Iowa's Child Death Review Report to the Governor and General Assembly Annual Report 2012

The goal of the Iowa Child Death Review Team is to identify those risks or factors in childhood (ages 17 and under) that result in fatal outcomes through a retrospective review of child death cases. A multidisciplinary team approach to reviewing child death cases is conducted. Recommendations made by the Team are based on data, which then are used to identify trends that require systemic solutions.

Children's Mental Health and Well-Being Work-group Final Report, December 15, 2015

The children's mental health and well-being work group was formed in response to legislative direction to facilitate a study and make recommendations regarding children's mental health and the systems that assist children and families in Iowa.

Family Support Subsidy and Children at Home Program SFY 2015, October 30, 2015

The Family Support Subsidy (FSS) program provides a monthly payment to help families with the cost of raising a child with a developmental disability. Parents of children with disabilities were very active in getting state and federal policy makers to look at how they could divert some of the funds going to institutional care. Families with severely disabled children wanted to raise their children at home but were met with a lot of resistance and policy barriers when they tried to get home-based support.

This report reflects one entire fiscal year of the Accountable Care Act changes. One of those changes was the method of how eligibility on January 2014, changing the income levels to 168 percent to 302 percent of the Federal Poverty Level.

This report was prepared pursuant to Senate File 505, which tasks the Office of the State Long-Term Care Ombudsman with convening a multiagency workgroup to gather information and provide recommendations for the establishment of a Health Consumer Ombudsman Alliance.

Health Care Power of Attorney FAQs

A health care power of attorney (HC-POA) is a document authorizing an attorney-in-fact (your designated agent) to make health care decisions on your behalf if you (the principal) are unable, in the judgment of your attending physician, to make health care decisions. Health care is defined as any care, treatment, service or procedure required to maintain, diagnose or treat a physical or mental condition. Through your HC-POA, you may authorize someone else to consent, refuse or withdraw consent to health care on your behalf.

Iowa Rural and Agricultural Health and Safety Resource Plan, 2011

The Iowa Department of Public Health (IDPH) convened the Health and Long-Term Care Advisory Council (HLTCAC) to assist in the development of its strategic plan. One component of the strategic plan is a rural health care resource plan. The intent of this document is to present reliable information and data as a valuable resource for the officials, agencies, and organizations responsible for strengthening and supporting the rural health systems vital to 43 percent of Iowa residents.

2012 Iowa Small Hospital Improvement Program (SHIP) Briefing and Survey Report, January 22, 2013

In August of 2012 the Iowa State Office of Rural Health (SORH) conducted a survey to determine the value of the Small Rural Hospital Improvement Program (SHIP) in Iowa. This survey was distributed to the 84 participating hospitals however, because some hospitals network their SHIP funds we only asked the contract administrators of the contracts to complete the survey. 58 of the 78 SHIP contract administrators completed the survey (74%). Background: SHIP brings in roughly $750,000.00 annually to Iowa to assist small Iowa hospitals. Average distribution of approximately $7,500 per hospital. Seventy three of ninety nine Iowa counties are represented.
