Airway surgery for obstructive sleep apnea and partial upper airway obstruction during sleep

This study analyzed the feasibility and efficacy of surgical therapies in patients with sleep-disordered breathing ranging from partial upper airway obstruction during sleep to severe obstructive sleep apnea syndrome. The surgical procedures evaluated were tracheostomy, laser-assisted uvulopalatoplasty (LUPP) and uvulopalatopharyngoplasty (UPPP) with laser or ultrasound scalpel. Obstructive sleep apnea and partial upper airway obstruction during sleep were measured with the static charge-sensitive bed (SCSB) and pulse oximeter. The patients with severe obstructive sleep apnea syndrome were treated with tracheostomy. Palatal surgery was performed only if the upper airway narrowing occurred exclusively at the soft palate level in patients with partial upper airway obstruction during sleep. The ultrasound scalpel technique was compared to laser-assisted UPPP. The efficacy of LUPP to reduce partial upper airway obstruction during sleep was assessed and histology of uvulopalatal specimen was compared to body fat distributional parameters and sleep study findings. Tracheostomy was effective therapy in severe obstructive sleep apnea. Partial upper airway obstruction and arterial oxyhemoglobin desaturation index during sleep decreased significantly after LUPP. The minimal retropalatal airway dimension increased and soft palate collapsibility decreased at the level where the velopharyngeal obstruction had occurred before the surgery. Ultrasound scalpel did not offer any significant benefits over the laser-assisted technique, except fewer postoperative haemorrhage events. The loose connective tissue as a manifestation of edema was the only histological finding showing correlation with partial upper airway obstruction parameters of SCSB. Tracheostomy remains a life-saving therapy and also long-term option when adherence to CPAP fails in patients with obstructive sleep apnea syndrome. LUPP effectively reduces partial upper airway obstruction during sleep provided that obstruction at the other levels than the soft palate and uvula were preoperatively excluded. Technically the ultrasound scalpel or laser surgeries are equal. In patients with partial upper airway obstruction the loose connective tissue is more important than fat accumulation in the soft palate. This supports the hypothesis that edema is a primary trigger for aggravation of upper airway narrowing during sleep at the soft palate level and evolution towards partial or complete upper airway obstruction during sleep.

Tietojärjestelmän käyttöönotto ja ylläpito

Tässä kandidaatintyössä tarkastellaan tietojärjestelmän käyttöönottoa ja ylläpitoa yleisesti sekä käyttöönottoa pilvipalveluna. Tietojärjestelmien käyttöönotto ja ylläpito ovat perinteisten elinkaarimallien viimeisiä vaiheita. Erityisesti järjestelmän käyttöönotto on hyvin kriittinen vaihe koko tietojärjestelmäprojektin onnistumisen kannalta. Käyttöönottoprosessi on monivaiheinen ja sisältää monia haasteita. Käyttöönottoon osallistuvien tahojen tulee sopia roolit etukäteen, jotta prosessi voi onnistua. Tietojärjestelmien ylläpidolla ajatellaan perinteisesti esille tulleiden virheiden korjaamista. Siihen kuuluu kuitenkin muitakin toimia, kuten järjestelmän kehittämistä, sopeuttamista ja ennalta ehkäiseviä toimenpiteitä. Ylläpito vie usein suuren osan koko järjestelmän budjetista, joten se tulee hoitaa järkevästi ja suunnitellusti. Työssä tutkitaan tietojärjestelmän käyttöönottoa pilvipalveluna käymällä lävitse pilvipalvelun käyttöönoton elinkaari sekä myös ERP -järjestelmän käyttöönottoprosessin eri vaiheet pilvipalveluna. Käyttöönoton elinkaaren ja käyttöönottoprosessin pohjalta havaitaan, että tietojärjestelmän käyttöönotto helpottuu ja nopeutuu pilvipalveluna, mutta käyttöönottoprosessi vaatii kuitenkin huolellista suunnittelua. Työssä perehdytään käyttöönoton yhteydessä tehtävään palvelutasosopimukseen ja siinä käsiteltäviin asioihin. Lisäksi sivutaan pilvipalveluiden käyttöönotossa huomioitavina asioina myös tietoturvallisuutta, tietosuojan lainsäädäntöä sekä vanhojen sovellusten toimivuutta pilviympäristössä.

THE IMPACT OF GUANXI TO MULTINATIONAL COMPANIES BUSINESS PERFORMACE, CORPORATE REPUTATION, NEGOTIATIONS AND MANAGEMENT IN CHINA

The modern business development of China began during 1978 and during the last decades Chinese have created one of the most dynamic and strongest economies in the world. China is now second largest trading power calculated in dollars. The special characteristic of this economic development is that it is not copy of any existing economy, instead the developments have been strongly influenced by the Chinese cultural characteristics. One cultural characteristic, guanxi, which is a “network of services and counter- services” is argued to be major component of successful business in China where these changes of services happen between people but also between companies. Obtaining introductions and the guanxi when doing business in China will give attentive audience and security for companies business. Despite the evidenced importance of guanxi, China´s business environment is rapidly changing towards Western economies, which might then reduce the importance of guanxi. Therefore the current impact of guanxi is a important topic to study. The main purpose of this study is to explore the impact of guanxi for Western big and small sized multinational companies´ business processes when doing business in China. This study looks What is the impact of guanxi in contemporary Chinese business environment for foreign companies doing business in China. Sub-questions in this research focus on what is the impact of guanxi on corporate reputation, management and negotiations. Findings present a mixed view where the importance of guanxi is not anymore critical, especially among younger people in the cities, however guanxi was clearly important outside the cities and when dealing with government officials. Efficient use of guanxi can be extremely profitable especially during early stages of business operations and guanxi plays role when bargaining prices and ensuring product quality from the factories. Therefore guanxi should be considered as essential element for successful business in China.
