In the framework of health services research sponsored by the Swiss National Science Foundation, a research was undertaken of the activity of the large majority of the public health nurses working in the Swiss cantons of Vaud and Fribourg (total population 700,000). During one week, 130 nurses gathered, with a specially devised instrument, data on 4165 patient visits. Studying the duration of the contacts, one has distinguished contact duration per se (DC), duration of the travel time preceding the contact (DD), and total duration in relation with the contact (DTC-addition of the first two). It was noted that the three durations increased significantly with patient age (as regard travel time, this is explained by the higher proportion of home visits in higher age groups, as compared with visits at a health center). Examined according to location of the visit, contact duration per se (without travel) is higher for visits at home and in nursing homes than for those taking place at a health center. Looked at in respect to the care given (technical care, or basic nursing care, or both simultaneously), our data show that the provision of basic nursing care (alone or with technical care) doubles contact duration (from 20 to 42-45'). The analyses according to patient age shows that, at an advanced age (beyond 80 years particularly), there is an important increase of the visits where both types of care are given. However, contact duration per se shows a significant raise with age only for the group "technical care only"; it can be demonstrated that this is due to the fact that older patients require more complex technical acts (e.g., bladder care, as compared with simpler acts such as injection). A model of the relationships between patient age and contact duration is proposed: it is because of the increase in the proportions of home visits, of visits including basic nursing care, and of more complex technical acts that older persons require more of the working time of public health nurses.

The significance of the insurance industry in the functioning of the world economy is often underestimated, with premiums reaching 7.5 % of world gross domestic product (GDP), three times as much as worldwide military expenses. Insurance services mutualise risks in such a way as they provide a form of private governance that complements or makes up for guarantees otherwise supplied by the State. This case study of international standards developed for the insurance market provides evidence that deviates from conventional accounts considering service standards as heavily dependent of national environments and industry specificities. The chapter examines the relationship between tertiarisation, internationalisation and standardisation of contemporary economies by highlighting the complementarity between institutionalist approaches of the French regulation school and international political economy scholarship shedding light on the polarisation in the possible use of standards, notwithstanding thesectoraland institutional specificities of the activities concerned.

Du cyclisme professionnel des années 1980-2000, on retient d'abord les scandales, sans connaître le rôle exact joué par la question du dopage dans la culture professionnelle des cyclistes. L'intention des auteurs n'est pas de dénoncé les coureurs consommateurs de pharmacologies ni de justifier le dopage mais d'adopter un regard sociologique sur une pratique constante dans ce milieu professionnel. Cet ouvrage s'intéresse aux acteurs et institutions qui en fixant les normes du dopage transforment les conditions d'exercice de ce métier, il montre les premières initiations, des premières compétitions à l'entrée dans le milieu professionnel. En s'appuyant sur les récits de cyclistes, les auteurs identifient les étapes d'une carrière autant physique que morale et montrent que la dynamique de la profession échappe aux cyclistes. Celle-ci dépend du cadre normatif imposé aux coureurs, de la structure du marché et des parcours individuels.

Introduction : Plusieurs études épidémiologiques et de laboratoire basées sur des estimations subjectives de la durée et de la qualité du sommeil suggèrent que celles-ci pourraient être associées à une augmentation du risque de troubles métaboliques ou cardiovasculaires. Objectif : Dans cette étude nous avons examiné les associations entre les caractéristiques du sommeil évaluées objectivement par Polysomnographie (PSG) et le syndrome métabolique ainsi que ses composants (hypertension, diabète, obésité). Matériel et méthodes : Nous avons analysé les données de 2162 sujets de la population générale (dont le 51.2% étaient des femmes, âge moyen : 58.4±11.1 ans, fourchette d'âge: 40.5-84.4) qui ont participé à l'étude Hypnolaus. Tous les sujets ont eu une évaluation clinique et biologique et ils ont bénéficié d'une PSG complète à domicile. Résultats : Les analyses univariées ont montré que les sujets présentant un syndrome métabolique avaient une diminution du temps total de sommeil, du sommeil lent profond, du sommeil paradoxal et de l'efficacité du sommeil, ainsi qu'une augmentation de l'index de microéveils par rapport aux sujets qui n'avaient pas un syndrome métabolique. Nous avons aussi trouvé des différences significatives de la structure du sommeil en fonction de la présence ou de l'absence d'hypertension, de diabètes et de surpoids/obésité. Cependant, ces différences s'atténuent après ajustement pour des facteurs confondants (âge, genre, tabagisme, prise d'alcool, activité physique, médicaments qui affectent le sommeil, dépression, santé globale et indice de masse corporelle). Seules des différences marginales, non statistiquement significatives, persistaient dans le modèle multiajusté et après stratification en fonction de la présence de troubles respiratoires au cours du sommeil. Conclusions: Dans cet échantillon de la population générale nous avons mis en évidence des associations significatives entre la structure du sommeil et le syndrome métabolique ainsi que ses composants. Cependant, ces associations ne sont pas indépendantes des autres facteurs de risque cardiométabolique connus. Nous en concluons que les variations normales de la durée et de la structure du sommeil contribuent peu ou pas au syndrome métabolique et ses troubles associés.

It presents the dynamics at work in 36 first division leagues in UEFA member countries. This edition covers our biggest ever survey comprising more than 520 clubs and 13,000 footballers. Statistical indicators relative to nine thematics (morphology, age, experience training, origin, etc.) allow the comparison of player profiles and squad compositions at league and club level. Through easily-understable regression analyses, the Study brings to light the principle differences between clubs and leagues according to economic and sporting level of championships.
