The annual survey to collect data on the contact lens prescribing preferences of practitioners has continued for 12 years in Canada. Practitioners were surveyed for information on the first 10 patients they fitted with contact lenses immediately after receipt of said survey. A total of 121 completed surveys were returned, detailing contact lens fittings for 1,184 patients.

Of the patients fitted with contact lenses, the results indicated that the majority were prescribed soft lenses. The preferred modality was monthly planned replacement and over 67 per cent of the soft lens fits were made of silicone hydrogel materials. Of the rigid lens fits, the majority were prescribed for daily wear, as the overnight use of rigid lenses appears to be mainly for orthokeratology. Practitioners are recommending multipurpose solutions for the majority of their soft lens patients (85.6%).

This article presents our fourth annual survey of international contact lens prescribing trends. In 2004, we analyzed prescribing habits in 14 countries: Australia, Belgium, Canada, Germany, Greece, Italy, Japan, the Netherlands, New Zealand, Norway, Singapore, Sweden, the United Kingdom and the United States. To each, we sent 1,000 surveys (or a pro-rata number in countries with less than 1,000 practitioners) to clinicians randomly selected from appropriate national registers.

The surveys elicited prospective information about contact lenses and care products that respondents prescribed to up to 10 patients fit after the study began. We employed a weighting system to better reflect the trends of busier practices.

To determine the extent of rigid contact lens fitting worldwide and to characterize the associated demographics and fitting patterns. Survey forms were sent to contact lens fitters in up to 40 countries between January and March every year for five consecutive years (2007 to 2011). Practitioners were asked to record data relating to the first 10 contact lens fits or refits performed after receiving the survey form. Survey data collected between 1996 and 2011 were also analyzed to assess rigid lens fitting trends in seven nations during this period. Data were obtained for 12,230 rigid and 100,670 soft lens fits between 2007 and 2011. Overall, rigid lenses represented 10.8% of all contact lens fits, ranging from 0.2% in Lithuania to 37% in Malaysia. Compared with soft lens fits, rigid lens fits can be characterized as follows: older age (rigid, 37.3 ± 15.0 years; soft, 29.8 ± 12.4 years); fewer spherical and toric fits; more bifocal/multifocal fits; less frequent replacement (rigid, 7%; soft, 85%); and less part-time wear (rigid, 4%; soft, 10%). High-Dk (contact lens oxygen permeability) (36%) and mid-Dk (42%) materials are predominantly used for rigid lens fitting. Orthokeratology represents 11.5% of rigid contact lens fits. There has been a steady decline in rigid lens fitting between 1996 and 2011. Rigid contact lens prescribing is in decline but still represents approximately 10% of all contact lenses fitted worldwide. It is likely that rigid lenses will remain as a viable, albeit increasingly specialized, form of vision correction.

Each year we survey contact lens prescribing colleagues around the world (optometrists, opticians, or ophthalmologists depending on the country) and request prospective information about the first 10 patients that they fit with contact lenses after receipt of our survey. In this, our 12th consecutive annual report for Contact Lens Spectrum, we present information relating to 20,566 contact lens fits across 36 countries.

The 13th annual survey of Australian contact lens prescribing was conducted between January and April 2012. The same format as in previous years was employed. About 3,000 members of Optometrists Association Australia were sent an e-mail message with a link to a downloadable questionnaire, and a request that this be accessed, printed and completed to provide details of the first 10 patients fitted with contact lenses after receipt of the questionnaire.

We have been collecting data on worldwide contact lens prescribing habits for almost 20 years. Over this time period, we have amassed prospective information about 315,000 contact lens fits in 59 countries. This article marks our 15th report in Contact Lens Spectrum and features a breakdown of more than 23,000 contact lens fits in 34 markets.As in previous years, our international network of coordinators distributed survey forms to eyecare practitioners in their market who then recorded generic information about the first 10 patients fit with contact lenses after receipt. Information is gathered about patient age and gender; whether the contact lenses are prescribed as a new fit or a refit; contact lens material, design, and replacement frequency; number of intended days per week of use; wearing modality; and care system. Contact lens fits are weighted to reflect the number of fits undertaken by each eyecare practitioner. The study data were entered and processed at the University of Manchester and at the University of Waterloo.

Purpose:  The purpose of the study was to obtain anterior segment biometry for 40 normal eyes and to measure variables that may be useful to design large diameter gas permeable contact lenses that sit outside the region normally viewed by corneal topographers. Also, the distribution of these variables in the normal eye and how well they correlated to each other were determined.

Methods:  This is a cross-sectional study, in which data were collected at a single study visit. Corneal topography and imaging of the anterior segment of the eye were performed using the Orbscan II and Visante OCT. The variables that were collected were horizontal K reading, central corneal/scleral sagittal depth at 15 mm chord, and nasal and temporal angles at the 15 mm chord using the built-in software measurement tools.

