Briefing 9 - Understanding the economics of investments in the social determinants of health This document, commissioned by Public Health England, and written by the UCL Institute of Health Equity, examines how to use measures of economic investment to improve and increase local investment in the social determinants of health. The paper provides information to support decision-making on actions to address the social determinants of health and the development of business cases for investment. It supplements the evidence reviews in this series, which include information on the economic impacts of actions on health inequalities, and should help the reader to be an intelligent customer and commissioner of economic analyses and to understand their limitations. The paper covers: - The rationale for understanding, measuring and taking into account the economic impact of decisions and interventions that impact on the social determinants of health.- The benefits and limitations of various ‘economic measures of impact’ – commonly used terms which can be confusing, sometimes leading to misinterpretation of which measure of economic impact is appropriate for what purpose.- What is currently known about the economic impact of intervening in the social determinants of health.- Good practice and further resources which will support better decisions. Understanding socio-economic inequalities affecting older people

In 2012, CARDI was asked by The Office of the First Minister and Deputy First Minister in Northern Ireland to carry out a series of research projects on ageing in Ireland, North and South. This research project, 'Understanding socio-economic inequalities affecting older people’ , was carried out by Paul McGill, CARDI. The research sought to answer the following questions: Are there inequalities that affect older people as a group compared with younger people, or inequalities that exist within the older population? How are these inequalities changing over time? Do these socio-economic inequalities have a detrimental impact on older people or on a substantial number of them? How can any harmful socio-economic inequalities be reduced or eliminated and what are the implications for policy-making? Key Findings*: In RoI the poorest older people had a rise of €32 per week between 2004 and 2011 in total incomes while those with the highest incomes had a rise of €255 (CSO 2013). Total incomes of the poorest pensioner couples in NI did not change between 2003-06 and 2008-11 but the best off had a rise of �37 per week (DSD 2013). Employees aged 60+ earn €10,000 less per year than earners in their peak years in RoI and �2,400 less in NI (CSO Database and NISRA 2012). The richest older people in RoI earn 14 times more from employment than the poorest. In NI it is 36 times more for single pensioners and 44 times more for pensioner couples (CSO 2013; NISRA 2013). The gap in weekly earnings between top and bottom earners aged 60+ in NI rose from �294 to �430 between 2005 and 2012 (NISRA 2012). In the two years 2009-2011 the incomes of the poorest older people in ROI declined by €24 per week (11.4%) (CSO, 2013).

Cleaning up neighbourhoods - Environmental Problems and Service Provision in Deprived areas

There has been growing concern that the quality of public services can be affected by the nature and scale of problems in deprived neighbourhoods and that poor services can contribute to a widening gap۪ between deprived and non-deprived neighbourhoods. There is also an increased emphasis within national policy on the quality of neighbourhood environments the so-called liveability۪ agenda. This report explores the challenges of delivering street scene۪ environmental services such as street sweeping and refuse collection in deprived and less deprived areas and examines the gap in environmental amenity between these different neighbourhoods. It also contributes to our understanding of the interplay between poor services and neighbourhood decline. The research involved a telephone survey of chief officers in local authority environmental service departments across the UK and detailed case studies of policy and practice in environmental service provision in four local authorities with significant levels of deprivation. Each case study involved work in three neighbourhoods within the authority two deprived and one less deprived as well as focus groups with residents and frontline environmental operatives, interviews with senior council staff and observation on the ground
