Purpose: Increased risk of arrhythmic events occurs at certain times during the circadian cycle with the highest risk being in the second and fourth quarter of the day. Exercise improves treatment outcome in individuals with cardiovascular disease. How different exercise protocols affect the circadian rhythm and the associated decrease in adverse cardiovascular risk over the circadian cycle has not been shown. Methods: Fifty sedentary male participants were randomized into an 8-week high volume and moderate volume training and a control group. Heart rate was recorded using Polar Electronics and investigated with Cosinor analysis and by Poincaré plot derived features of SD1, SD2 and the complex correlation measure (CCM) at 1-h intervals over the 24-h period. Results: Moderate exercise significantly increased vagal modulation and the temporal dynamics of the heart rate in the second quarter of the circadian cycle (p = 0.004 and p = 0.007 respectively). High volume exercise had a similar effect on vagal output (p = 0.003) and temporal dynamics (p = 0.003). Cosinor analysis confirms that the circadian heart rate displays a shift in the acrophage following moderate and high volume exercise from before waking (1st quarter) to after waking (2nd quarter of day). Conclusions: Our results suggest that exercise shifts vagal influence and increases temporal dynamics of the heart rate to the 2nd quarter of the day and suggest that this may be the underlying physiological change leading to a decrease in adverse arrhythmic events during this otherwise high-risk period.

This thesis presents an intellectual history of the historiography of Australian Economic History between 1918 and 1965. More specifically, it is a contribution to a relatively novel area of research into 'disciplinary history’. It takes as its basic analytical material the four books widely used for significant lengths of time for undergraduate teaching during the period of the study. The thesis consists of five main chapters, plus an appendix which surveys the institutional development of Australian Economic History and provides the empirical basis for the selection of the works named above. After a brief introduction and overview, the next four chapters consist of a detailed study of one of these works, the historical context in which each was written, and an intellectual biography. The fifth chapter is largely theoretical and conceptual. It analyses the epistemological bases of History and Economics and explores the implications of different models of knowledge for the relationship between Economic History and its two antecedent disciplines, History and Economics. Current perceptions of the state of the discipline in Australia and overseas are also examined. There are three main propositions advanced and their implications explored in the fifth chapter. First, that changes which occurred in Australian Economic History during the period 1918-1965 shifted the discipline from the broad area of History to the broad area of Economics. Second, that the inherent tension and fundamental differences between the two disciplinary areas of History and Economics have profound and complex implications for Australian Economic History at a number of levels and in a number of areas. The third proposition posits that the paradigm shift of the 1950s/1960s in Australian Economic History, and the paradigm shift of the 1960s/1970s in Economic History respectively have resulted in crisis. The final part of the chapter summarises the contents of the preceding chapters, and draws some conclusions based on those detailed studies.

A study of Governor-General Ronald Munro-Ferguson with particular attention to the views of modern historians. It describes his constitutional role and interventions in the Australian political process. It concludes that he was a conscientious King’s representative, with a deep interest in Australian political and social affairs and was accepted by members of both parties.

While there are relevant studies on both local political subcultures and party activism in Italy, the literature misses the relations between these two social and political phenomena. This article aims at bridging the lacuna by presenting a typology of the local branches of the Italian PD (Democratic Party) based on the relationship between the features of party activism and the local political subcultures. Four types of local PD branch emerge: the ‘showcase’ branch, the ‘administrative’ branch, the ‘company’ branch and the ‘committee’ branch. The article discusses each type, while drawing on 40 in-depth interviews collected during field research. Insights into the relationship between local political subcultures and party activism in Italy are offered.

This paper presents the case of the Romanies in Italy and the ‘forgotten’ nature of their genocide. The crimes committed by the Fascist regime towards these peoples during the Second World War were not disclosed until recently. In past decades it was commonly believed that Fascism had targeted Romanies merely as a problem of ‘public order’, rather than as a racial issue. This study argues that a lack of official acknowledgement, together with recent authoritarian approaches towards them (such as the introduction of 2008 ‘Nomad Emergency’ and the ongoing adoption of the highly criticized ‘camps policy’), could all be interpreted as an indirect consequence of the government's incapacity to deal with a shameful past and its unbroken ties. The existence of ‘gaps’ in Italian collective memory is now harming the health of Italy's democratic polity, allowing racism to re-emerge, while resuscitating a deep-seated belief in the ‘legendary generosity’ of Italians.

