The article reports on an increase which has been seen in the number of Australian hospitals that have decreased the number of shared patient rooms and have opted for single patient rooms in their ward designs. A discussion of support for single occupancy rooms which has been voiced by clinicians and patients, and of the role that single occupancy rooms can play in infection control, is presented.

One of the major challenges in healthcare wireless body area network (WBAN) applications is to control congestion. Unpredictable traffic load, many-to-one communication nature and limited bandwidth occupancy are among major reasons that can cause congestion in such applications. Congestion has negative impacts on the overall network performance such as packet losses, increasing end-to-end delay and wasting energy consumption due to a large number of retransmissions. In life-critical applications, any delay in transmitting vital signals may lead to death of a patient. Therefore, in order to enhance the network quality of service (QoS), developing a solution for congestion estimation and control is imperative. In this paper, we propose a new congestion detection and control protocol for remote monitoring of patients health status using WBANs. The proposed system is able to detect congestion by considering local information such as buffer capacity and node rate. In case of congestion, the proposed system differentiates between vital signals and assigns priorities to them based on their level of importance. As a result, the proposed approach provides a better quality of service for transmitting highly important vital signs.

AIMS AND OBJECTIVES: The aims are to (1) measure occupancy rates of single and shared rooms; (2) compare single room usage patterns and (3) explore the practice, rationale and decision-making processes associated with single rooms; across one Australian public health service.

BACKGROUND: There is a tendency in Australia and internationally to increase the proportion of single patient rooms in hospitals. To date there have been no Australian studies that investigate the use of single rooms in clinical practice.

DESIGN: This study used a sequential exploratory design with data collected in 2014.

METHODS: A descriptive survey was used to measure the use of single rooms across a two-week time frame. Semi-structured interviews were undertaken with occupancy decision-makers to explore the practices, rationale decision-making process associated with single-room allocation.

RESULTS: Total bed occupancy did not fall below 99·4% during the period of data collection. Infection control was the primary reason for patients to be allocated to a single room, however, the patterns varied according to ward type and single-room availability. For occupancy decision-makers, decisions about patient allocation was a complex and challenging process, influenced and complicated by numerous factors including occupancy rates, the infection status of the patient/s, funding and patient/family preference. Bed moves were common resulting from frequent re-evaluation of need.

CONCLUSION: Apart from infection control mandates, there was little tangible evidence to guide decision-making about single-room allocation. Further work is necessary to assist nurses in their decision-making.

RELEVANCE TO CLINICAL PRACTICE: There is a trend towards increasing the proportion of single rooms in new hospital builds. Coupled with the competing clinical demands for single room care, this study highlights the complexity of nursing decision-making about patient allocation to single rooms, an issue urgently requiring further attention.

In fragmented landscapes, a species' dispersal ability and response to habitat condition are key determinants of persistence. To understand the relative importance of dispersal and condition for survival of Nephrurus stellatus (Gekkonidae) in southern Australia, we surveyed 92 woodland remnants three times. This gecko favours early post-fire succession conditions so may be at risk of extinction in the long-unburnt agricultural landscape. Using N-mixture models, we compared the influence of four measures of isolation, patch area and two habitat variables on the abundance and occurrence of N. stellatus, while taking into account detection probability. Patch occupancy was high, despite the long-term absence of fire from most remnants. Distance to the nearest occupied site was the most informative measure of patch isolation, exhibiting a negative relationship with occupancy. Distance to a nearby conservation park had little influence, suggesting that mainland-island metapopulation dynamics are not important. Abundance and occurrence were positively related to %-cover of spinifex (Triodia), indicating that niche-related factors may also contribute to spatial dynamics. Patterns of patch occupancy imply that N. stellatus has a sequence of spatial dynamics across an isolation gradient, with patchy populations and source-sink dynamics when patches are within 300 m, metapopulations at intermediate isolation, and declining populations when patches are separated by >1-2 km. Considering the conservation needs of the community, habitat condition and connectivity may need to be improved before fire can be reintroduced to the landscape. We speculate that fire may interact with habitat degradation and isolation, increasing the risk of local extinctions.

Unprecedented global human population growth and rapid urbanization of rural and natural lands highlight the urgent need to integrate biodiversity conservation into planning for urban growth. A challenging question for applied ecologists to answer is: What pattern of urban growth meets future housing demand whilst minimizing impacts on biodiversity? We quantified the consequences for mammals of meeting future housing demand under different patterns of compact and dispersed urban growth in an urbanizing forested landscape in south-eastern Australia. Using empirical data, we predicted impacts on mammals of urban growth scenarios that varied in housing density (compact versus dispersed) and location of development for four target numbers of new dwellings. We predicted that compact developments (i.e. high-density housing) reduced up to 6% of the area of occupancy or abundance of five of the six mammal species examined. In contrast, dispersed developments (i.e. low-density housing) led to increased mammal abundance overall, although results varied between species: as dwellings increased, the abundance or occurrence of two species increased (up to ∼100%), one species showed no change, and three species declined (up to ∼39%). Two ground-dwelling mammal species (Antechinus stuartii and Rattus fuscipes) and a tree-dwelling species (Petaurus australis) were predicted to decline considerably under dispersed rather than compact development. The strongest negative effect of dispersed development was for Petaurus australis (a species more abundant in forested interiors) which exhibited up to a 39% reduction in abundance due to forest loss and an extended negative edge effect from urban settlements into adjacent forests. Synthesis and applications. Our findings demonstrate that, when aiming to meet demand for housing, any form of compact development (i.e. high-density housing) has fewer detrimental impacts on forest-dwelling mammals than dispersed development (i.e. low-density housing). This is because compact development concentrates the negative effects of housing into a small area whilst at the same time preserving large expanses of forests and the fauna they sustain. Landscape planning and urban growth policies must consider the trade-off between the intensity of the threat and area of sprawl when aiming to reduce urbanization impacts.
