Objective: To estimate Pharmaceutical Benefits Scheme (PBS) subsidies for drugs to treat smoking-related cardiovascular disease (CVD) in 2001-02, and over the period of the government's Intergenerational Report (IGR), assuming current smoking prevalence rates and a 5% absolute reduction.

Design and setting: An Australian epidemiological study, using prescribing data, aetiological fraction methodology, and IGR trends.

Main outcome measures: Estimated smoking-related PBS subsidy costs in 2001-02 and predicted cumulative subsidies until 2041-42, under current and reduced smoking prevalence assumptions.

Results: The PBS costs of smoking-related CVD in 2001-02 were $126 million, 9.77% of the cost of drugs for CVD and 2.96% of total PBS subsidies. The cumulative difference in these costs over the 40-year period with a 5% drop in smoking prevalence was predicted to be $4.5 billion, a 17% reduction. The saving would be $1.14 billion discounting future costs at 5% per year.

Conclusions: Further investment in tobacco control interventions could curb the increasing cost of the PBS and contribute to government efforts to ensure the viability of Australia's healthcare-financing programs. The net present value of a campaign to reduce smoking prevalence was estimated at $1 billion, with an internal rate of return of 33%.

This paper examines aspects of knowledge management that are particularly important in the network of human service delivery agencies in Victoria. This network is characterised by four features: it is a cluster of networked organisations; professionals and others may act as knowledge brokers within and between organisations in the network; rapid change in both knowledge and organisation accentuates the importance of innovative knowledge and emergent organisation over and above routine instrumental knowledge within stable organisation; and consequently there is an underlying concern with dialogical rather than instrumental knowledge and its management, and particularly how it constitutes and is constituted by organisation. The paper describes the analytical tools that we consider particularly important in examining this situation – in particular, the distinction between instrumental and dialogical knowledge, and the role of knowledge brokers (and professionals as knowledge brokers). It concludes by relating this analysis to broader issues in organisation studies, and suggests paths for further examination of these issues.

Early in 2001, after a damning public report by the Auditor-General, the Australian Federal Government was forced to abandon its highly promoted “whole of government” infrastructure outsourcing initiative. This about-face was greeted in the press with reports that the initiative was a “fiasco”. Yet a four-year case study of the initiative suggests a more complex picture. The initiative can be viewed in a quite different light on the basis of comparisons with a contemporary survey of 240 Australian organisations engaged in IT outsourcing. This reveals that many of the negative outcomes associated with this “fiasco” are typical of those experienced by large Australian organisations. This has important implications for decision makers confronted with choices about sourcing IT service delivery.

This paper examines the welfare and revenue effects of consumption and income taxes in a general equilibrium model with variable supply of labour and public goods. We derive the optimal consumption and income tax rates for such an economy. It is established that it is better to lower the income tax rate and increase the rate of consumption tax when this economy is in a downturn.

There is a need for more effective and efficient social behavioural change programs instigated by public sector environmental management organisations. The concepts of intrinsic motivation and extrinsic motivation have been shown to provide insights into the environmental behaviour of various groups. The aim of the paper is to investigate the relationship between Intrinsic Motivation, Extrinsic Motivation and Government influence. In particular this study examines the inter-relationship between intrinsic and extrinsic motivation in environmental management. A random sample of 566 land managers in South-eastern Australia was obtained. A structural equation model was used to develop a model which identities the relationship between intrinsic motivation, extrinsic motivation and government influence.

This article seeks to provide a brief overview of the current development of digital democracy in Australia, with emphasis on the use of the Internet to extend and enhance citizen participation. Use of the Internet within the definition of digital democracy proposed is categorized into three overlapping groups: (1) e-government services and administration; (2) participatory technologies; and (3) informal modes of participation.

The following article provides an introductory overview of Extensible Markup Language (XML) and how it may be applied to certain processes of e-government to improve accessibility to government services as well as to make governments’ services more widely available. In particular, this discussion explores how the kind of single-source framework used in digital publishing can be used to drive the storage, delivery, and exchange of e-government information and services. The first section of this article provides a basic introduction to XML. The second part illustrates how content is prepared and disseminated using XML, providing some existing and hypothetical examples of this single-source approach. The final section looks at future trends as well as some of the challenges that may be encountered when using this XML-based single-source framework.
