This delivery plan outlines the key steps that will be taken over the next three years to deliver the Governnment White Paper Choosing Health: making healthier choices easier. This delivery plan highlights how the DH and the NHS, within the framework of government policies, will help more people make more healthy choices and reduce health inequalities. It outlines clearly the priorities for delivery at national, regional and local levels and what will be done by whom and when. It brings into one place all of the actions on the White Paper commitments, alongside related Public Service Agreements and local targets to improve health. It lists 45 'big wins' - key interventions which the evidence and expert advice suggest will make the greatest impact on health in the shortest period of time It explains how new policies and programmes will be developed and implemented. It describes how Government will drive forward delivery through Government targets to improve health new partnerships

This report presents the findings of an evaluation of how the 12 pathfinder local authorities in the LGA/DH sponsored Shared Priority Project began engaging with new requirements to promote healthier communities and narrow health inequalities. The purpose of the report is to capture the learning from the pathfinder authorities' experience of this initial planning phase and share it more widely now that all local authorities have to focus on the shared priorities.

This factsheet on learning disability has been compiled by the Department of Health South East. It highlights the health inequalities experienced by those with learning disabilities and summarises the main health-related issues. It details key publications such as 'Valuing People Now', which was launched in Jan-09, and provides information about strategies, resources and national drivers including Local Area Agreements. It can serve as a useful tool for PCTs, commissioners, those involved in service development, and those with a public health remit in order to improve practice and health outcomes among those with learning disability at both local and regional levels. This collection contains a number of factsheets which have been produced by the Department of Health South East (the Department's presence in the region based in the Government Office for the South East) to promote understanding and awareness of key public health issues affecting the health of the population of the region. The factsheets are not policy documents, but are intended to promote good practice among the wider public health workforce in the NHS, Local Authorities and the voluntary and community sector. Each factsheet contains an overview of the extent and impact of the health issue, as well as a summary of the evidence-base regarding 'what works' to improve health.
