Venous wounds cause physical, psychological and financial problems that impact the quality of life of patients. Treatment alternatives are investigated in order to reduce healthcare costs and improve quality of life of people affected by this problem. Physical resources, such as therapeutic ultrasound (US), are being considered in the treatment of ulcers as a potential healing agent. This study aimed to investigate the application of US as a treatment for venous ulcers. Subjects were divided into two groups: US group, where treatment consisted of 5 sessions of pulsed US (3 MHz, 1W/cm²) associated with compression and kinesiotherapy; and sham group, where individuals went through the same procedures, but with sham US therapy. Subjects were evaluated for wound size by planimetry and digital photography, visual analogue scale for pain, quality of life by the questionnaires SF- 36 and VEINES-QoL/Sym and enzymatic activity of metalloproteinases 2 and 9 by zymography. It was observed mean reduction in wound area of 41.58±53.8% for the US group and 63.47±37.2% for the placebo group, maintenance of quality of life scores in the US group and significant improvement (p<0.05) in the placebo group by VEINES questionnaire. It was observed decreased perception of pain in the placebo group. Sample feasibility for analysis of the protein activity of metalloproteinases 2 and 9 by zymography collected by swab method was also confirmed. Our data did not give us evidence to support the theory that the US accelerates healing of venous ulcers in a short-term analysis. However, we observed that standard care associated with compression therapy and kinesiotherapy were able to significantly shorten the progression of chronic venous ulcers

Chronic venous disease (CVD) is evident among the chronic diseases and affects the elderly population and primarily is responsible for leg ulcers in this population. The use of dressings in the care of a venous ulcer is a fundamental part of the treatment for healing, however, evidence to assist in choosing the best dressing is scarce. The main objective of this study was to evaluate the effectiveness of treatment with hydrogel in the healing of venous ulcers using search methods, synthesis of information and statistical research through a systematic review and meta-analysis. Randomized controlled trials were selected in the following databases: CENTRAL; DARE; NHS EED; MEDLINE; EMBASE; CINAHL. Beyond these databases three websites were consulted to identify ongoing studies: ClinicalTrials.gov, OMS ICTRP e ISRCTN. The primary outcomes were analyzed: complete wound healing, incidence of wound infection and the secondary were: changes in ulcer size, time to ulcer healing, recurrence of ulcer, quality of life of participants, pain and costs of treatment. Four studies are currently included in the review with a total of 250 participants. The use of hydrogel appears to be superior to conventional dressing, gauze soaked in saline, for the healing of venous leg ulcers; 16/30 patients showed complete healing of ulcers (RR 5,33, 95%CI [1,73,16,42]). The alginate gel was shown to be more effective when compared to the hydrogel dressing in reduction of the wound area; 61,2% (± 26,2%) with alginate e 19,4% (± 24,3%) with hydrogel at the end of four weeks of treatment. Manuka honey has shown to be similar to the hydrogel dressings in percentage of area reduction. This review demonstrated that there is no evidence available about the effectiveness of the hydrogel compared to other types of dressings on the healing of venous leg ulcers of the lower limbs, thus demonstrating the need of future studies to assist health professionals in choosing the correct dressing.

Introdução: o zinco é um importante micronutriente para numerosos processos bioquímicos em animais e humanos, desempenhando papel de destaque no crescimento e desenvolvimento. Em populações mundiais, a deficiência primária grave de zinco não é comum, embora, a deficiência leve seja bastante prevalente. Considerando que o zinco é essencial para a saúde humana e regula o sistema hipotálamo, hipófise, fígado e osso, buscamos averiguar os seus efeitos no eixo GH-IGF1-IGFBP3 agudamente, mediante administração intravenosa com o elemento zinco, e cronicamente, mediante suplementação oral com o elemento zinco, usando doses fisiológicas de 0.06537 mg Zn/kg (via intravenosa) e 10 mg Zn/dia (via oral). A inclusão de crianças pré-púberes aparentemente saudáveis e eutróficas sem deficiência de zinco é raro na literatura, pois grande parte das publicações foram reportadas em crianças apresentando deficiência de zinco. A metodologia aplicada foi absolutamente inovadora e original, tornando o estudo altamente relevante para a interface entre endocrinologia e nutrição. Objetivo: investigar os efeitos da suplementação oral e administração intravenosa com o elemento zinco sobre a secreção de GH, IGF1, IGFBP3, OCN, ALP, TRAP e PT em crianças aparentemente saudáveis e eutróficas sem deficiência de zinco. Métodos: o estudo foi conduzido durante um período de três meses, e caracterizado por ser randomizado controlado triplo cego. As crianças foram selecionadas por amostragem não probabilística de conveniência, provenientes de escolas públicas municipais, de ambos os gêneros, na faixa etária compreendida entre 8 e 9 anos de idade, divididas em grupo controle (20 crianças recebendo solução placebo contendo 10% de sorbitol) e grupo experimental (20 crianças suplementadas com o elemento zinco na forma de sulfato de zinco heptahidratado – ZnSO4.7H2O). As crianças foram submetidas à suplementação oral de zinco elementar (10 mg Zn/dia) e à administração intravenosa de zinco (0.06537 mg Zn/kg de peso corporal), na forma de ZnSO4.7H2O, cujas amostras sanguíneas foram coletadas em 0, 60, 120, 180 e 210 minutos. Foram realizadas avaliações antropométricas e dietéticas e dosagens bioquímicas e hormonais nas crianças estudadas. Resultados: após a suplementação oral, foi observado no grupo experimental (i) aumento significativo dos valores de ingestão de energia total, proteína e gordura total (p = 0.0007, p< 0.0001, p< 0.0001, respectivamente), (ii) aumento significativo do zinco sérico basal (p< 0.0001), aumento significativo das concentrações plasmáticas de fostatase alcalina (p = 0.0270), e (iv) correlação positiva com o IGF1, IGFBP3, OCN, comparando antes e após a suplementação (p = 0.0011, p< 0.0001, p< 0.0446, respectivamente). Durante a administração venosa de zinco, as concentrações plasmáticas de IGF1 e IGFBP3 aumentaram significativamente no grupo experimental (p = 0.0468, p < 0.0001, respectivamente). Em relação o cálculo da adequação aparente, segundo as DRI, para o cálcio, houve inadequação da dieta com 85% de confiabilidade dos dados; para o ferro, adequação da dieta, com 85% de confiabilidade dos dados. Para o zinco, adequação da dieta, com 50% de confiabilidade dos dados. Conclusões: a suplementação oral com o elemento zinco pode ter estimulado um aumento na ingestão de energia total, proteína e gordura total, assim como, nas concentrações basais de zinco sérico e nas concentrações plasmáticas de fosfatase alcalina. A administração intravenosa de zinco aumentou as concentrações séricas de zinco e as concentrações plasmáticas de GH, IGF1 e IGFBP3 no grupo experimental.

