This thesis resulted in a grounded theory that described and explained how Australian residential aged care residents’ continence care needs were determined, delivered, and communicated. The researcher identified a basic social problem that influenced overall care, and a basic social process staff used to deal with the problem.

Contemporary attempts to ‘organise’ risk and manage uncertainty are remaking many ‘industrial-era’ institutions – including maternity hospitals. Health policies are encouraging a shift away from hierarchical, medically dominated structures towards new governance systems and ‘women-centred’ care, often led by midwives. To understand the resulting contestation, in this article we argue for a wider conceptual frame than a focus on neo-liberal state regulation of the professions. We utilise theories of the ‘second modernity’, in particular those concerning socio-cultural changes associated with shifts in risk regimes, to interpret findings from qualitative research studies undertaken in Australian maternity hospitals. Whereas analysis confined to macro or institutional levels emphasises stability and hegemony, we demonstrate that when cultural and interactional levels are examined, considerable fluidity and uncertainty in the identification and negotiation of risk is evident, resulting in new work practices with inevitable shifts in professional identities and allegiances.

Oxygen therapy is a major intervention used to manage respiratory dysfunction. Research findings revealed a need for health care professionals to review oxygen device selection in specific clinical settings, the importance of involving patients in their care decisions and to document appropriately care that is provided associated with oxygen therapy.

A systematic review was undertaken to evaluate the effect of lateral positioning compared to other body positions on morbidity, mortality, and clinical adverse events for critically ill patients. Twenty three studies met the criteria. Hypoxaemia was not consistently reported. No clinical practice recommendations could be drawn from the included studies due to methodological shortcomings.

This mixed methods, case study identified key patient, clinician and environmental factors associated with hospitalised patients’ ability and willingness to participate in their recovery after cardiac surgery. Patient participation is a significant component of the processes for achieving quality and safety outcomes. The findings inform redesign of the care delivery system to facilitate participation within acute care environments.

Patient safety is a global imperative aimed at reducing the incidence and impact of preventable adverse events in healthcare. This study has demonstrated how effective nursing surveillance and successful everyday nursing performance can bridge ‘gaps’ in the system of care, mitigate errors, and protect patients from suffering otherwise preventable harm.

Between 2002 and 2005, two high-profile Australian cases of nurse whistleblowing received widespread media attention. Little is known about the processes that influence nurse whistleblowing. This study investigated the nurse actions and highlighted the need to take seriously matters reported that pose a threat to patient safety and quality care.

 Getting a pressure injury or ulcer whilst in hospital may cause debilitating physical effects, pain or even death. This research found that assessment of mobility alone compares well with the more commonly used and more complex risk assessment scales when used to identify person's risk for developing a pressure sore.

 The main finding of the qualitative study is cultural beliefs significantly influenced diabetes self-care. The findings described how Chinese immigrants with diabetes living in Victoria managed diabetes self-care by rebuilding own liveable balance through implementation of several culture-related beliefs, attitudes and strategies to maintain their life balance after they were diagnosed with diabetes.

The purpose of the current study was to understand the processes Thai people receiving HD used to adapt to CKD and its treatment. Religion, spirituality, social support and self-management were powerful coping strategies. These coping strategies were strongly influenced by Thai culture and other belief systems.

 Transition to Specialty Practice Programs (TSPPs) provide structured support and education to novice nurses. This study examined the prevalence, design, characteristics and functions of emergency nursing TSPPs in Australia, and found that TSPPs served an important function in recruitment, preparation and retention of emergency nurses. A framework for future emergency nursing TSPPs was developed based on study findings.

BACKGROUND: Simulation is frequently being used as a learning and teaching resource for both undergraduate and postgraduate students, however reporting of the effectiveness of simulation particularly within the pharmacology context is scant. OBJECTIVES: The aim of this pilot study was to evaluate a filmed simulated pharmacological clinical scenario as a teaching resource in an undergraduate pharmacological unit. DESIGN: Pilot cross-sectional quantitative survey. SETTING: An Australian university. PARTICIPANTS: 32 undergraduate students completing a healthcare degree including nursing, midwifery, clinical science, health science, naturopathy, and osteopathy. METHODS: As a part of an undergraduate online pharmacology unit, students were required to watch a filmed simulated pharmacological clinical scenario. To evaluate student learning, a measurement instrument developed from Bloom's cognitive domains (knowledge, comprehension, application, analysis, synthesis and evaluation) was employed to assess pharmacological knowledge conceptualisation and knowledge application within the following fields: medication errors; medication adverse effects; medication interactions; and, general pharmacology. RESULTS: The majority of participants were enrolled in an undergraduate nursing or midwifery programme (72%). Results demonstrated that the majority of nursing and midwifery students (56.52%) found the teaching resource complementary or more useful compared to a lecture although less so compared to a tutorial. Students' self-assessment of learning according to Bloom's cognitive domains indicated that the filmed scenario was a valuable learning tool. Analysis of variance indicated that health science students reported higher levels of learning compared to midwifery and nursing. CONCLUSION: Students' self-report of the learning benefits of a filmed simulated clinical scenario as a teaching resource suggest enhanced critical thinking skills and knowledge conceptualisation regarding pharmacology, in addition to being useful and complementary to other teaching and learning methods.

 A qualitative research aimed to explore decision-making on birth choices following a caesarean delivery in Taiwan. Safety and risk management were the major influences for both Taiwanese women’s and obstetricians’ decisions. Biased information provision regarding birth options and over-medicalisation of the birth environment contributed to women seeking repeat caesarean delivery.
