BACKGROUND: It is not clear what the population's demand for dermatologists is, nor how many professionals are needed in order to provide adequate care in this area of expertise. Knowledge of the flow of patients at dermatological clinics throughout the country allows for the formation of expansion and distribution policies regarding professionals, and provides backing for the decision to increase medical residency places. OBJECTIVES: To evaluate the time it takes to schedule a private dermatological consultation in Brazil, and other factors concerning consultations. METHODS: Survey with a random sample of 14% of Brazilian dermatologists, simulating the scheduling of emergency clinical and cosmetic consultations, and botulinum toxin procedures. Also, details relating to cost and professionals, were studied. Data were adjusted for each region of the country. RESULTS: A total of 873 dermatologists were evaluated. Full SBD members represented 85%, and 66% were women. The median time to schedule a consultation ranged from 6 (out-of-pocket payment) to 7 (medical insurance) consecutive working days. Times varied depending on the region. A multivariate analysis showed that out-of-pocket consultations and procedures were scheduled sooner than with medical insurance, regardless of whether they were clinical or cosmetic. CONCLUSION: The characteristics of dermatologists are varied throughout regions of the country. Private consultations and procedures are scheduled sooner than with insurance companies.

P>A 36-day trial was conducted to determine the effects of repetitive periods of food restriction and refeeding on growth and energy metabolism in pacu (Piaractus mesopotamicus). A total 264 juvenile fish (36.9 +/- 2.8 g) were fed with the experimental diet for 36 days using three regimes: (i) feeding daily to satiation (FD); (ii) no feed for 3 days, then feeding the same amount offered to the control groups for the next 3 days (NF/R controlled); and (iii) no feed for 3 days, then feeding to apparent satiation for the next 3 days (NF/R at satiation). The treatments were distributed into four tanks each. WG and SGR were higher in FD group. Fish refed showed hyperphagia just up to the second day of refeeding. The worst feed conversion rate and the lowest protein efficiency ratio were found in fish NF/R controlled. The lowest values of visceral fat somatic index were found in both fasted fish groups, particularly in NF/R at satiation. The LL and glycogen concentrations, and the hepatosomatic index were all elevated in both feed restricted fish. Muscle lipid showed a tendency to decrease after the cycle of fasting and refeeding. Plasma free fatty acids and glucose levels were elevated in fish subjected to feeding restrictions while serum triglycerides levels were reduced. Triiodothyronine levels were significantly depressed in fish from the NF/R-controlled group and remained at the same levels as the control fish in fish NF/R at satiation. Results indicated that fish subjected to cyclic periods of 3-day satiation or controlled feeding after 3-days of fasting were unable to achieve the final body weight of fish fed to satiation after 36 days.

Metabolic changes during the transition from post-feeding to fasting were studied in Brycon cephalus, an omnivorous teleost from the Amazon Basin in Brazil. Body weight and somatic indices (liver and digestive tract), glycogen and glucose content in liver and muscle, as well as plasma glucose, free fatty acids (FFA), insulin and glucagon levels of B. cephalus, were measured at 0, 12, 24, 48, 72, 120, 168 and 336 h after the last feeding. At time 0 h (the moment of food administration, 09.00 h) plasma levels of insulin and glucagon were already high, and relatively high values were maintained until 24 h post-feeding. Glycemia was 6.42 +/- 0.82 mM immediately after food ingestion and 7.53 +/- 1.12 MM at 12 h. Simultaneously, a postprandial replenishment of liver and muscle glycogen reserves was observed. Subsequently, a sharp decrease of plasma insulin occurred, from 7.19 +/- 0.83 ng/ml at 24 h of fasting to 5.27 +/- 0.58 ng/ml at 48 h. This decrease coincided with the drop in liver glucose and liver glycogen, which reached the lowest value at 72 h of fasting (328.56 +/- 192.13 and 70.33 +/- 14.13 mumol/g, respectively). Liver glucose increased after 120 h and reached a peak 168 h post-feeding, which suggests that hepatic gluconeogenesis is occurring. Plasma FFA levels were low after 120 and 168 h and increased again at 336 h of fasting. During the transition from post-feeding to fast condition in B. cephalus, the balance between circulating insulin and glucagon quickly adjust its metabolism to the ingestion or deprivation of food. The aim of this experiment was to evaluate the hypothesis that the M. amazonicum prawns accept feeding in trays. Six ponds were stocked with 10 juveniles II (1.2 +/- 0.7 g) per m(2) in 02/14/2003. An entirely randomized experimental design with 2 treatments (feeding in trays or feeding to the throw) and 3 replicates was used. Prawn average weight, survival, productivity, and physical and chemical variables of the water were compared between treatments using "t" test of Student. Average weight, survival and productivity were, respectively, 7.2 +/- 0.3 g, 46.8 +/- 7.8% and 335 +/- 45 kg/ha in feeding tray treatment and 6.8 +/- 0.1 g, 85.8 +/- 5.2% and 586 +/- 42 kg/ha in feed to the throw treatment. Survival and productivity differed significantly (p<0.5), while that average weight didn't differ significantly. M. amazonicum needs to use further number of trays per area, possibly due to territorial and aggressive behavior.

