic cream containing AHA/BHA and sunscreen. It was studied in relation to its physical-chemical and microbiological characteristics. According to the results, the formulation developed present a shelf life of 758 days and the preservative system was effective. Considering the parameters evaluated, the cream probably would be commercially accepted.

The use of medicinal plants occurs since the Antiquity, but has become more popular in the present time. In the United States, 50% of the population uses the phytotherapy. Calendula officinalis and Melampodium divaricatum have components called flavonoids. Matricaria chamomila Linn and Acchilea millefolium, popularly known as camomila and mil folhas, respectively, have sesquiterpenic components called azulens. These substances present many effects; some of them offer benefits to the human health. Thus, these plants had their extract incorporated in cosmetic-dermatological formulations. This study aimed to prove the presence of the active substances, sesquiterpenes and flavonoids, in cosmetic creams with Matricaria chamomila Linn, Acchilea millefolium, Calendula officinalis and Melampodium divaricatum extracts. Also, aimed to evaluate the microbiological quality of the products. Flavonoids and sesquiterpernes were detected in the emulsions and the microbiological quality was verified. Thus, the products are safe to the users in relation to the microbiological aspects and should present beneficial effects due to the presence of flavonoids and sesquiterpenes.