OBJECTIVE: To dev elop and evaluate the use of a body adjustable device for training of peripheral venous catheterization for healthcare undergraduate students . METHODS: This study involved two phases: I) development of the innovative simulator and II) a controlled trial co mparing the performance of the body adjustable device in relation to the commercial simulator for the training of venipuncture skills. A total of 79 first - year medical students participated in the intervention phase, which consisted of pretest assessment, lecture on peripheral venous access, randomization into two groups according to the simulator used for training (Commercial Simulator and Experimental Simulator), real venipuncture procedure, post - test assessment and evaluation of satisfaction. RESULTS: Gr oups were homogeneous in age, sex, pre - test and post - test scores, attitudinal assessment and performance in performing the real venipuncture. Students from the Experimental Simulator group performed better on the filling of simulated records. At the end of the study, cognitive gain significantly increased in both groups. The degree of realism perceived by students was equivalent for two groups. A total of 85.7% of students rated the Experimental Simulator as good or excellent. CONCLUSIONS: Experimental simu lator proved to be a low cost alternative for the training of venipuncture skills in upper limb. The cognitive procedural and attitudinal performances of students who used the experimental simulator were similar to those observed in the group trained with commercial simulator.

Angiogenesis and lymphangiogenesis are changes that occur due to gingival inflammation caused by microorganisms present in the biofilm, as well as the migration of immune cells and secretion of mediators in the aggressed site. This study aimed to research angiogenesis and lymphangiogenesis in 90 specimens of clinically healthy, with gingivitis and chronic periodontitis gingival tissue biopsies. The histological sections were evaluated by hematoxylin and eosin and the immunohistochemical technique through immunostaining for CD34 and podoplanin. To evaluate the angiogenic and lymphangiogenic indexes we performed a microvessel counting technique. The results showed that there is a correlation between the indexes (p = 0.030), however, we observed that periodontitis showed less lymphatic vessels than clinically healthy gingival tissue (p = 0.016). Podoplanin showed positive staining in the basal layers of the epithelium, and we observed a relationship between immunostaining intensity and the intensity of inflammatory infiltrate, with more intense staining in the presence of severe inflammatory infiltrate (p = 0.033). For this study, we concluded that there are fewer blood vessels in periodontitis compared with clinically healthy gingiva. The signaling present in the inflammatory process and the actual role of gingival blood and lymphatic vasculature are not fully understood, with further studies on angiogenesis and lymphangiogenesis being suggested.

Detectar os principais achados fundoscópicos em crianças portadoras de hemoglobinopatias falciformes. Métodos: Foram estudados 26 pacientes com hemoglobinopatias falciformes, no Serviço de Oftalmologia do Hospital Universitário Onofre Lopes, Natal, RN, que foram submetidos a protocolo de pesquisa pré-estabelecido. Os resultados foram avaliados estatisticamente pelo teste qui-quadrado. Resultados: A idade média foi de 10,6 anos, com acuidade visual igual ou melhor que 20/25 na maioria, excetuando-se 3 olhos, que apresentavam outras doenças associadas. O tipo mais freqüente foi o SS com 57,7% (15/26) dos casos, seguido pelos SC e SA com 15,4% (4/26) cada, e pelo S-Thal com 11,5% (3/26). A freqüência da retinopatia por células falciformes foi maior após os 10 anos de idade, sendo mais freqüente, em valores relativos, no tipo S-Thal (100% dos casos) e, em valores absolutos, no tipo SS (9 casos). Os dois achados mais comuns foram tortuosidade venosa (12/26) e “black sunburst” (7/26). Conclusões: Observamos que a incidência de retinopatia por células falciformes aumentou após os 10 anos de idade e não evidenciamos achados da doença proliferativa. Portanto, enfatizamos a necessidade do exame oftalmológico precoce nos portadores de anemia falciforme, como forma de prevenir futuras complicações oculares