Três búfalos e três bovinos zebuínos adultos com cânulas ruminais foram alimentados à vontade com dieta de cana-de-açúcar fresca e picada, suplementada com 3 kg de concentrado/animal, durante 13 semanas. Após duas semanas de adaptação, amostras do conteúdo ruminal foram coletadas semanalmente, em dois tempos de amostragem: antes da alimentação e 40 minutos após para determinação da concentração e composição da fauna ruminal. Na quinta semana do experimento, os conteúdos ruminais foram misturados e reinoculados entre animais de mesma espécie e, na última semana, estimadas as degradabilidades in situ da MS, PB e FDN da cana-de-açúcar e MS e PB do concentrado. A concentração média de protozoários foi maior em bovinos, de 4,85 x 10(5)/mL, que em búfalos, de 3,82 x 10(5)/mL. As composições genéricas para Entodinium e subfamília Diplodiniinae foram 79,2 e 6,2% em bovinos e 32,0 e 54,9% em búfalos, respectivamente. Não houve interação significativa entre espécies animais e tempo de amostragem. Houve aumento da concentração média dos holotricos após 40 minutos da alimentação. As degradabilidades efetivas dos nutrientes estudados foram semelhantes entre as duas espécies animais, observando-se diferenças na cinética da degradação. Concluiu-se que os búfalos apresentaram menor concentração de ciliados no rúmen, maior composição de Diplodiniinae e menor de Entodinium que os bovinos; os holotricos exibiram capacidade de migração e seqüestro no rúmen em ambas as espécies; e as diferenças na fauna ruminal não influenciaram a degradabilidade efetiva dos nutrientes da cana-de-açúcar e do concentrado.

The study of growth and uptake of nutrients is important to determine the times of increased demand in order to schedule the fertigation. The present research was developed with the objective of evaluating the effects of different levels of electrical conductivity on growth and accumulation of nutrients in chrysanthemum plants under greenhouse conditions. The electrical conductivity levels on the applied solution were 1.42, 1.65, 1.89, 2.13 and 2.36 dS m-1 (bud vegetative stage); 1.71, 1.97, 2.28, 2.57 and 2.85 dS m-1 (bud stage). The dry mass of the aerial portion of the plant and the contents of macronutrients and micronutrients were determined every 14 days. The nutrient accumulation in chrysanthemum plant showed similar trend as compared to dry matter accumulation. The nutrients presented the following order of absorption: K>N>Ca>Mg>P>S (1425, 892, 184, 150, 110 and 59 mg plant-1) and Fe>Zn>B>Mn>Cu (2254, 2219, 1725, 1287, 210 μg plant-1). The content of most nutrients increased with increase in electrical conductivity of the solution, without salt effect up to EC of 2.85 dS m-1.

PURPOSE: To analyze the time between the first symptom and treatment in patients treated for breast cancer in public hospitals in the Federal District. METHODS: This was a cross-sectional analysis. We interviewed 250 women diagnosed with breast cancer treated in six hospitals of the State Department of Health of the Federal District from November 2009 to January 2011. The time intervals studied were the time between the detection of the symptoms and treatment subdivided into intervals until and after the first medical appointment. The variables were: age, menopausal status, color, educational level, average monthly household income, origin, reason for the initial consultation, staging, tumor size, laterality, metastasis to axillary lymph nodes, neoadjuvant chemotherapy, and type of surgery. The Mann-Whtney test was used to assess the association of these variables with the time intervals until treatment. RESULTS: The mean age was 52 years, with a predominance of white women (57.6%), from the Federal District (62.4%), with a family income of up to 2 minimum wages (78%), and up to four years of schooling (52.4%). The staging of the disease ranged from II to IV in 78.8% of the women. The time between the first symptom and treatment was 229 days (median). After detection of the first symptom, 52.9% of the women attended a consultation within 30 days and 88.8% took more than 90 days to start treatment. Women with elementary school education had a greater delay to the start of treatment (p=0.049). CONCLUSIONS: There was a significant delay to start treatment of women with breast cancer in public hospitals of the Federal District, suggesting that efforts should be made to reduce the time needed to schedule medical appointments and to diagnose and treat these patients.

Forty-five non-lactating, pregnant Holstein animals (18 heifers and 27 multiparous cows; BW = 561 +/- 114 kg; BCS = 2.9 +/- 0.3; days pregnant = 110 +/- 56 d) were stratified by initial BW and BCS, and randomly assigned to receive daily (as-fed basis) 0.50 kg of ground corn plus 0.22 kg of kaolin (CON), calcium salts of saturated fatty acids (SFA) or polyunsaturated fatty acids (PF) for 14 d. Blood samples were collected on days 0, 7 and 14, immediately prior to (0 h) and 3, 6, 9 and 12 h after feeding, to determine the serum concentrations of P 4 and insulin. No treatment effects were detected for serum concentrations of P 4 (5.52, 6.13 and 5.63 +/- 0.41 ng/mL for CON, SFA and PF, respectively). No treatment effects were detected for serum concentrations of insulin (11.5, 10.5 and 10.1 +/- 1.43 mu IU/mL for CON, SFA and PF, respectively). Heifers had greater serum concentrations of P 4 than multiparous cows (6.35 vs. 5.16 +/- 0.42 ng/mL), but lower serum concentrations of insulin (7.0 vs. 14.4 +/- 1.49 mu IU/mL). Feeding 0.22 kg of calcium salts of polyunsaturated fatty acids is not sufficient to increase the serum concentrations of P 4 and insulin of non-lactating, pregnant dairy cows.

Feline hepatic lipidosis or fatty liver disease is a cholestatic syndrome that affects domestic cats and is characterized by excess fat accumulation in the liver of cats. Symptoms commonly seen with this syndrome are anorexia, weight loss, lethargy, vomiting, jaundice, and occasionally behavioral or neurologic signs such as excessive drooling, blindness, coma, and seizures. The diagnosis is based on the patient history, clinical examination, complementary examination, and the definitive diagnosis is obtained by cytology and/or histopathology of hepatic tissue. In serum biochemistry, the main findings include increased serum alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate amino transferase (AST) and bilirubin. The gamma glutamyl transferase (GGT) is normal or slightly increased. The cornerstone of therapy is aggressive feeding to supply the cat full caloric requirements. Without aggressive nutritional support and intensive monitoring the fatty liver disease can be fatal
