Patients who went through a Stroke may require mechanical ventilation (MV) in the acute phase of the disease and, on MV, they may show alterations in the respiratory mechanic. Physiotherapy techniques are applied in patients on MV to improve their respiratory mechanic. Thus, the purpose of this study was to evaluate the effects of the Respiratory Physiotherapy into the dynamic compliance (Cdyn), static compliance (Cst) and airway resistance (Rwa) in Stroke patients on MV. In order to do that, patients with Stroke diagnostic on MV were evaluated, before and after the use of Physiotherapy techniques (manual rib-cage compression, thoracic decompression, zero end expiratory pressure and suctioning). Cdyn, Cst and Rwa were the researched variables. Statistical Analysis was made using Paired T-Test with statistical significance with p-values no greater than 5%. Eleven patients were part of the study, with an average of 64,6±12,5 years old. Cdyn increased after physiotherapy from 29,3±16,1 ml/cmH2O to 33,8±16,7 ml/cmH2O (p=0,03). Cst also increased from 44,4±20,7 ml/cmH2O to 54,0±26,6 ml/cmH2O (p=0,024). No significant difference was detected for Rwa between the before and after moments (Before – 8,0±3,2 cmH2O/l/s, After – 7,3±2,25 cmH2O/l/s; p=0,45). It was possible to conclude that both dynamic and static compliance increased after physiotherapy, but the airway resistance did not increase in patients with stroke on mechanical ventilation.

Congenital toxoplasmosis is a serious public health case, for it causes irreversible damage to the embryo/fetus, which may cause its death. The identification and the care to pregnant women with suspect acute toxoplasmosis (IgM+) is performed in prenatal monitoring. This study aimed to measure the incidence and know the profile of positive pregnant women for toxoplasmosis in a Basic Health Unit (BHU) in Matão - SP, between the years 2011 to 2013. This is a retrospective descriptive study, from the medical records of pregnant women attended. The project was approved by the Municipal Departament of Health and BHU was chosen along the Municipal Epidemiological Surveillance. From 2011 to 2013, 189 women began prenatal care in the unit, an annual median of 71(±26.91), of which 17 (8.99%) were positive for the serological test indicative of acute phase (IgM+). The distribution over the trial period was: four cases in 2011, twelve cases in 2012 and one in 2013. Pregnant women IgM positive for toxoplasmosis attend by BHU were: age 24(±5.47) years; color: equally distributed among white, black and brown; as the number of pregnancies: multiparous (2±0.97), most of them with a cesarean delivery as obstetric history and possessed no other risk factors associated with pregnancy (94.12%); gave entrance at BHU with 13.65(±7.35) weeks of gestation and had a median of 5(±2.36) consultation on their prenatal care. The examination for toxoplasmosis was requested as recommended by the Health Ministry (HM) and the medication prescribed was Roxamicina® - spiramycin, as soon as the test results (IgM+) arrived. Of the 17 pregnant women, only 10 completed the pre-natal at BHU - attendance at the postpartum consult (58.8%). Of these, the deliveries were vaginal (55.55%), made preterm with 36.5 weeks of gestation at the Municipal Hospital. Canine ehrlichiosis is caused by the bacterium Ehrlichia canis and is characterized by a systemic febrile disease of unknown pathogenesis. This study evaluated the expression of cytokines TNF-alpha, IL-10, IFN-gamma, in splenic cells and blood leukocytes during the acute phase of ehrlichiosis and after treatment with doxycycline hyclate in dogs experimentally infected with the E. canis Jaboticabal strain. The study results showed a significant expression of TNF-alpha 18 days post-inoculation, reducing by approximately 70% after treatment. There was a unique peak of expression of IL-10 and IFN-gamma 18 and 30 days post-inoculation, respectively. This study suggests that TNF-alpha plays a role in the pathogenesis of the acute phase of canine ehrlichiosis and that treatment with doxycycline hyclate reduces the systemic effects of this cytokine, possibly by reducing or eliminating parasitemia.

To establish guidelines based on scientific evidence for the management of familial Mediterranean fever. The Guideline was prepared from 5 clinical questions that were structured through PICO (Patient, Intervention or indicator, Comparison and Outcome), to search in key primary scientific information databases. After defining the potential studies to support the recommendations, these were graduated considering their strength of evidence and grade of recommendation. 10,341 articles were retrieved and evaluated by title and abstract; from these, 46 articles were selected to support the recommendations. 1. The diagnosis of FMF is based on clinical manifestations, characterized by recurrent febrile episodes associated with abdominal pain, chest or arthritis of large joints; 2. FMF is a genetic disease presenting an autosomal recessive trait, caused by mutation in the MEFV gene; 3. Laboratory tests are not specific, demonstrating high serum levels of inflammatory proteins in the acute phase of the disease, but also often showing high levels even between attacks. SAA serum levels may be especially useful in monitoring the effectiveness of treatment; 4. The therapy of choice is colchicine; this drug has proven effectiveness in preventing acute inflammatory episodes and progression towards amyloidosis in adults; 5. Based on the available information, the use of biological drugs appears to be an alternative for patients with FMF who do not respond or are intolerant to therapy with colchicine.
