A core challenge that emerged from the historically based critique of Freud's work that constitutes Freud Evaluated: The Completed Arc (Macmillan, 1991, 1997) was to Freud's claim that the basic rule of psychoanalysis for inquiring into the causes of symptoms was unaffected by his “suggestions.” That is, in demanding that the patient tell the analyst whatever came to mind with complete candor, the patient would not be led astray by the analyst's ideas about the possible causes of the symptoms. In this article, the assumptions underlying Freud's belief in the validity of the method of free association are made explicit and criticized. The basic indetermination inherent in interpreting material apparently recovered by the method is also made explicit. Some of the consequences of the method for psychoanalytic developmental theories and for deciding between the variant schools of psychoanalysis are set out.

The substituted judgement principle is often recommended as a means of promoting the self-determination of an incompetent individual when proxy decision makers are faced with having to make decisions about health care. This article represents a critical ethical analysis of this decision-making principle and describes practical impediments that serve to undermine its fundamental purpose. These impediments predominantly stem from the informality associated with the application of the substituted judgement principle. It is recommended that the principles upon which decisions are made about health care for another person should be transparent to all those involved in the process. Furthermore, the substituted judgement principle requires greater rigour in its practical application than currently tends to be the case. It may be that this principle should be subsumed as a component of advance directives in order that it fulfils its aim of serving to respect the self-determination of incompetent individuals.

Child protection legislation has undergone a number of changes since its inception, changes that have redefined the population of children in need of protection. However, child protection data on notifications and substantiations remain the most common source of data for statistics on the rate of maltreatment and the breakdown of specific maltreatment types. In the present study, three factors are identified that have compromised the accuracy of child protection data reporting the incidence of child abuse and neglect: (i) the legislative changes that mandate child protection services to protect children from harm rather than from identifiable adult actions; (ii) the shift from the Harm Standard to the Endangerment Standard; and (iii) the assignment of responsibility solely to parents.

This research was undertaken to explore the effectiveness of an already established community of practice among staff at a small rurally-based training organisation as a vehicle through which to develop innovative practice in online collaboration and learning. The research was situated within that ongoing innovation, and used interview and observational techniques to generate the research data from staff and management personnel. Substantial limitations to using an already existing community of practice to develop innovative practice were shown in the research. Development of new behaviours was substantially inhibited by the power of already established behaviours and practices. Additionally, lack of sufficient experience among community members in online technologies was a further barrier to effective and orderly development. The research indicates that the achievement of innovative practice through communities of practice within existing workplaces may best be served by the strategic development of members of those communities, focusing on change from already established behaviours, and by ensuring a mix of skill and experience to support and lead less experienced participants.

Objectives
To elicit descriptive data about limited joint range of motion (ROM) in subjects with type II or III spinal muscular atrophy (SMA) and to examine the relation between the number of motions with limited range and both age and functional ability.
Design
Descriptive cross-sectional study.
Setting
Neurologic pediatric outpatient clinic at a hospital in Taiwan.
Participants
Twenty-seven subjects with SMA type II (mean age, 9.8±6.5y) and 17 with SMA type III (mean age, 12.2±8.7y).
Intervention
Measurement with transparent goniometers of joint ROM bilaterally of the shoulder, elbow, wrist, hip, knee, and ankle.
Main outcome measures
The proportion of participants with each ROM limitation compared with all participants with the same SMA type, age distribution of the participants with each ROM limitation, mean range loss of each motion limitation, and the contracture index (risk index of joint contracture).
Results
Eighty-nine percent of the participants with SMA type II experienced knee extension limitation. Approximately 50% of the participants with both types of SMA had ankle dorsiflexion limitation. The motions of knee and hip extension and ankle dorsiflexion also had a relatively high contracture index. The number of motions with limited range positively correlated (P<.001) with age and upper-extremity functional grade (the higher the functional grade, the poorer the functional ability) for SMA type II.
Conclusions
We found varying degrees of joint ROM limitation. Certain motions were noted to be high risks for the development of contractures. This risk was higher mostly in younger children.

Environmental Management Systems (EMSs) address environmental risks in supply chains and certification of environmental claims. Governments supporting EMSs have encouraged producers to respond to anticipated consumer environmental concerns. Attempts at implementing EMSs have rarely been in direct response to market demand but are usually farmer organisation driven – to forestall increased regulation. In Australia, consumer demand for foods produced to environmentally sustainable standards is minimal because consumers don’t believe these products offer special benefits. EMS implementation is expensive and onerous; and the products require a market premium. Food consumers have difficulty differentiating the terms organic, environmentally-friendly, and sustainably-produced in food labelling.

Best-practice guidelines in the area of investigative interviewing of children specify the importance of using open-ended questions. However, use of open-ended questions per se does not maximise interview outcome; open-ended questions can vary markedly in quality. The aim of this study was to identify the nature of investigative interviewers' limitations when using open-ended questions, and to compare how representative these limitations are in three distinct interview paradigms. These interview paradigms include: (a) interviews in which trained actors played the role of a 5-6-year-old child; (b) interviews where 5-6-year-old children recalled an innocuous event that was staged in their school; and (c) actual field interviews where child witnesses aged 5 to 7 years recalled an abusive event. Overall, several common problems that would restrict children's opportunity to provide elaborate and accurate narrative accounts of events were identified and described. Our identification of these problems (using a dichotomous rating scale) was found to be consistent with an independent expert, and their presence was not limited to those interviewers who used a low frequency of open-ended questions. The implications of the findings for researchers and trainers in the area of investigative interviewing of children are discussed.

In the last decade, the Internet email has become one of the primary method of communication used by everyone for the exchange of ideas and information. However, in recent years, along with the rapid growth of the Internet and email, there has been a dramatic growth in spam. Classifications algorithms have been successfully used to filter spam, but with a certain amount of false positive trade-offs. This problem is mainly caused by the dynamic nature of spam content, spam delivery strategies, as well as the diversification of the classification algorithms. This paper presents an approach of email classification to overcome the burden of analyzing technique of GL (grey list) analyser as further refinements of our previous multi-classifier based email classification [10]. In this approach, we introduce a “majority voting grey list (MVGL)” analyzing technique with two different variations which will analyze only the product of GL emails. Our empirical evidence proofs the improvements of this approach, in terms of complexity and cost, compared to existing GL analyser. This approach also overcomes the limitation of human interaction of existing analyzing technique.

Since the 1990s there has been a rise in both the prevalence of party drug use in Australia and the use of party drug-related websites. This study investigates whether it is feasible to recruit and survey party drug users via the internet. It took place in Victoria, Australia. Participants were directed to a website where they completed a brief, structured internet-based survey. A total of 460 responses were received over 31 days, 393 of which fitted all inclusion criteria. The sample consisted predominately of young, male polydrug users and is one of the largest samples of party drug users in Australia reported thus far. It was concluded that it is feasible to recruit and survey current party drug users via the internet and that this method is quicker and cheaper than traditional survey methods, although samples are not necessarily representative of the party drug-using population. Other limitations and advantages are discussed.