From the thermodynamic point of view, the global warming problem is an ''energy balance'' problem. The heat (energy) accumulation in the earth and its atmosphere is the cause of the global warming. This accumulation is mainly due to the imbalance of (solar) energy reaching and the energy leaving the earth, caused by ''greenhouse effect'' in which the CO2 and other greenhouse gases play a critical role; so that balance of the energy entering and leaving the earth should be the key to solve the problem. Currently in the battle of tackling the global warming, we mainly focus on the development of CO2-related measures, i.e., emission reduction, CO2 sequestration, and CO2 recycle technologies. It is right in technical aspect, because they are attempting thinner the CO2 ''blanket'' around the earth. However, ''Energy'' that is the core of the problem has been overlooked, at least in management/policy aspect. This paper is proposing an ''Energy Credit'' i.e., the energy measure concept as an alternative to the ''CO2 credit'' that is currently in place in the proposed emission trading scheme. The proposed energy credit concept has the advantages such as covering broad activities related to the global warming and not just direct emissions. Three examples are given in the paper to demonstrate the concept of the energy measure and its advantages over the CO2 credit concept.

Background : The aim of the ACE-Obesity study was to determine the economic credentials of interventions which aim to prevent unhealthy weight gain in children and adolescents. We have reported elsewhere on the modelled effectiveness of 13 obesity prevention interventions in children. In this paper, we report on the cost results and associated methods together with the innovative approach to priority setting that underpins the ACE-Obesity study.

Methods : The Assessing Cost-Effectiveness (ACE) approach combines technical rigour with 'due process' to facilitate evidence-based policy analysis. Technical rigour was achieved through use of standardised evaluation methods, a research team that assembles best available evidence and extensive uncertainty analysis. Cost estimates were based on pathway analysis, with resource usage estimated for the interventions and their 'current practice' comparator, as well as associated cost offsets. Due process was achieved through involvement of stakeholders, consensus decisions informed by briefing papers and 2nd stage filter analysis that captures broader factors that influence policy judgements in addition to cost-effectiveness results. The 2nd stage filters agreed by stakeholders were 'equity', 'strength of the evidence', 'feasibility of implementation', 'acceptability to stakeholders', 'sustainability' and 'potential for side-effects'.

Results :
The intervention costs varied considerably, both in absolute terms (from cost saving [6 interventions] to in excess of AUD50m per annum) and when expressed as a 'cost per child' estimate (from <AUD1.0 [reduction of TV advertising of high fat foods/high sugar drinks] to >AUD31,000 [laparoscopic adjustable gastric banding for morbidly obese adolescents]). High costs per child reflected cost structure, target population and/or under-utilisation.

Conclusions : The use of consistent methods enables valid comparison of potential intervention costs and cost-offsets for each of the interventions. ACE-Obesity informs policy-makers about cost-effectiveness, health impact, affordability and 2nd stage filters for important options for preventing unhealthy weight gain in children. In related articles cost-effectiveness results and second stage filter considerations for each intervention assessed will be presented and analysed.

The Rank Forum on Vitamin D was held on 2nd and 3rd July 2009 at the University of Surrey, Guildford, UK. The workshop consisted of a series of scene-setting presentations to address the current issues and challenges concerning vitamin D and health, and included an open discussion focusing on the identification of the concentrations of serum 25-hydroxyvitamin D (25(OH)D) (a marker of vitamin D status) that may be regarded as optimal, and the implications this process may have in the setting of future dietary reference values for vitamin D in the UK. The Forum was in agreement with the fact that it is desirable for all of the population to have a serum 25(OH)D concentration above 25 nmol/l, but it discussed some uncertainty about the strength of evidence for the need to aim for substantially higher concentrations (25(OH)D concentrations>75 nmol/l). Any discussion of ‘optimal’ concentration of serum 25(OH)D needs to define ‘optimal’ with care since it is important to consider the normal distribution of requirements and the vitamin D needs for a wide range of outcomes. Current UK reference values concentrate on the requirements of particular subgroups of the population; this differs from the approaches used in other European countries where a wider range of age groups tend to be covered. With the re-emergence of rickets and the public health burden of low vitamin D status being already apparent, there is a need for urgent action from policy makers and risk managers. The Forum highlighted concerns regarding the failure of implementation of existing strategies in the UK for achieving current vitamin D recommendations.
