 Mid 1999 Investigation into cardiac surgery practice on children at Bristol Royal Infirmary. Dec 1999 Inquiry into organ retention in Alder Hey Children’s Hospital in Liverpool. May 1999: Contact from Our Lady’s Hosptial Crumlin re pathology and post mortem practice Dec 1999 PFJ meetings with Minister Apr 2000: Government Decision setting up Inquiry. Apr 2000: Ms Anne Dunne appointed Chairman. Feb 2001: Public Notice published, setting out terms of reference of Inquiry and inviting submissions. Mar 2001: Terms of Reference and Interpretation Published. Contact invited. “Six months N/A. Now 18 months” Jun 2001: Department commenced Discovery. Aug 2001: Memorandum on procedures received in Department. Sept 2002: 18 months time-frame had expired. Minister requested a meeting with Ms Anne Dunne. This happened in early September – Minister requested a progress report. 2 Oct 02 Ms Dunne’s progress report received by Minister – although substantial progress no definitive timeframe for completion of her work – a lot more work outstanding. Co-operation forthcoming from all parties 16 Oct 2002: Minister’s meeting with Parents for Justice – he assured them of his commitment to resolve any difficulties, but they decided to withdraw from the Inquiry process 2004: Minister calls on PFJ to co-operate. Chairman informs Minister that she has sufficient involvement of parents to conclude her report. Minister expects paediatric report by the end of the year

On being requested to complete an urgent report by the Minister for Health and Children the Authority undertook a preliminary review of the information known and drew up a series of questions for Our Ladyâ?Ts Hospital for Sick Children (OLHSC) in order to inform the writing of the report. The Minister for Health and Children established the Task Force on Sudden Cardiac Death (SCD) in the Autumn of 2004, with the following terms of reference:1) Define SCD and describe its incidence and underlying causes in Ireland.2) Advise on the detection and assessment of those at high risk of SCD and their relatives.3) Advise on the systematic assessment of those engaged in sports and exercise for risk of SCD.4) Advise on maximizing access to basic life support (BLS) and automated external defibrillators (AEDs) and on:- appropriate levels of training in BLS and use of AEDs, and on the maintenance of that training- priority individuals and priority groups for such training- geographic areas and functional locations of greatest need- best practice models of first responder scheme and public access defibrillation, and- integration of such training services.5) Advise on the establishment and maintenance of surveillance systems, including a registry of SCD and information systems to monitor risk assessment, and training and equipment programmes.6) Advise and make recommendations on other priority issues relevant to SCD in Ireland.7) Outline a plan for implementation and advise on monitoring the implementation of recommendations made in the Task Force’s report. In undertaking its work the Task Force was mindful of national health policy, relevant national strategies and of the recently reformed structures for health service delivery in Ireland. 14.10.2014 International Working Group Report on the Optimal All-Island Hospital Service for Cardiology and Cardiac Surgery for Congenital Heart Disease in the Republic of Ireland and Northern Ireland. The International Working Group (IWG) visited Belfast and Dublin from April 7- April 11, 2014. The IWG consisted of Dr. John Mayer, Dr. Adrian Moran, Dr. John Sinclair, and Dr. Patricia Hickey. This guidance is intended for use by organisers of events such as music festivals or agricultural shows where a temporary water supply may be required. It applies to all events that may require a connection to a new water supply as well as events that may require a connection to an existing supply, e.g. annual events taking place on the same showground.
