The Community Development and Health Network (CDHN) aim to end health inequalities using a community development approach - campaigning, influencing policy and developing best practice work which shows that communities, both geographical and of interest & identity, can define their own health needs and design and implement preventative and radical solutions. It believes that health is affected by more than access to health services, individual lifestyle choices and our own genetic make-up. These other factors can include poverty, the environment, education, living and working conditions, housing, access to food and social and community networks. The Building the Community-Pharmacy Partnership has worked to improve local health outcomes by encouraging members of the community to develop skills relating to managing their own health, and providing accessible information on the availability of services of which they can avail of. The aim is to facilitate local people to gain knowledge on various health issues through books, videos, leaflets and enable the most vulnerable and in need to access additional support through the pharmacy to complementary therapy. Outcome: A room in the pharmacy has been renovated and offers access to other support services. Funding has allowed the pharmacist to equip the room with videos, leaflets, books etc. There has been an increase in partnership working between the community group, pharmacist and more contact has been made with primary care. Further funding has allowed for the development of a subsidised support referral service for counselling and complementary therapies. As well as this, the pharmacist has worked with local groups to provide information sessions on lifestyle and on health issues identified by the local groups

This is their 3rd BCPP project and this is a Level 3 project. Rural Health Partnership is a community based initiative which assists people to recover from a mental illness. Previous BCPP projects focussed on supporting the needs of women in relation to mental health and more specifically, those experiencing post natal depression through the 2nd BCPP project using a lay health approach. This project seeks to build on this previous work. A very good working relationship has developed between the pharmacist and RHP. A programme of activities that can enhance the skills and knowledge base of the participants will be developed and so will relationships with other services eg GPs, primary care team etc. The project aims to educate the community at large on the issues faced by vulnerable women, with particular emphasis on symptoms of postnatal depression and anxiety.

This research was commissioned by Derry Well Woman and carried out on its behalf by the Institute of Public Health in Ireland in association with the Institute for Conflict Research and Rethink.The research had two distinct aims:- to improve understanding of the impact of the border and of the conflict on both sidesof the border on women’s health- to improve understanding of women’s roles, particularly as they impact on mental health, in post conflict society.- The research was conducted with a view to its recommendations being used to inform the work of the Cross Border Women’  Health Network as well as other cross border health forums or organisations responsible for service planning and delivery.- The findings of this research are based on a series of 31 in-depth interviews and one focus group with women both north and south of the border and on one focus group and six interviews with women who were specifically consulted as service providers.

This project will promote and introduce a healthier lifestyle for people over 50 through healthy eating, exercise and self-help in the Atticall and surrounding area. The project will focus on local produce and give participants an opportunity to cook and taste samples of their own 'healthy food'. The Pharmacist will attend these sessions to give diet realted talks and anwer any queries people may have.

This project will promote health awareness and encourage a more pro-active approach to looking after your health for12-16 year olds. Although focusing on young people, the approach being used hopes to cascade the information- the young people will take what their understanding into the home and the wider community. In order to reach their objectives they will carry out workshops with the youth in ways that will grab their attention and encourage a ‘change of lifestyles’ for example; painting murals relating to health issues etc. An information day was held as was an art project and several workshops, 1 of which allowed students to make a video on the role of the pharmacist. Results showed teenagers found it beneficial to get information on areas of health that they wanted to increase their awareness in. The project also identified a determination amongst staff to become more aware of the dangers of smoking and the project gave them a means of empowerment to help students who smoke to stop. All those involved in the project either through the information day, workshops or the art project felt it was tangible and was a sensitive wasy to deal with teenge health problems. The project created an understanding of why there is a need to take a communtiy response to such problems.

To enhance the health and well-being of members in the Craigavon and Cullyhanna communities. To identify and forge links between both community groups, Trust bodies etc, To offer members of both communities the opportunity to receive information and attend sessions on relevant health related issues. Outcomes - Pharmacists have introduced channels of communication between themselves, community groups and other statutory agencies. - Health benefits through information sessions have been delivered and tailored to meet the needs of local population groups. - The pharmacist has developed group working skills and how to link with others working outside the dispensary environment. - There has also been the opportunity to meet the needs of ethnic minorities in the Craigavon area

This project will build on previous work carried out over the last 3 years. There are 4 main aspects: - intergenerational work focusing on the development of a vegetable garden with pharmacy input on health advice - develop and promote the lunch club which has been very successful to date and has targeted people previously not accessing support or community services in the village - annual health fair - local carer’s advice centre to provide practice information and signposting services. This last aspect is the result of ongoing identification of the need to support carers over the last 2 years of the BCPP project. This project will be driven forward by a lay health worker working closely with the pharmacist Outcome: The project built on previous work. Some of the work included: health fayre, intergenerational garden, and luncheon club. Results from questionnaires showed that the project increased people's awareness of the causes of ill-health and how they might deal with them. People now see the pharmacist as a local and not just as someone dispensing medicine.

This has been one of the first opportunities to get health on the agenda in the rural area of Larne. This builds on the work and experience of a previously funded BCPP project in Larne town. This project will carry out information sessions in order to; facilitate discussion, disseminate information and identify relevant health issues. The project will mainly target women in the rural areas of the Larne Borough. Other aims of the project include; supporting local women to share health message to the wider community. This project will mainly be based in the pharmacy, with the pharmacist working with the women to identify how the can develop additional rooms in the pharmacy to meet local community health needs

To address diet and lifestyle issues and improve the health of the over 50s in Gilford. Outcome: Health related issues for example heart disease, osteoporosis and diabetes were raised and members became more aware of their health, lifestyle and specifically diet through individual consultations with pharmacist and nutritionist and through group sessions. The pharmacist has worked well with the GP and is broadening links with other support agencies eg CAB. The GP / Pharmacist relationship has strengthened.
