Objectives:
To determine the safety and acceptability 
of the TrueBlue model of nurse-managed care in the primary healthcare setting.

Design
A mixed methods study involving clinical record audit, focus groups and nurse interviews as a companion study investigating the processes used in the TrueBlue randomised trial.
Setting:
Australian general practices involved in the TrueBlue trial.
Participants:
Five practice nurses and five general practitioners (GPs) who had experienced nurse- managed care planning following the TrueBlue model of collaborative care.
Intervention:
The practice nurse acted as case manager, providing screening and protocol management of depression and diabetes, coronary heart disease or both.
Primary outcome measures:
Proportion of patients provided with stepped care when needed, identification and response to suicide risk and acceptability of the model to practice nurses and GPs.
Results:
Almost half the patients received stepped care when indicated. All patients who indicated suicidal ideations were identified and action taken. Practice nurses and GPs acknowledged the advantages of the TrueBlue care-plan template and protocol-driven care, and the importance of peer support for the nurse in their enhanced role.
Conclusions:
Practice nurses were able to identify, assess and manage mental-health risk in patients with diabetes or heart disease.

Despite the widespread use of ground rules in forensic interview guidelines, it is unknown whether children retain and apply these rules throughout narrative interviews. We evaluated the capacity of 260 five- to nine-year-olds to utilize three ground rules. At the beginning of the interview all children heard the rules; half also practiced them. Children then responded to open-ended prompts about a repeated laboratory event and were assessed for their application of the rules. Logistic regressions revealed that practice only benefitted the use of the "don't know" rule. Although the children accurately answered "don't understand" and "correct me" practice questions, practice appeared to give no greater benefit than just hearing the rules. Results suggest that the current format of ground rule practice in interview guidelines is appropriate for the "don't know" rule, but the other rules may require more extensive practice with this age group. There is growing interest, worldwide, in collaboration between schools and community organisations in contributing to and enriching school science programs, yet such collaborations are inadequately understood. This paper reports data from an Australian study designed to probe the views of members of the community who have participated in a broad range of such collaborations in school science programmes in order to better understand the issues which impact on their operation. The data were collected by interviews with the community participants selected by opportunistic sampling. The analysis reveals a number of issues—purposes, communication, organisational structures and curriculum—which can be seen as impacting on the collaborations. These are examined through the concepts of communities of practice, boundaries and boundary crossing, associated with people from scientific communities of practice interacting with school communities. The paper reflects on the implications of these findings for constructing effective school-community collaboration in school science programs.

Individuals involved in fostering interprofessional collaborative practice in health are employed in the education, practice and political arenas. While the need to innovate and develop optimal training and practice is not new, the uniqueness of interprofessional collaborative practice is that it exists across professional backgrounds and transcends traditional hierarchies (entry-level to senior practitioners). As such, alternate models of support are required to assist champions to progress learning and innovation. One such model is a group of educators and practitioners networking across Australasia, resulting in the Australasian Community of Interprofessional Collaborative Practice (ACoIPCP). ACoIPCP is a lively community of practice (CoP) group across Australia and New Zealand, which is abreast of current activity in the relevant arenas and provides members with an avenue to share information and, therefore, respond appropriately to changes in the environment. Membership includes likeminded individuals who work in the area of interprofessional collaboration from a broad range of perspectives in both health education and practice. This paper describes the development of ACoIPCP and its aims, activities and achievements. By developing a community of practice framework in a cross-organisational environment, ACoIPCP members have been able to support one another, share resources, seek feedback and learn with and from one another to foster interprofessional collaborative practice within educational, clinical and political settings. Information about the processes and outcomes of ACoIPCP may provide guidance to others interested in facilitating learning and innovation through a community of practice model.

Aim

Overweight and obesity affects approximately 20% of Australian pre-schoolers. The general practice nurse (PN) workforce has increased in recent years; however, little is known of PN capacity and potential to provide routine advice for the prevention of child obesity. This mixed methods pilot study aims to explore the current practices, attitudes, confidence and training needs of Australian PNs surrounding child obesity prevention in the general practice setting.

Methods

PNs from three Divisions of General Practice in New South Wales were invited to complete a questionnaire investigating PN roles, attitudes and practices in preventive care with a focus on child obesity. A total of 59 questionnaires were returned (response rate 22%). Semi-structured qualitative interviews were also conducted with a subsample of PNs (n = 10).

Results

Questionnaire respondent demographics were similar to that of national PN data. PNs described preventive work as enjoyable despite some perceived barriers including lack of confidence. Number of years working in general practice did not appear to strongly influence nurses' perceived barriers. Seventy per cent of PNs were interested in being more involved in conducting child health checks in practice, and 85% expressed an interest in taking part in child obesity prevention training.

Conclusions

Findings from this pilot study suggest that PNs are interested in prevention of child obesity despite barriers to practice and low confidence levels. More research is needed to determine the effect of training on PN confidence and behaviours in providing routine healthy life-style messages for the prevention of child obesity.

Research into the experiences of casual relief teachers (CRTs)(substitute or supply teachers) across Australia and internationallyhas reported feelings of marginalisation among participants. Thesefindings are concerning when one considers that students mightbe in the care of CRTs for an equivalent of 1 year or morethroughout their schooling. When CRTs describe such feelingsthere is a suggestion that they do not feel a part of the communityof practice in which they work. Accordingly, their opportunities forprofessional learning are often compromised, which has implicationsfor their ability to maintain pedagogical knowledge andskills. This study used cluster sampling survey data to offer insightsinto professional challenges faced by CRTs. The discussion examinesthe self-determined skills of 59 Australian CRTs and the wayschooling is organised that may leave them feeling excludedrather than members of what should be their “communities ofpractice.”

Despite its ubiquitous employment by users of English to achieve authentic communicative goals, taboo language has received little attention in the education literature. Even less focus has been placed on such language in English language teaching - specifically, in teaching English as an Additional Language (EAL). Given the multiplicity of communicative struggles experienced by EAL learners surrounding the use of taboo language in authentic communication, meaningful consideration of this aspect can be seen as crucial in EAL instruction. Classroom learning could prepare learners for navigation and negotiation of taboo language use they will inevitably encounter in social interactions in target language communities of practice. However, EAL teachers' uncertainty or reluctance to introduce taboo language in classroom instruction is a key impediment in developing learners' sociocultural knowledge regarding such language use. We foreground one case of such uncertainty and reluctance surrounding the introduction of taboo language in EAL instruction derived as interview data from an experienced EAL teacher.

A common response to the need to place increasing numbers of social work students in field education or practice learning placements has been to broaden the range of organisations in which placements are sought. While this strategy has provided many beneficial learning opportunities for students, it has not been sufficient in tackling ongoing difficulties in locating work-integrated learning opportunities for social work students. We argue that new approaches to finding placement opportunities will require a fundamental rethink as to how student placements are understood. This paper introduces an innovative project which started with a consideration of learning opportunities and built a structure to facilitate these, rather than rely on organisational availability to host students on placements.