Results:  The central horizontal K readings for the 40 eyes were 43 ± 1.73 D (7.85 ± 0.31 mm), with ± 95% confidence interval (CI) of 38.7 (8.7 mm) and 46.6 D (7.24 mm). The mean corneal/scleral sagittal depth at the 15 mm chord was 3.74 ± 0.19 mm and the range was 3.14 to 4.04 mm. The average nasal angle (which was not different from the temporal angle) at the 15 mm chord was 39.32 ± 3.07 degrees and the ± 95%CI was 33.7 and 45.5 degrees. The correlation coefficient comparing the K reading and the corneal/scleral sagittal depth showed the best correlation (0.58, p < 0.001). The corneal/scleral sagittal depth at 15 mm correlated less with the nasal angle (0.44, p = 0.004) and the weakest correlation was for the nasal angle at 15 mm with the horizontal readings (0.32, p = 0.046).

Conclusion:  The Visante OCT is a valuable tool for imaging the anterior segment of the eye. The Visante OCT is especially effective in providing the biometry of the peripheral cornea and sclera and may help in fitting GP lenses with a higher percentage of initial lens success, when the corneal sag and lens sag are better matched.

The use of rigid materials with high oxygen permeability (Dk) is on the increase, their major benefit being the reduction of hypoxia. However, there is a reluctance to use these materials possibly due to increased surface scratching, reduced wettability, increased deposition, reduced life expectancy and parameter instability. Considering parameter stability, various studies have demonstrated contradictory results or used high Dk materials based on the silicone acrylate polymer. This study was designed to investigate whether the parameters of a high Dk rigid fluorocarbon contact lens material changed during daily wear and extended wear schedules. Thirty five subjects, divided into group groups, Group I wore the lens on a daily wear basis, whereas those in Group II wore the lens on a monthly extended wear basis. The parameters and integrity of the lenses were monitored in both groups every 3 months. For lens integrity a statistically significant increase in surface scratching was demonstrated for the lenses worn by the subjects of both groups over the time of the study (Group I, F=7.990, P <0.0001 [ANCOVA]; Group II, F=6.241, P=0.003 [ANCOVA]). The only parameter to show a statistically significant variation over the study period was that of centre thickness for the lenses worn by the subjects in Group I (F=3.976, P=0.0063 [ANCOVA]), with a mean reduction in centre thickness of 0.022 mm at the 12 month visit.

No change was noted for either group or between groups for the other parameters measured. This study demonstrates that the parameters of rigid contact lenses manufactured from high Dk fluorocarbons are stable with only a non-clinically significant reduction in centre thickness for the contact lenses worn by the subjects in Group I.

Contact lenses can affect the cornea in a variety of ways. Corneal structure can be altered so that its thickness changes to involve the epithelium and the stroma. As a result, the curvature may be affected, but whether it is the front or the back surface that is affected depends on the type of lens used. If thickness increases sufficiently, corneal transparency may decrease. Contact lenses can also affect cellular structure of all layers of the cornea through mechanical trauma, hypoxia, or toxicity from solutions that are used in association with lenses. More serious complications, such as inflammation and infection, can arise. All these changes can be detected by clinicians using slitlamp biomicroscopes and keratometers if the changes are significant enough. Since the development of computers, optical instruments have become more sophisticated and have enabled the detection of subtle changes but have also facilitated more precise measurement of these conditions along with the ability to capture images of the alterations or defects. This article describes some of the newer techniques and, specifically, the application of optical coherence tomography, confocal microscopy, and esthesiometry.

Purpose. To report the development of a new apparatus for non-invasive collection of human corneal epithelial cells.

Methods. Previous methods of non-invasive, irrigative corneal cell collection resulted in low cell yields limiting potential analysis. A new ocular surface cell collection apparatus (OSCCA) was designed to collect more epithelial cells from direct irrigation of the corneal surface to allow for clinical comparisons. Forty-five samples were obtained (unilateral or bilateral over seven visits) from five human participants. Cell yield, size, phenotype, and corneal staining (prior and post eye wash) were examined.

Results. On average 364 ± 230 epithelial cells were collected from the cornea per eye. Epithelial cell sizes ranged from 8.21 to 51.69 μm in diameter, and 67.30 to 2098.85 μm2 area. The proportion of corneal specific cells collected per sample was 75 ± 14% as determined by positive K3 expression with AE5. On average, 77 ± 0.2% of epithelial cells harvested were nucleated, the remainder were non-nucleated ghost cells. Corneal staining was reduced in the OSCCA-washed vs. contralateral non-washed eyes (p = 0.02).

Conclusions. The OSCCA allows collection of human corneal epithelial cells with significantly higher yields, and greater specificity than previously reported. Reduced corneal staining observed post eye-wash demonstrated the safety of the technique, and its ability to remove cells directly from the corneal surface. The OSCCA could provide an objective non-invasive method of investigating pathological changes, effects of topical therapeutics, and impact of contact lenses and care-solutions of the cells of the ocular surface.
